An outside truck driver came onto this site to get loaded where the driver then parked their truck and was out of view from out scale house. Driver began to work on the back of the truck where the air bag blew out from under the truck and hit the driver in the head. Driver was not our employee, nor was the driver supposed to be working on their truck.

Mostrando los 7An employee was fatally injured during maintenance work on the hopper. EE was operating a CAT forklift in close proximity to a Volvo excavator. EE exited the vehicle without notifying the other employees working in the area. Other employees stated they heard a "pop" and found the forklift operator unconscious next to the fork of the forklift.
The employee was last seen on top of the tower and determined to be missing. The employee was found in a pipe traveling from the cyclone discharge box to the thickener tank. Investigation has revealed that the employee traveled outside the provided safe access to the cyclone discharge box. It is not yet determined how the employee entered the pipe.
Sub-contracted tow truck was dispatched to tow super sucker truck. Boom of tow truck was being operated by tow truck employee #1 when it struck tow truck employee #2 in the head. Employee #2 was taken by ambulance to local ER for treatment where he later passed away. Most data unknown due to HIPPA and subcontractor status of deceased
On Monday 9/16/13 at 09:25a.m. a pit/haul truck driver, in truck number 22-2012, with 25 years, 42 weeks experience turned left and crashed through the protection berm and fell 80 feet to the pit floor
WHILE PERFORMING MAINTENANCE ON CONVEYOR BELT EMPLOYEE WAS PINNED BETWEEN WA500 KOMATSU LOADER AND CONVEYOR BELT LINE RESULTING IN HIS DEATH.
WHEN POSITIONING THE DRILL, THE RIGHT REAR LEVELING FOOT SANK INTO A MUD SEAM ALLOWING THE DRILL TO OVERTURN. THE DRILL SLID OVER THE EDGE OF HIGHWALL AND THE ROCK LEDGE CRUSHED THE DRILL OPER ATORS COMPARTMENT CAUSING DEATH OF DRILL OPERATOR.
