While dismounting an endloader, Employee alleges that he slipped & fell- thus injuring his back. Management questions this accident. This mine was idled the week of Thanksgiving.

THE ROOF BOLTER OPERATOR SET ATRS. EMPLOYEE WALKED UP TO SWING BOOM OUT. ROCK FELL PAST LAST ROW OF BOLTS AND HIT CANOPY WHICH BROKE, CAUSING ROCK TO STRIKE EMPLOYEE IN THE BACK AND LEGS. (PERM. DISABILITY - SPINAL CORD INJURED, BUT NOT SEVERED; BROKEN BACK.)
Employee was bolting top on corner turn break. Rock fell out of corner hitting canopy and then hitting his right foot.
EE STEPPED INTO A WATER HOLE AND IN ORDER TO BALANCE HIS FOOTING, HE GRABBED A CABLE HUNG FROM THE TOP. THERE MUST HAVE BEEN A DAMAGED PLACE IN THE CABLE AND BECAUSE IT HAD POWER ON IT, HE RECEIVED AN ELECTRICAL BURN.
EMPLOYEE WAS LOADING ROOF BOLTER, CARRYING A BUNDLE OF PIN PLATES & HE TRIPPED OVER A ROCK, HURTING HIS LEFT KNEE & LOWER BACK.
Employee was climbing down off of a Caterpillar 992G and as he was climbing down he grabbed hold of a hose as a hand hold instead of the ladder. In doing so, he tore, possibly fractured, left shoulder.
EMPLOYEE WAS HELPING WORK ON THE ROOF BOLTER & GOT HIS RT. ARM PINNED BETWEEN ATRS & HYDRAULIC JACK.

EE was building and while carrying block across belts he tripped and fell on finger.
EE was told to service belt head, while greasing belt head the grease hose got caught in chain and pulled fingers in chain causing the amputation of the distal part of the 3rd digit to be lost at 1st joint and stitches repaired cuts on the 2nd and 4th digits of right hand.
EMPLOYEE WAS CUTTING A HOLE IN AN OLD PIECE OF BELT. THE KNIFE SLIPPED, CUTTING HIS RIGHT THUMB REQUIRING 17 STITCHES. EMPLOYEE NEVER LOST TIME.
EMPLOYEE HAD JUST FINISHED BOLTING #6 ENTRY. LOADED UP TRAYS FOR NEXT PLACE & WALKED BACK TO END OF BOLTER. HE TOOK HIS HAT OFF TO WIPE OFF SWEAT. ROCK FELL FROM TOP STRIKING EE ON TOP OF HIS HEAD CUTTING TOP OF HIS HEAD. ROCK FELL APPROX. 15' & WAS 6'X 8'.
Working on 1st floor of prep plant running a grinder and something went into left eye, had safety glasses on but still blew a foreign object into eye. he didn't think it was serious until the eye didn't get any better.
EMPLOYEE WAS WORKING OUTSIDE ON THE SUPPLY YARD, HE BENT DOWN TO PICK UP A TIE DOWN ROLLER ON A PALLET AND FELT SHARP PAIN IN HIS LOWER BACK.

MINER CABLE BLEW UP CAUSING FLASH BURNS TO EYES.
Roof fall on 8E Belt 1/2 break inby spad #29184. fall was approx 8' high 20' wide and 65' long. Bolted with 4' glue bolts and will be cleaned up. Fall id approx 2600 ft from #004 section.
WHILE STANDING ALONG THE CONTINUOUS MINER, HE WAS STRUCK ON THE CHIN BY A PIECE OF ROCK CAUSING A LACERATION REQUIRING FOUR SUTURES.
ROOF FALL ONE BRK INBY SPAD #28758 BRK 30 OF 8-C BELT. FALL WAS APPROX 50' X 20' X 6' THICK AND WILL BE CLEANED UP. FALL WAS BOLTED WITH 4' GLUE BOLTS AND IS 5,000 FEET FROM SECTION.
LOOSENING BOLTS TO TAKE A TRIPPER DRIVE OUT, USING A CHEATER PIPE, THE CHEATER PIPE, RATCHET AND SOCKET SLIPPED OFF OF A BOLT HEAD AND HE CUT THE BACK OF HIS RIGHT HAND ON GUARDING. HE LACERATED THE BACK OF HIS HAND AND RECEIVED SIX SUTURES TO CLOSE THE WOUND.
EE said he was bolting top and put too much pressure on drill steel. he stated that steel sprung out and hit his rt arm and thumb.
EE WAS CLIMBING INTO A PORTAL BUS - SLIPPED AND FELL, CAUSING PAIN IN HIS LOWER BACK.
EE WAS OPERATING A 10 SC JOY SHUTTLE CAR & ENCOUNTERED ROUGH ROADWAY CONDITIONS. JAMMED HIS HEAD IN CANOPY HURTING HIS NECK & BACK. EE HAD WORKED UP TO 2/21/05 THEN WENT OFF ON THIS ACCIDENT.
CLIMBING DOWN A LADDER, HE SLIPPED ON THE BOTTOM RUNG, CAUGHT HIMSELF WITH THE TENSAR ALONG THE RIB CAUSING A LACERATION TO THE RIGHT SMALL FINGER REQUIRING 3 SUTURES.
CUTTING A NYLON ROPE FROM THE SLOPE BELT, THE KNIFE SLIPPED CAUSING A LACERATION TO THE LEFT PALM REQUIRING 3 SUTURES.

