EE was lowering self down from the structure below the goodfellow screen. EE felt pain in the left shoulder. EE and the supervisor reported the next day. First aid was administered. On 02/20/2020 EE requested to see a doctor. Prescribed 2 RX Meds. and light duty until 02/26/2020. The incident became reportable then.

Mostrando los 35Employee was trying to connect a line with hydrated lime flowing through it. Employee failed to close the valve to stop the flow of material, which caused hydrated lime splash the employee in eye.
EE tripped and hit leg on retaining wall causing a contusion. First aid was provided. EE worked the next 5 shifts. On 1/22/20 EE requested to see a doctor. Cellulitis was diagnosed and antibiotics prescribed. Restricted from work for the remainder of the day.
Employee was retracting the chain on an overhead hoist when the chain snagged. The chain popped free causing the hook to strike the employee in the face causing lacerations to the nose and nasal fractures.
EE was working on the primary main hopper grizzly. EE believes metal dust fell into eye. EE rubbed the eye. EE was taken to the Optometrist. Doctor removed a metal flake from right eye. Antibiotic drops were prescribed. Doctor gave full release for return to work.
Employee was helping carry an oxygen cylinder when it slipped from their grasp and landed on their left foot just above the steel toe. Next day they couldn't put the safety boot on due to swelling.
Miner was reviewing prints for a bucket elevator and stepped backwards off of a raised foundation. While attempting to break the fall, EE's wrist was dislocated. Miner was taken to an urgent care facility.
Employee was working on a manlift crimping wire compression lugs with a battery operated crimper, tool slipped employee reached for it accidentally pressing the trigger with EE's left hand and getting EE's RT index finger caught in between causing a 1" laceration to the inside of EE's RT index finger.
Employee was removing a spray bar with right hand and it slipped and struck the left hand causing a laceration.
Company EE was loaded with product at the mine. EE attempted to close trailer tarp via cab control. Cab control did not work, EE exited cab to use trailer mounted tarp control when the swing arm broke from motor and struck EEs hard hat and left hand. EE was transported to local ER where sutures were applied to head.
EE was cleaning at the Primary when a rock came out the diester, fell and struck EE on the hard hat. EE was transported to medical and placed on restricted duty as a precaution.
EE was under dozer splice at Primary when a foreign object became lodged in left eye.
EE was washing material under C9 tail pulley with 1 1/4" water hose. The nozzle connected to the hose disconnected. EE felt pain in Rt shoulder. EE was transported to ER. In the waiting room, the EE reset the shoulder. EE told Supervisor they had a preexisting condition. This case is under investigation.
During inspection employee fell through a door (7-8 feet) causing fractures in left leg.
EE flushing a (lime) line with water. EE loosened the clamp on the hose, stepped to the side, tapped, & the line broke free spraying the EE with hydrated lime. EE was treated at the facility. EE requested professional advice on 5/11/2018 and as a precaution the EE was issued a prescription to avert potential infection.
Employees were removing a conveyor belt from a conveyor that had been locked out. An employee's gloved hand became caught between the pulley and the belt causing a tuft fracture of the left index finger.
Employee slipped and stopped self from falling by bracing self against the kiln wall. EE's left arm struck the kiln wall causing a fractured humerus.
The employee was struck by a hose. The employee received a minor abrasion to the bridge of EE's nose and an alleged concussion as a result of the incident.
ON 01/13/2018, EE was walking to mobile equipment and right leg became stuck in mud up to about shin. EE was offered professional medical treatment and declined. Instead treated leg with ice (first aid). EE requested professional medical treatment and was treated on 02/26/2018. EE was assigned restrictions by Medical.
EE was clearing a chute in the 40/70 tunnel and as EE struck the chute the bar EE was using bounced back and struck nose. EE was taken to medical and prescription for pain meds was issued.
The employee stepped out of the Skid steer ee was being trained on to use the bathroom. EE stepped on a slick surface and hit hard hat on the bucket of the skid steer. EE had a contusion on the back of head.
EE was operating a 745C truck,& was swerving for an 1/8 of a mile. As ee approached the top of the pit ramp, ee was seen by the oncoming operator to be slumped over in truck. As ee coasted down the ramp, the truck stopped as the front left tire came into contact with the toe of high wall. An operator put the truck in park, & woke ee. EE has low blood sugar.
While removing a screen deck from top deck of a Rotex the employee's right shoulder popped.
An All-Terrain Forklift was unloading a trailer of poly pipe. While the All-Terrain Forklift was traversing away from the trailer, a single piece of pipe fell on the driver's side of the truck.
The wet plant operator was changing screens on the screen deck, he had finished and was laying on his back moving back and forth to get out of the screen deck and tweeked his back.
Injury occurred while installing new shaker motor. Motor was rigged to a Skytrak that was equipped with special rigging arm. All workers noticed beforehand that Skytrak would start to shake due to angle load was in. Skytrak shook pinching ee hand in between motor being lowered and another motor that was already installed on roof. Pinch caused lacerations requiring stitches.
Wet Plant operator was using a pipe wrench to tighten the take up on BF1 at primary plant. While applying pressure the wrench slipped off and hit his middle finger against the frame of the conveyor. His supervisor was notified around 5:30pm. The operator was wearing gloves at the time of the incident.
The night maintenance person was walking to the NEW wet plant. One of our plant operators watched him fall, he ran over to him and then radioed that he had passed out. After asking him what happened he said he had not been eating or sleeping hardly at all due to a separation with his wife.
Worker started complaining of left arm numbness. He followed a manager up to the main office where his condition worsened to nausea, sweating, more numbness, so he was taken to the nearest hospital.
One of the Quality Control personnel while walking back into the Quality Control office tripped and fell on his right knee. I took him to the doctor and he was x-rayed. He was told to follow up with another doctor so we set up an appointment with concentra and we are waiting for his cat scan results to come back in. He is off work per the doctor.
I called MSHA as soon as I found out there was a fire that lasted for more than 30 min. MSHA did send a rep out to survey the incident. I just found out that I had to report this on a 7000-1.
I did not know that this was not filed until I checked on it 5/30/13. The miner was driving a haul truck over a rut causing the seat to raise up and the force caused the nut holding the seat belt in place to come loose. She immediately stopped, got help and fixed the seat belt. Then told us her shoulder was hurt.
The driver was getting into his water truck to and said he felt something pop in his stomach. He then walked over to a manager who was watching him and said that something popped in his stomach and the pain from it was getting worse. He was then taken to the emergency room.
The miner was shoveling sand since early morning and was working to clean up sand near a walkway and hurt his lower back.
Employee was walking to fill up a fuel tank and twisted his knee.
