Operator of a 777C Rock truck came into contact with a berm located in the haul road and claimed pain in back.

Setting a transmission in a 777 - boom kicked out, extended boom so it would work then transmission started to swing and while trying to keep boom from swinging with right hand felt pain in right shoulder - strain to shoulder.

Employee pulled a muscle in his back while lifting his hood during pre shift inspection. Note: Filing solely to meet deadline regulations. Incident is under investigation.

EE was traveling down an embankment from Eagle creek #5 came into at right hand curve and turned over.

Operator trammed over a rock, causing dozer to shift. Causing pain in lower back.

Employee was investigating a truck fuel leak when his left knee contacted the turbo causing a burn. Employee did not report incident until 08/25/2014 and refused medical attention. Because of no medical attention or restricted duty, it was considered a non-reportable. Employee requested medical attention on 09/04/2014 and was taken to the doctor on 09/05/2014.

Mechanic and Foreman were attempting to change out a 992G tooth. While striking the tooth to free it, a shard of metal broke free from the hammer and struck the fueler in the neck.

Operator stated he had neck pain and numbness in his left arm.

Employee was cutting a limb from a tree when his saw kicked back and cut his hand.

Welder was working on tracks for a D10 dozer when a seal blew out of track pin and oil struck welder in the face

Miner was removing a sensor above his head when he felt a pain in his neck

Caterpillar D11 Dozer was engulfed in flames. No injuries to Operator

Parts runner was attempting to remove parts from bed of pickup and strained his shoulder.

Operator heard a loud pop from the engine compartment and the Cat D11T Dozer became engulfed in flames.

Mechanic was replacing hydraulic pump on bulk truck, pump slid off of pto shaft striking employee on right forearm requiring 6 stitches.

Operator was in a dozer pushing out a shot when he stated he felt a sharp pain in his back.

Operator was on a 773 water truck when the water load shifted truning it on its side. Operator recieved a 1" laceration to his forehead requiring 6 stitches

Foreman started a D11 dozer prior to the start of the shift to warm up the equipment for the operator. When he returned with the fueler, the equipment was in flames.

Operator slipped while dismounting and fell injuring his left shoulder

Employee felt chest pains prior to work activity, presumed heart attack by EMT personnel, but turned out to be excessively low blood sugar. Immediate reportable hotline called as precautionary measure and no dangers were found by MSHA investigation. Employee finished the shift

Mechanic replaced valve hoses of lifting jacks on rock truck. Lifting jacks on rock truck not completely filled with oil and released truck bed when the mechanic attempted to lower it.

Mechanic was climbing onto a Cat D11 Dozer when his foot slipped off of the foot step and he fell landing on his right foot slanted inward.

While opening the magazine door the top lock stuck EE pushed on the top of the door and bumped the bottom of the door with his left foot at the same time He then felt a pain(burning) in his lower abdomen/groin area and it began to swell The hospital released him back to work and had a checkup appt. on 9/1/11 at which it was determined to be a Hernia requiring surgery to repair

The driver involved in the incident suffered from an unforeseen medical condition. The driver was unconscious and does not recall the incident.

Fueler was removing fueling hose on a 992 loader when he felt a sharp pain in his left calf and felt pressure on it. He fell to the ground for a minute and then continued to work.

Operator was pushing with a Cat D9 dozer when smoke started from the engine compartment. Operator turned off machine, called for assistance from foreman. Both tried to extinguish the fire but could not. Local fire department called to extinguish fire.

Foreman was struck by a metal shard in the cheek and eye when aligning and hammering a hardbar pin on a dozer.

Cleaning out front idler, hit finger with slate bar.

Operator/Fueler was attempting to put antifreeze into equipment, when he missed the opening for the radiator and the antifreeze splashed on him.

Operator was operating a loader on the upper stockton coal pit, when he became light headed and hot. The operator stopped the loader and stepped out onto the cat walk to get some fresh air. At this time he briefly lost consciousness and fell, striking his lip and cheek on the railing. Operator was taken to Logan Hospital for Dr. evaluation.

Operator was climbing down a ladder on the side of his loader when he felt a strong pull in his leg. Pain continued and operator was transported to the hospital for an examination and Dr. review.

Operator was pulling coal from pit floor when the loader bucket came into contact with solid ground causing the operator to lunge forward. Operator complained of neck and back pain and was transported to the hospital for doctor evaluation.

Operator was pushing overburden on a drill bench over a spoil bank. The operator was backing the equipment up, when it struck a small rock ledge, causing a jarring motion to the equipment. The operator complained of minor discomfort and finished out the shift.

EMPLOYEE WAS INSTALLING BELLY PAN WHEN CHAIN SLIPPED. THE BELLY PAN MASHED HIS THUMB.

Employee tripped entering dozer and caught himself by grabbing handrail to keep from falling. He twisted his left arm.

Employee was tramming D10R on road over top of sediment ditch when he ran up on top of several rocks where he got stuck. When he started to back up, the D10R turned over on its right side into sediment ditch. Employee immediately, walked off the job, stopping at security guard shack to phone his wife. He left job, not reporting accident or if any injuries to his supervisor.

Jerked neck while taking a drink while tramming Excavator. He did not seek medical attention or miss work until 9/13/2008.

Using a hand grinder and it kicked back and struck him on his arm, 3" above his elbow. Laceration is 3" long. Did not cut into muscle.

Employee slipped off track of dozer while changing fuel filters, jarring lower back.

THE MAINTENANCE EMPLOYEE ATTEMPTED TO NOTIFY THE LOADER OPERATOR OF HIS APPROACH TO PERFORM MAINTENANCE ON THE LOADER. THE OPERATOR DID NOT HEAR HIM AND WHILE MAINTENANCE EMPLOYEE WAS CLIMBING UP THE LADDER OF LOADER, THE LOADER OPERATOR PULLED OUT CAUSING HIM TO LOSE HIS GRIP.

Berm gave way while dumping rock load causing truck to fall over 400 feet, ejecting driver.

Climbing off of left side of 992G Loader and fell from steps.

Employee was stepping down from grader. He said his right hand gave away and let him fall onto the corner of blade and circle of the grader.

Dropped off steep spoil bank in front of Loader - Dozer nose dived

Loader dumped bucket of rock into truck - knocking down overhead radio.

Operator was climbing down from D9 Dozer when he slipped off step, cutting left leg.

The driver was coming down the haul road loaded and started sliding. He cut to the hill side and went into a sump.

EE slipped and fell on drill bench injuring lower back. There were no witnesses to the accident.

Employee had gotten off dozer, attempted to go back up and got to the second step and fell to the ground.

Employee was changing filter on dozer. He went to throw filter on the ground, oil came out of the filter & on to the step, EE fell from the step to the ground.
