Mostrando los 31Employee was tightening down a ratchet strap while hauling a bundle of bolts when their hand slipped and made contact with the mass of the telehandler. At the time of the incident the employee reported it as a report only incident. Then days later turned into a reportable injury after employer was notified.
Employee tripped over a rock which caused EE's other knee to make contact with another rock causing a laceration.
Employee was bending over to pick up an extension cord when EE felt a pop in their knee. EE was placed on restrictions until further evaluations.
Employee was trying to remove a loader bucket tooth from the 136 conveyor belt. The employee stopped the belt and the tooth was under the belt magnet. When they grabbed the tooth, the magnet pulled their hand up with the tooth and smashed a finger.
Haul truck operator was traveling on roadway when they hit some potholes in the road causing the truck to bounce. Operator stated that their back was sore.
Employee was replacing the grizzly bars at 1448 when a rock fell from the apron feeder striking right hand.
The man basket of the TL642 telehandler became detached from the boom and fell 12' in 801-24 S7-P8 while loading of a cut was underway. Employee was in the basket at the time of the incident.
At 2:15 AM, 12/09/23, #26008 Getman A-64 ANFO rig reportedly caught fire while 2060E headings were being loaded. Source of fire located at on-board air compressor on ANFO rig. Reported that a hydraulic hose ruptured prior to the incident.
The employee was making repairs to #33010 Sandvik TH540 Haul Truck Air Conditioning system. During live testing, the employee's thumb came into contact with the condenser fan resulting in a fracture/laceration.
Around 7:00 am employee was dressed in street clothes and getting ready to walk to parking lot to go home after the night shift rotation had ended. As employee was walking out of change house and up the steps they felt a pop in their left knee. Could not bear any weight on left leg.
Employee was walking from service bay after parking haul truck and slipped and rolled an ankle.
Employees were leaving work area in 2250, when the driver of the tractor was getting out of the way for the 460 TH to pass. When re-entering the road, the tractor was in loose material. Upon breaking free, the tractor swung to the right contacting the rib. The employee seated on the same side, hit hard hat on the railing of the man carrier upon hitting the rib.
At approximately 8:30AM on 8/28/23, a mechanic was attempting to repair a blown turbo boot on the engine of the 37605 supply hauler truck. EE stood up on the offside fender stepping backwards and tripped over the exhaust guard bracket causing EE to fall backwards into the oscillating section of the supply hauler hitting face and ribs on the truck.
Employee reported that EE had twisted EE's left ankle while delivering grader tires to the shop for work to be completed.
An employee's right pinky finger was amputated at the tip while adjusting telehandler forks for an operator who was moving a load. The operator had tilted the forks back without warning before the employee had completed the adjustments.
Employee was checking hydraulic hoses on extra swing of rig when EE attempted to turn around EE's foot stayed planted, and EE's right knee twisted.
Employee states EE was working on the ball mill trying to fix an oil leak. EE went to leave and lost EE's balance and grabbed the handrail, EE then felt burning and sharp pain in EE's left shoulder area.
Employees were changing a sump pump. The pump was attached to the telehandler basket. While repositioning the pump, this caused the pump to fall, bouncing the employee around in the basket jarring EE.
At approximately 2:30 am blasting crew was loading heading in 1887 Upper. While handler was loading left side of cut rock came loose from top left corner of cut and fell and struck employee in left arm
Employee was working on a planned brake pad replacement on 21002 Locomotive at the car shop. While using a hammer to remove a pin, the hammer slipped off the intended target and came into contact with employee's hand.
Employee was operating the 23011 truck and struck the rib in the roadway. Employee states employee hit the rib as the truck was bouncing and would not stop. Employee was complaining of lower back pain, dizziness and vomiting.
Employee fell while washing out under a silo with a water hose. When the hose was turned on, the pressure made employee take a step backwards and employee tripped, landing on a step and felt pain in lower back.
An employee smashed their finger while installing rebar with a 4lb hammer. As the employee swung the hammer EE missed the rebar and EE's finger hit the side of the form board.
Crusher operator fell after EE's feet became entangled in crusher wash down hose. Employee felt a sharp pain and heard a pop in EE's ankle when EE fell.
Employee (mine supply hauler) was unloading grader tires off west side cage. While unloading the 2nd tire it spun around and pinned employees left leg against cage.
Employee was riding the man cage and dropped lunch box. When the employee went to pick it up, the handle was at an angle on the cage floor. As the employee went to grab the handle, EE's finger got cut from a sharp piece of metal from the cage floor.
Technician was removing a cone seal from the engine of 33010 Truck. When the seal broke loose, technician's hand nicked the corner of the block and cut EE's knuckle.
Operator was fueling up pipe rig. When the tank filled up it didn't shut off right away and diesel splashed up in operators face and on EE's arms. Diesel ran down behind operators safety glasses getting fuel into EE's eyes. Operator not knowing EE had diesel on arms, wiped eyes with sleeve.
The employee was getting off a tractor to move a piece of pipe out of the way for a telehandler. As the employee was getting off of the tractor when employee stepped off into some water, employees foot was half on rock and when EE put weight on it, EE's foot rolled outward injuring their ankle.
The blaster was in the process of tying in a 3 tall pod bench when a rock came out between 4 rib bolts roughly 8 to 9 off the ground sliding down and made contact with the back of employees heel. Employee received stitches to the bottom of the foot and had a dislocated toe.
