Mostrando los 16Miami County Highway truck pulled up to stone pile behind Rock Industries Hauler truck. Rock Industries EE was unaware that the County truck pulled in behind him and backed into the passenger side of the County Truck. No injuries occurred. The County truck had cab damage to the passenger side.
EE was lifting a piece of wood off of the hopper and when he turned the wood caught onto the hopper and he lost his balance and fell.
Tripped over backhoe and landed on his wrist. Had a sprain to his wrist.

EE WAS WALKING AROUND INSIDE SHOP AREA WHEN HE LOST HIS BALANCE. WHEN HE STOOD UP HE CUT HIS HEAD ON THE WINDOW WHICH REQUIRED 3 STAPLES.
EE WAS TRYING TO REMOVE A DOOR LATCH ON COMPANY WATER TRUCK WHILE USING A WRENCH. IT SLIPPED CAUSING HIM TO HIS HIS FOREARM ON THE INSIDE DOOR PANEL. THIS CAUSED A CUT REQUIRING SEVEN STITCHES
EMPLOYEE CUT LEFT THUMB WHILE EXAMINING A SNAD SHAKER WHICH REQUIRED EIGHT STITCHES.
EMPLOYEE WAS WORKING ON #5 BELT COMING OUT OF THE WHOLE. HE WAS ON THE CONVEYER AND TWISTED HIS KNEE, MAKING IT POP.
COLD, WET, AND MUDDY DRILLER TURNED AROUND TO WALK BACK TO PICKUP TRUCK AND RIGHT KNEE POPPED, THIS ACCIDENT BECAME REPORTABLE 2-14-00 WHEN HE WAS RESTRICTED FROM NORMAL DUTY.
