Employee was shoveling tailroller on the screen when employee experienced a sharp pain in lower back.

Employee was shoveling material on 04/16/14 when he felt pain in his abdomen and noticed swelling later due to a hernia. His initial nature of medical treatment was Treated & Released. He did not begin missing work until 06/10/14 due to surgery for the hernia.

Employee was sawing timber when the saw jumped, hitting his left index finger.

Employee was removing left rear hub assembly on a rock truck. While he was removing the bolts he placed his hand on the back of the impact wrench and once it engaged, his left hand on injured.

Mechanic was removing "belly pan" on 320 tractor type dozer. Upon inspection pan was "cockeyed" requiring the use if a hammer and wedge. During the removal process the wedge became dislodged and struck EE on left side of face under left eye. EE sustained a deep laceration and glue was applied.

Employee was making a splice in cable when his knife slipped, cutting his left hand.

Employee, who is a security officer, was salting the road into the mine when he slipped and fell.

Employee and witnesses were changing inline mixers on slurry line feeding #1 belt press. After completing the job, employee was cleaning up old pipe. When he lifted a piece of pipe that was 6" and approximately 4' long, he felt something tear in his left bicep.

A roof fall was found by EE on his weekly air way examination. The fall was found 1 break inby spad #1129 (3000 feet outby). The fall is approx. 55 feet long - 18 feet wide and 8 feet high. This mine is idle and has been since Nov-12.

Employee was retrieving nails from roof bolter when a piece of rib material, 6'L x 3'W x 12" thick, fell, striking him from behind, injuring his left knee.

Employee was moving section when the scoop tire got on his Left foot.

Employee alleges that on 01/31/13 his right knee "exploded" while walking and carrying something for an electrician. He did not complete an accident report. He notified his supervisor on 02/01/13 that he sought medical treatment. The worker's comp paperwork employee completed on 01/31/13 reports a DOI of 12/19/12.

Employee was servicing equipment. When he dismounted, he hyperextended his right knee. Employee finished his shift. EMPLOYEE DID NOT BEGIN MISSING ANY WORK UNTIL 01/21/13.

Roof fall-#4 Entry at spad #43. 20' long X 18' wide X 4' high.

Employee was stepping from pusharm of #320 dozer to track when his foot slipped and left knee came down on pusharm of dozer.

Employee stated that he missed the break going from #4 Entry to #5 Entry and hit the rib. The collision threw him into the shuttle car canopy, injuring his neck.

Employee was cutting belt for a belt splice when his knife slipped out of the belt, cutting the palm of his left hand.

Roof fall #1 return entry, spad #1383. Fall area 30 ft long, 20 ft wide, 4 ft thick.

Employee was operating scoop when a rock fell from the roof and struck him in the head.

Employee was changing transmission in #125 777F truck when the boom on the mechanic's truck broke striking employee.

Employee was in an area where the operator working the control box for beams could not see him. Employee was pinned between base frame brace and a beam that was being moved; was struck in the lower abdomen.

Employee was trying to loosen fitting on water block when he pulled a muscle in his left arm.

Employee was working on a piece of equipment when his foot slipped and scraped his shin.

Employee was climbing off grader and trying to stop door from closing when he twisted his left knee.

Employee was pulling and pushing on 3" solid water line when he felt a pain in his right calf muscle.

Employee was cutting belt to make a splice. Knife slipped and cut employee's right upper leg.

Employee was taking cutting edge off of 309 blade when he cut between his right index and middle finger.

The employee was in a bucket Bobcat when it went over the hillside.

Employee was in a mantrip that was sitting still when another mantrip ran into them.

Employee was taking a hydraulic hose off a track hoe, when the hydraulic hose guard fell, striking him in the head.

Employee was drilling on middle stockton pit. He went to dismount the drill on front and caught his left little finger on the mounting bolt to handrail.

Employee was taking down MRS cable to take outby when a piece of rock approx. 4" thick, 4" long, and 3" wide fell from top striking him in the back of his head, neck, shoulders and back, driving him to the ground. * Did not report prior because employee did not begin missing work until 01/06/2011.

Employee was standing on a rock truck ladder and reached out to clean off the headlights when his feet slipped out from the ladder causing him to fall to the ground, dislocating his right shoulder.

Driver was bent over hooking up the product tube on the back of the trailer when he felt a pain in his lower back

Employee was removing a bearing with a wedge when he mashed his finger between the hammer and bearing.

Employee was leaning against an oxygen dolly when it rolled forward. He fell to the ground and hit his shoulder on bottom. * Did not report prior because employee did not begin missing work until 01/05/2011.

Employee was drilling 4' hole with steel in top, when he removed drill head the steel came out & struck his hand.

Employee suffered a fatal heart attack while sitting in a chair in the security office. It is believed that the heart attack occured shortly after he began his security shift. He was alone at the time of his death. He was not involved in the mining process. He only provided weekend security coverage at the site.

EE moving 6" I Beam. Did not stop work until 4/22/10.

EE was doing engine oil change on D10 dozer while he was coming down off machine instead of coming down the approved 3 point of contact method he stepped down front words not using the hand rails and fell hurting his right arm.

Employee had installed a hose on 309 D11 dozer. When he was throwing the air hose off the dozer his ring finger went across the rain cap of the dozer causing his finger to get cut.

Employee was working on drill. He was pulling on a wrench and the wrench slipped off bolt and hit him in middle of upper lip causing a laceration.

Employee was loading a bulk truck. While on the top of the truck, he bent to go under a beam on the bin. In doing so, something in his back went out. He elected to seek medical attention.

Employee was helping change a battery on a man trip when battery lead arced and a spark struck employee in face & eye.

Employee stepped on rock on drill bench and twisted his ankle.

After replacing a 4" hose on a clean up pump two ee's were putting the grating over the sump. As the grating went into place the hose came off of the pump causing the water and mud to hit employee in face filling his eyes with dirt.
