Checking the vent for an obstruction, but didn't have the fan off, and finger got hit by the fan blade.

Pulling conveyer belt and felt something pop in groin. Has to have Hernia operation.

Was pounding liner out of crusher cone and caught his finger between hammer and cone top. Severe laceration and top of finger broke.

Loosening bolt when wrench slipped, bent his right hand backwards and broke it.

He was installing a new screen. He was pulling and twisting and hurt his lower back.

Taking braces off double screen and was putting the bolts back on it and brace fell on his hand and cut it.

Welding all day and air arcing with auto darkening hood. Burned his right eye.

Employee was using a disc grinder and something got in his left eye. He was wearing safety glasses at the time of the accident. His eye continued to get worse and finally went to the Dr on the 7th.

Two oversized rocks were lodged in the jaws of the portable crusher. I managed a chain around one rock and had someone else pull it out. He then put his right toes in a pinch point for a step to reach the remote control. On grabbing the remote control, he absentmindedly switched the jaws on and caught his foot. His 4th and 5th toes on his right foot were fractured.

Employee was repairing a conveyor belt and was hammering a pin through a hole and hit his left hand with the hammer. He went to the ER and got stitches.

EE WAS CLIMBING INTO LOADER AT THE NIBLEY PIT. HE FELL BACKWARDS FROM LOADER AND LANDED ON HIS BACK, RIGHT HIP AND SHOULDER. HIS HEAD HIT LAST. EE WENT TO THE DOCTOR ON 11/20/04 AND HE HAS A BROKEN TAIL BONE.

Using crow bars to remove a peice of hard pan in the trap (at crusher) and hurt lower back.

Loading Metal chutes into the Bobcat when he smashed and cut his ring finger on the right hand

climbing into loader, hand slipped off hand rail, right foot wedged between step and bumper of loader

EE WAS SETTING UP CRUSHER & PUTTING IN PROTECTIVE RAILING WHEN IT SHIFTED, HE THEN FELL BREAKING HIS HEEL.

EE WAS ON CONVEYOR BELT TRYING TO LOOSEN A TURNBUCKLE, WHEN IT GAVE WAY HE LOST HIS BALANCE & FELL. HE LANDED ON HIS ANKLE & FRACTURED HIS LEFT HEEL.
