Employee was operating a rock truck on 09/05/2025 and felt pain in lower back after hitting a pothole. EE went to the Emergency Room on 10/05/2025 and the CT Scan showed bulging disc. EE had a follow-up appointment on 10/13/2025 and they want Pre-Approval for PT and MRI. Employee began missing work on 10/08/2025 and estimated return to work is 11/24/2025.

Due to the freezing and thawing of the highwall a large chunk of rock approximately 4'x2'x3' came down striking the enclosed cab of the mobile drill, ripping the cab off and striking the employee.
The employee was mowing weeds with a Tractor and EE stepped out to use the restroom and bumped EE's knee on the step of the tractor causing a laceration in which EE had to go have stitches.
The injured and another were lifting a 3" water pump from the rear of the portal bus to the ground. As they lowered it to the ground the injured said they felt a pop and felt pain in their back.
Approximately at 4:20pm radio traffic was heard about smoke in the area of the idle D11 dozer. The agencies and Fire department where contacted. This D11 dozer has been idle for 2.5 weeks. No injuries.
Employee was pulling HWM cable with J hook and felt a pop in right elbow.

Employee was guard of Arms security Services and was contacted by radio at 6:00pm. At 8:00 pm an attempt was made to contact EE by radio EE did not respond. The night shift traveled to the guard shack to check on EE and found EE deceased in the guard shack
Employee was operating grader when hydraulic hose burst spraying fluid on turbo causing fire. Employee jumped out of grader to ground injuring
Employee was passenger of man bus when it ran up on mound of dirt and throttle stuck and hit rock.
Employee was driving bus to the pit when bus ran up on a hump causing fuel pedal to stick. Bus sped up stopping against rock.
Employee was backing a 773 truck down with a load of material to beef up berm and truck did not stop at berm but pushed through berm and descended down slope and landed on its side at lower road.

High voltage line broke, dropped to ground and caught grass and hills on fire.
Employee walked around mechanic truck to get bolts and stepped on unlevel ground causing them to twist an ankle.
At approximately 1:30am, the security guard was making rounds at the plant smelled a sulfur smell at the road above the clean coal silos. EE contacted the foreman who came and found a black steam coming out of the top vents of the #4 silo. After a plan approval the coal was removed from the silo through a stacking tube, spread out and water was applied to cool the heated coal.
Moving batteries from the deck of the drill to the truck, hurt low back
Employee was repairing ditch drain grating when foot fell into ditch injuring rt. knee. Employee continued to work until 11-23-21 where employee had surgery on rt. knee

Employee was standing 25' away watching blades of industrial mower be removed by pulling with a chain when chain broke and section of chain hit EE in lower Jaw.
Employee was dismounting fuel truck at end of shift and as employee stepped down employee felt shoulder pop. On 2-24-2021 employee was taken off work and scheduled to see a specialist and an MRI scheduled.
Employee was installing a splash guard on the clean coal screen when the guard rolled over and pinned employee's hand against the side causing a laceration that required 7 stitches.
Employee was filling water pump to prime pump and slipped and caught finger on clevis. Employee continued to work until 05/20/2020 where employee was taken off work and scheduled to see orthopedic surgeon.
Employee stated hip area started hurting while operating the end-loader.

The employee was going from the worksite to the plant with a load of coal when the driver came upon a rise in the road and they missed a gear at the top of the rise. The truck was then in neutral and they lost control on the down grade of the rise and the truck turned on its side. The gear shift struck the employee in the rib cage causing a possible fracture to rib.
Employee was climbing up step of rock truck and fell back onto ground.
Pulled a muscle in right shoulder while cleaning out dirt from the rod wiper on the drill.
Employee was climbing down ladder of Cat 785 Rock Truck. When they stepped down foot landed on lump of coal twisting left ankle.

Employee was pulling wire in conduit while standing on extension ladder in shop area when ladder slid down the wall causing them to fall injuring shins and right elbow. EE continued to work until 11-15-19 where they received an MRI and was schedule for surgery on 11-18-19 on right elbow and was taken off work.
EE was assisting in the removal of a pump skid from the pump building. EE was behind the pump operating the ratchet hoist when the pump moved and the suction pipe kicked backward striking their left knee. EE was seen at the ER, referred to ortho who released EE back to work. On 10-21-2019 Dr. took EE off work after an MRI reveled a meniscus tear in EE left knee.
Employee was bleeding airline on endloader and during the process employee strained neck.
EE was checking AC on Grader when EE reached to grab handrail and missed and fell off machine landing on left side injuring left elbow. EE continued to work until 08/25/19 where EE visited ER and was dx. with fx. of left elbow and referred to specialist on 08/30/19 where a full arm cast was applied to left arm.
Employee was helping other employees to change out a roller on a D10R Dozer. The old roller was lying on the track and was sharp from wear. Employee lost footing, which caused employee to place a hand on the old roller, resulting in a laceration to the left pinky which required 12 stitches.
Employee was removing laminated coal to expose coal seam with end loader bucket when they hit rock slinging them and injuring neck.
Employee was climbing down ladder of HWM Launch when EE's foot slipped off step causing EE to fall. EE caught self by holding onto handrail straining EE's right shoulder.
Employee was climbing off of Rock Truck and when employee touched the ground felt something pull in the left side of back.
Surveying pit, went to let bottom coal shot. As the employee was stepping down, the top edge gave out and their footing slid on the loose coal. Their knee popped and began to swell.
Employee was operating endloader when the machine jerked causing pain in neck and shoulder. Employee visited family chiropractor who ordered x-rays which revealed mild degenerative disk disease with mild levoscoliosis.
Operating CAT D10R dozer, pushing a big rock out of pit. Using corner of blade to spin the rock out to get behind it. Blade slipped off rock and slammed right shoulder into back of operator's seat.
On 12-7-2018, this mine was notified that this employee was diagnosed with Pneumoconiosis. Last day worked was 7/21/2015.
Loading a shot, walked to the next hole, right leg started to tingle, bent down to pick up a booster, strain to low back
Employee stated EE heard one of the back dual tires blow out. EE stopped to check and found a rock was in between the tires and had blown the outside tire. As EE got closer to the tires to inspect, the inner tire blew out and blew dust into EE's eyes.
Employee was changing oxygen sensor on company truck. When EE cut the clip off EE cut EE's right hand thumb with EE's knife requiring 3 stitches.
June 2nd, EE was working with another person on the bottom of a Cat 785B truck. The bed was turned upside down exposing the bottom side of the bed. EE was walking down the length of the bed & was stepping over of the bolsters. Due to moisture on the bed, EE's foot slipped & EE fell awkwardly backward on right leg.
Employee was helping lift Drill Pontoon into back of truck when they felt pain in the right side. Employee continued to work until 06-06-18 when family Dr. took them off work and scheduled them to see a specialist.
