Employee was standing on the first rung of a ladder while repairing/replacing a guard. Employee slipped off the ladder and landed on a knee. The knee made contact with the concrete pad. EE visited a physician on 09/23/21 and returned to work on light duty. On 09/30/21, the clinic notified us the EE is to be withheld from work pending the results of the MRI.
Customer O/Operator Truck Driver was driving from the AG Lime Stockpile to the scales and had a medical emergency. Customer was transported to a hospital. Customer passed away at the hospital.
While working on a tarp axle employee got debris in right eye.
Employee was installing driveline on Caterpillar 769 Haul truck, driveline fell on outside lower right leg above ankle.
Employee had cut a rubber mat with a razor knife. While picking up the mat, EE struck left thumb with the open razor knife cutting EE's thumb.
While entering the office building, the employee tripped on a boot scraper and fell forward contacting the door jamb/outside wall of the back door to the office with their left shoulder.
Miner was removing a ripper tooth by hitting it with a hammer when a piece of metal broke off and cut EE's leg.
On 02/19/2020 the EE lifted a 12 battery from office porch and felt a pop and pain in right elbow and shoulder. The EE was taken for medical treatment, x-rays were done and was diagnosed with a strain and returned to work. The EE had continuing discomfort but was unable to be seen due to COVID-19 restrictions. 04/01/2020 the EE was again seen and given injections.
Employee was lifting rocks out of a crusher and felt a pain in back. Employee was taken for a medical evaluation and x-rays and was diagnosed to have muscle strain and returned to work. On 02/07/2020 the employee reported continued pain was sent back for medical evaluation and 02/10/2020 was given an epidural and instructed to be off till 02/14/2020.
The accident occurred at the rotary feeder that had stopped rotating. When attempting to get the feeder rotating again, the employee pulled on the drive chain and subsequently got hand caught between the drive sprocket and the chain.
Employee was clearing a blockage in the oversize return chute from the inspection door when a large rock unexpectedly shifted smashing the right index finger between the rock and the inspection door frame .
Employee was removing old plate metal from a JCI screen chute for repairs using a cutting torch. Employee was wearing safety glasses and a face shield. While cutting the employee hit a pocket of dirt cause molten metal to splatter and blow out. A piece of splatter went under the employees face shield and safety glasses causing a burn to the employees right eye lid and cornea.
EE was removing the screw cover for the bag house screw. After removing the bolts the EE discovered there was a tack tac weld. The EE took a 6" angle grinder to remove the weld as the wheel contacted the weld it kicked back striking the EE in the Right thigh causing a laceration.
Employee was cleaning rocks off the back of a trailer and slipped and fell.

Friday morning 12/7, due to the cone screen stopping, the crew was working to get some of the material off the screen in order to reduce weight and restart it. While wearing cut resistant gloves EE started digging material off the screen. While performing the task EE caught the back of hand on a guard
