Employee was kneeling on the ground, preparing to close bottom gate on railcar and heard EE's right knee pop. EE was in pain and wanted to go to the Dr to be checked out. EE is now waiting for an orthopedic Dr. to see EE.

The employee climbed over into a Gleason Clay Shredder to remove a piece of metal without locking the machine out. EE only killed the power. Another employee came by and started the machine. EE lost part of EE's left leg.

Employee was walking across platform to turn off belt conveyor and stepped on a hose that was lying on the platform.

Was disconnecting wiring from an auger that was no longer in use. Checked for voltage with pocket type voltage detector and no voltage was detected. I reached into the box to remove wires from disconnect and two of the wires touched and caused arc flash from where the wires were still energized. The arc flash caused 1st & 2nd degree burns to left hand & inner forearm.

Employee walked across truck scale to get to his equipment. As he stepped onto the scale, his foot slipped on ice and he fell on his left hip.

Employee had emptied dump truck of clay. He had gone around to back of truck to see that all the clay had dumped out. He took a step backwards, tripped over a lump of clay & fell on his left ankle.
