(REPORTABLE DUE TO SUTURES): Employee was throwing an oil can for disposal at the end of the track on #6 Section when right hand middle finger was caught inside the spout of the can causing a laceration to right middle finger which required two (2) sutures.

While sitting bits on the H/G drum of the shear, a piece of scale rock fell out of the roof from in front of the shield and pushed / pinned them up against the drum on the shear. They did not start loosing time until 7/15/26.

Employee was walking up to the continuous miner, as they approached the miner they slipped and fell striking the right side of their face on the miner resulting in fracture to cheek bone.

Employee was operating a locomotive (LM43) on #4 Section (C-6) track at XC #42, when another motorman pulling a J-Loader on locomotive (LM85) lost control, slid backwards down the hill, and collided with EE's locomotive (LM43) causing two (2) fractured ribs and lacerations of the spleen.

A non-injury accident occurred on 1 East Belt at crosscut 16. The fall was above the anchorage bolts and measured 40' x 20' x 12'

Employee was performing maintenance on the 650-scoop battery while the battery lid was open. Employee began adding water to the battery; while performing the task the scoop battery lid shifted and unexpectedly and came down on EE's left arm

Employee was repairing a hose on the Skid Steer when cab came down striking EE on the hard hat and knocking EE to the ground hitting the back of EE's head on the concrete floor resulting in sutures to EE's head.

IGNITION - Occurred while doing repairs with an oxygen and acetylene torch in the 7/3 Service Shaft, approx. 1,153 feet down the shaft at 7 West. They ignited a bleeder which burned approx. 45 seconds and was extinguished with gallon container of water.

Employee was preparing to rock dust on #6 Section, while reaching for the rock dust hose EE's left hand was caught between the ram and the bucket of the scoop (c/n #161) causing a contusion/fracture to EE's 3rd & 4th metacarpals on EE's left hand.

Employee was attempting to widen the forks on a telehandler. During the process, one of the forks slipped from EE's hand and pinched EE's left hand between the fork and the mast, resulting in a laceration to EE's thumb and forefinger which required sutures.

A non-injury accident occurred involving 7-10 hoist, Employee put a drop deck supply car on the elevator and the car was mis-aligned on the elevator causing the draw head of the car to strike the west side of the shaft resulting in elevator to be removed from service for approx. 3 hours.

Employee was operating the Lo-Trac cleaning up material when they hit a jack, shifting a timber which fell striking the Lo-Trac roll cage. The timber had struck employees hard hat pushing hat down on ears causing laceration to right ear requiring in sutures.

Employee was moving pan line to C-5 and, while attempting to re-rail a wrecked car, the track jack dislodged from underneath the car and struck their left hand, resulting in two fractures and a laceration to left little (5th) finger.

Employee was in the process of changing out a discharge sprocket. While releasing a come-along, elbow accidentally hit the brace of the sprocket causing a laceration.

Employee was attempting to repair a broken cat track on the miner (CM132) in the #4 Entry on #6 Section, when a piece of rock fell out from the top of the rib and struck EE's right ankle causing a fracture to the medial malleolus bone in EE's right ankle.

REPORTABLE DUE TO SUTURES - Employee was working on an eight-inch Aqua-mine discharge pipe on NE-9 in the #1 Entry at XC #50 when it burst striking the employee on the left side of the body causing a laceration to the left ear and a laceration to the left upper arm which required sutures to both locations.

Employee was standing in front of scoop bucket dusting, as they finished and went to put the hose in scoop bucket they stepped back and had a foot under scoop bucket as it was lowered, catching their foot, causing fracture.

Employee was running the miner on #6 Section in the #4 LOXC, when a rock EE loaded on the car rolled off the coal in the shuttle car and struck EE's right foot causing a fracture to the top of EE's foot.

Employee was riding in manbus when EE's foot caught a rock, twisting EE's left knee and ankle resulting in sprain to left knee.

Employee was running the miner in the #1 entry on #4 Section when a piece of blue creek seam (rib) rolled out and struck them and knocked them to the ground causing a fractured left hip/leg initially, but after further medical observations other injuries were determined.

Injured was removing Shim Pack from Ventilation Fan, the Shim Pack unexpectedly shifted striking the injured on the left wrist.

(REPORTABLE DUE TO SUTURES) Employee was working on a 4" air line at 4-7 Fan compressor station when a 4" VIC coupling / bull hose blew off and struck EE causing a laceration to EE's left upper leg which required twelve (12) sutures.

The Aux Fan 217 trailing cable went phase to phase creating Fault/Arc in the cathead resulting in a flash/ignition from a small bleeder in the mine floor at 10 section.

Employee was pushing a whole frame back on a car where it had almost slid off, as they were pushing the frame they felt a pop in their right Bicep causing injury. Employee has been working since date of injury up until 1/20/26 when they had surgery on right Bicep.

Employee was standing in a crosscut while a group of miners were pulling belt together. During the pulling process, a rope accidentally broke and an object struck employee in the hand causing injury.

Employee was standing on a 4 foot ladder taking slack out of chain when they lifted the ventilation tubing, they lost balance and fell off the ladder injuring right knee and right ankle.