Removing aftercoller fron engine, was in improper position for lifting while helping another mechanic, pulled muscle in back.
A friction caused methane ignition on the face of the longwall panel 49 near shield 144/ A blue and orange flame was observed as the shearer mined the fourth pass of the shift from the tail to the head. The flame lasted two to three seconds and was extinguished by the sprays on the shearer drum and spray bar.
Roof fall 7000-1-007-04: 1W #3 belt, x cut #88 to #89 diensions: h:25',w:19', l:45'. geologoy: soft, medium to dark gray shale with prominent roof jointing and slips in roof. roof support: 6' conventional bolts.
While walking alongside miner to help roof bolter a faceman put his hand on machine for balance. a 12"x8"x2" piece of rock fell and hit him on right hand.

Removing cover from a motor and, after removing bolts, pulled up on the cover. He felt pain in his left shoulder. INJURED CONTINUED TO WORK WITH NO LOST TIME UNTIL SEPTEMBER 30, 2004. He began losing time on that date.
THE ELEVATOR AT #6 PORTAL MALFUNCTIONED WHEN ONE OF THE BRAKES ON THE DRUM WOULD NOT RELEASE DUE TO THE BRAKE ARM BEING OUT OF ADJUSTMENT. THIS ELEVATOR IS PART OF THE PRIMARY ESCAPEWAY FROM THE MAINS WEST SECTION. ALL PEOPLE AFFECTED WERE INSTRUCTED TO USE ANOTHER ROUTE IN CASE OF EMERGENCY.
A FRICTION CAUSED IGNITION OCCURRED ON THE FACE OF LONGWALL PANEL 49 NEAR 143 SHIELD. THE OPERATOR STATED HE OBSERVED A BLUE AND ORANGE FLAME ABOUT 1.5 CUBIC FEET WRAP AROUND THE HEAD DRUM OF THE SHEARER FOLLOWING THE BITS INTO THE PANLINE. THE FLAME WAS REPORTED AS LASTING TWO TO THREE SECONDS AND WAS EXTINGUISHED BY THE DRUM SPRAYS AND SPRAY BAR.
STEPPING OVER CHAIN THAT WAS ANCHORED TO THE RIB AND BELT TAILPIECE AND TRIPPED. HE LANDED ON THE SIDE OF HIS RIGHT ANKLE CAUSING A SEVERE STRAIN.
A FRICTION CAUSED IGNITION OCCURRED ON THE FACE OF LONGWALL PANEL 49 WHEN A HORIZONTAL BOREHOLE WAS ENCOUNTERED AT 26 SHIELD. THE TAIL DRUM OPERATOR OBSERVED AN ORANGE AND BLUE FLAME COMING OUT OF THE BOREHOLE FOR A DISTANCE OF TWO TO THREE FEET AND THEN DOWN TO 12 INCHES. THE FLAME LASTED FIVE SECONDS. BOREHOLE WAS ABOUT 12" FROM DRAW ROCK THAT WAS BEING CUT.
While changing structure on the slope belt, beltman stepped backward onto a small piece of coal. He said he felt a "pop" in his right foot. He was diagnosed with a fracture.
