A roof fall was discovered in the return (no. 1 entry). The fall 18' in width, 40' in length and 7' in height. It was located 400' outby face area at spad 1082.

A ROOF FALL MEASURING 60' L X 40' W X 8' H WAS DISCOVERED ON THE ABANDONED NO.2 MAINS.
A roof fall was discovered at S.S. 128 and extending 70' inby No. 2 mains No. 7 entry.
Employee was tramming the roof bolter from the deck and had his foot hanging out of the bolter when he got his foot caught between the roof bolter and the rib in the number 5 entry.

A ROOF FALL WAS DISCOVERED IN THE INTERSECTION OF NO.3 HEADING ON THE 001 SECTION ONE CROSSCUT OUTBY FACE. SPAD NO. 916.
THE VICTIM WAS TRAVELING UNDER THE HUNG MINER CABLE WHEN HIS SHUTTLE CAR CAME INTO CONTACT WITH CABLE TEARING IT DOWN AND CUTTING HIM BEHIND THE LEFT EAR.
DUE TO HEAVY RAIN. FREEZING WEATHER, AND THAWING A ROCK FALL OCCURRED IN THE DRIFT OF THE NUMBER 1 ENTRY. THE ROCK MEASURED APPROXIMATELY 30' WIDE, 10' LONG, AND 6' HIGH. THIS FALL WAS REPORTE D TO MSHA IMMEDIATELY.