Employee was operating the tailgate shear when a rock came off the drum striking the employee in the face/nose resulting in fracture to nose.

Employee was changing bits in the miner, using a pick hammer to knock a bit loose, when a piece of metal flew off and struck them in the left arm/elbow causing the metal to insert the skin resulting in a laceration and surgery to remove the piece of metal. (Started losing time 10/28/2025)

Employee was cutting in the #1 Entry on #8 Section, EE went to the offside of the miner (CM130) to slide the tubing out and when attempting to walk back to the operator's side EE was struck by a shuttle car (SC125) due to limited visibility causing fractures in EE's right lower leg (fibula bone).

Employee was at the NW Track walking through a man door, as EE stepped through the door EE felt a pop in EE's right knee resulting in injury.

Employee was traveling on a man bus on C-5 Track, XC #29, and slid into another man bus when they struck lower legs on the seat that was flipped in the down position in front of them causing a contusion to left lower leg (unable to apply weight to stand/walk) and an abrasion to right lower leg. *STARTED LOSING TIME 8/25/2025*

Employee was entering the belt line and went to open large 4' man door, when lost their grip on the door causing it to close on right ring finger resulting in fracture to finger.

Employee had finished filling a tank for the rotary breaker oilers in the Prep Plant, when EE stepped off the platform and EE's right knee gave out and EE felt a pop causing a sprain to EE's right knee which later resulted in meniscus tears to the knee. (STARTED LOSING TIME ON 10/15/2025 DUE TO SURGERY)

Employee was riding in manbus toward the end of the track when the manbus lost traction and collided with parked bus at the end of the track. Employee felt pain in back. Employee has been working since the date of accident until taken off work as of 8/6/25.

Employee was lifting a cut 10"x7-1/2' long prop to set it as they were attempting to put it down they felt pain in lower back.

Employee was unloading tie down plates when EE lost EE's footing and fell, causing injury to EE's left knee and left ankle.

Employee was hanging blowing curtain in the #1 Entry on #4 Section when they stepped into an uneven area (rut) with some loose rock and twisted right knee and felt a pop causing a tibial plateau fracture in right knee.

Employee was walking outby on 4 West #1 Belt checking for splices around XC #112 when they tripped over a rock in the walkway going over the old overcast and fell into the walk side of the belt structure and top side roller and caught their right arm between the belt and the roller causing a fracture/abrasion to their right lower arm and a contusion to the right elbow.

Intersection #112 in the #5 entry of the 4 West N-Panel seal line, was an old fault area that had a mixture of 4' resin bolts and 8' cable bolts with mesh in some areas along with some straps. It fell out just above the anchorage point of the resin pins with some of the cable bolts still hanging showing they were still secured in good roof.

Employee was putting connecting links in face chain and hit EE's thumb with a hammer causing injury

The crew was in the process of forming up walls to pump concrete around the steel sets. Employee was evaluating the roof conditions around the steel set area and unexpectedly, a rock fell from the top area striking employee and causing injury.

Roof Fall - Occurred at the Northeast track, #5 entry and crosscut #31 intersection. The inby brow fell out above the anchorage bolts and the fall measured 15' length, 20' in width and 10' in height.

A Unplanned Roof Fall Occurred On NE-9 (#8 Section) At Crosscut Number 49 In The Long Crosscut Between The #2 Track Entry And The #3 Belt Entry. The Mine Roof Fell Above The Anchorage Point Of The Primary Roof Support. The Roof Fall Occurred Due To Undetected Separation With Laminations Above The Anchorage Point Of The Primary Roof Support Installed During Development.

Employee was trying to exit an air lock door on C-1 H/G, and when EE started to push on the outer door, EE felt a discomforting pain/buckled feeling in EE's left knee causing a sprain. After MRI and receiving results on 6/5/2025, EE had a torn meniscus and MPFL and EE started losing time on 6/6/2025.

Employee was helping install tubing inby the trunk line in the #1 entry of 12 section, when EE got EE's finger caught between 2 pieces of vent tubing resulting in stitches in L Hand finger.

Roof Fall - Occurred on the East Panel Track (East Main #4 entry) at x-cut 77, the old E6 track/belt overcast. The roof fall area measured approx. 15' long x 15' wide and fell above the anchorage bolt.

Employee was assisting with installing a bull hose on a 8" Aqua pipe for electric pump on NE-9, when employee lost balance and fell into the water and the 8" Aqua pipe fell on their left foot/ankle causing it to become caught between the mine floor and pipe, resulting in a sprain to their left foot/ankle.

Employee was riding in a manbus to the bottom, when a piece of draw rock fell out of the top striking employee on the left side of face/mouth, resulting in a broken tooth.

Employee was performing a pre-op inspection on the F250 pickup truck, and when they lifted the fire extinguisher out of the bed of the truck, they felt a pop in left shoulder causing a fracture to their left clavicle (collarbone).

Employee was lifting 10" aquamine pipe over the E22 return overcast to put the top piece on overcast when the pipe broke causing employee to fall approx. 8' to the mine floor resulting in fracture to right foot.
