Employee was operating skid steer when emergency brake activated at speed causing employee to hit head on cross-member and causing a neck strain
While removing bowl assembly was nudging cylinder to make contact. Employee completed task 20 times, each time removing hand prior to pushing. He then rested hand on the rear of the cylinder. As it pushed, his finger was between it and another cylinder. It rotated pinching the end of the pinky finger. He lost the end 3/4" tip of his finger.
Employee was attempting to remove lodged debris from a feed bin with a steel bar. Employee was able to free some material and stepped away from the bin to allow additional material to exit the bin. Employee was resting hand on a piece when a stone tumbled from the bin and struck the employee on the left hand, resulting in a laceration to his finger.

EE was cleaning out the back of the pickup at behind the shop. Using his gloved hand he started to brush some trash and debris into a pile when he felt a stabbing pain in his right ring finger. X-rays revealed a sliver of glass that had to be removed by a hand specialist. Employee returned to work on restricted duty.
After operating a Caterpillar Articulating Haul Truck, EE began to complain of knee pain. EE has been diagnosed with a torn Meniscus and will undergo surgery to repair the injury.
EE alleges that while walking along catwalk, he slipped on loose material on the catwalk straining his knee.
While exiting a yard loader, EE slipped on snow and strained his knee.
