EE was changing screens at the washplant and was cutting a piece of rubber using a utility knife. During the cutting motion the knife slipped past the rubber and stabbed the EEs thumb.

Mostrando 50 de 466Highland city started to fill the canal earlier than what was communicated. When the canal was filled (pressurized) the valve that feeds water to the plant was in an open position to prevent water from freezing over the winter months. With the canal being pressurized and the valve in the open position, water started to flow into the pond causing the damn embankment to fail.
EE stepped on platform of an isolated catwalk that collapsed beneath them causing them to fall approximately 7 feet to the next level. EE sustained superficial lacerations and bruising/soreness.
While cleaning in the wash plant tunnel, employee slipped on aggregate material on the floor and hurt EE's knee. EE requested to be evaluated at the occupational clinic where they prescribed restrictions along with medication for the knee injury.
Employee was running through the plant area and felt a pop in EE's left hamstring area. Injury is believed to be a strained or pulled hamstring.
EE was rotating the sand screws manually by grabbing a hold of the V-belts, as the EE was rotating the sand screws the EE's middle and ring finger was caught between the V-belt and Sheeve causing a Turf Fracture to the tip of the ring finger.
EE was involved with installing v-belts when they became twisted. EE was manually rotating the v-belts on the pulley to straighten them when the pulley unexpectedly rotated causing the EE's finger to become trapped between the pulley and the v-belt.
Employee dropped EE's hard hat on the ground, picked it up and started walking and hit forehead on walkway support beam. Laceration to forehead.

Employee was attempting to rotate a conveyor tail pulley with a bar; the bar slipped from pulley causing employee to lunge forward hitting EE's chin on the side of conveyor frame.

While employee was cleaning under the wash plant with a mini skid steer EE backed into the H beam which forced EE's arm and wrist into the boom lift lever which injured the employees wrist.
While descending a fixed ladder on the plant the EE reached the lower level and misplaced their right foot on the edge of the concrete causing them roll their right ankle.
While two employees were working around a tail pulley of a conveyor belt, there was miscommunication between the guys on the ground and the plant operator. This resulted in the belt turning on and grabbing one of the employee's sleeves, pulling them into the tail pulley.
Employee was assisting with repairs to the dredge using a cutting tool and torch to remove a bolt. Bolt head was struck with sledgehammer causing bolt to shear and fragment penetrating employee in lower abdomen. Employee was taken to ER and transferred to another hospital where employee is currently having emergency surgery
The employee was walking around EE's service truck walking on uneven surface throwing away trash, flashing, and old bearing housing that were in the back of the service truck, when the employee felt a pop in EE's left knee.
Employees were working on replacing some pipe on the dredge and while using a pry bar to pry a gasket off, the bar slipped and hit the miner in the face causing a laceration right under the left eye which required a total of 12 stitches.
Worker was driving through a ditch and EE's leg became stuck under the chair of EE's vehicle. EE also bumped EE's head on ceiling of vehicle. EMS was called due to severity of leg injury.
EE was adjusting screen rail using a wrench. wrench slipped and hit them in the mouth. EE was referred to dentist for extraction and grafting.
While drilling the drill steel came out of the rack and fell into windshield. EE tried to catch the steel with their left arm fracturing left radius at the elbow.
Miner was working inside the Raw Mill on the seal air vertical pipe #3 roller. Employee was disconnecting the pipe and rolled the pipe around out of the way of the work. When they rolled the pipe, the pipe swung around so quickly that it pinned their left index finger between pipe and yoke of the #3 roller. The pipe mashed it at the nail and busted the end of the index finger.
Team member was replacing a 480V breaker inside switchgear with proper lockout. During this process, their screwdriver contacted a grounded surface causing an arc flash causing burns to face, arms, eyes and lungs.

Employee was washing the calciner area with a straight tip lance in the upward position. A piece of build up dislodged and hit the lance, jarring it and pushing it backwards into EE's left shoulder area. Causing a contusion.

There was a fire in the GAR facility. The employee was fighting the fire and suffered smoke inhalation. The employee went to the hospital and received Vitamin B IV. They were just getting over Pneumonia. The employee was released from the hospital after 2 hours.
Employee was assisting delivery drive in offloading conveyor to jack stand. Conveyor shifted unexpectedly and crushed employees left hand.
Injured was climbing down the ladder of a 980M, left foot was on the ground, the right foot got caught on the rung, causing the injured to fall backwards into a parked pickup truck. Suffered a shoulder sprain.
EE was standing on ladder to check progress of the Maintenance work being completed when a co-worker pulled out the screen unaware of EE's hand placement on the ladder. The screen caught EE's finger pulling back EE's left pointer finger nail
Employees was adjusting blades on sepol fan in Finish Mill 1. The team was using an impact wrench to loosen set bolts causing dust to fall from above where the team was working. Dust fell between the employees face and safety glasses.

Employee was using a plasma cutter on a fab table. The sparks ignited a spray paint can which shot flames causing a minor burn to their right arm.
Employee was closing a shale gate. The gate did not fully close and EE used the handle lever (detachable square tubing) to strike against the gate. In doing so, EE hit EE's hand on the gate fracturing the tip of EE's left ring finger. Also lacerated the finger the required stitches.
EE slipped and fell while climbing out of a tub of a sand screw. EE did not report any injury to supervisor until EE was in emergency room after EE had gone home after EE's shift.
