Employee was climbing down step ladder and slipped off second step causing EE to loose EE's balance, fell stumbled backwards and hit EE's head on the bumper of EE's truck causing a cut that required stitches.

Mostrando los 34Water pump was not pumping and employee attempted to opened the drain valve when the pipe that is attached to the valve broke causing hot water to spray out.
Employee was operating a DM45 Highwall Drill on a drill bench. Employee was traveling parallel to the edge and the outside edge gave way. The drill slid off the bench and landed 25 feet below.
Employee was entering cab of dozer and didn't see that floor board was missing. Employee's foot went in the hole and employee caught right side of rib cage against steering clutches.
A pry bar slipped while attempting to align a pin. The bar hit the employee in the chin and resulted in a minor laceration requiring stitches.

Operator was backing down EE's ramp, a Hyd. hose must have broke close to the turbo on the engine causing a fire. operator shut the dozer off and hit the fire suppression system and exited the dozer unharmed.
Employee was walking across parking lot and slipped on the ice landing on their shoulder. Continued to work until 2/24/22 when EE informed me of the accident and that it was not getting any better. I instructed EE to go to hospital and they found that EE had a cracked shoulder bone.
Employee was walking in parking lot and slipped on the ice. Employee fell on arm, continued to work until 3/16, elbow was not healing so they put employee on Restrictive duty.
Employee was working on removing a pin from a leveling cylinder on the back of the rock truck. While the employee was performing this work, the haul truck bed fell on top of the employee.
Employee stated that employee and another employee were removing a piece of 350 highline from a substation in the pit. When employee gave it one last hard pull employee felt a pop and ripping feeling in right elbow.
A roof fall was discovered at spad #112+40in #1 and#2 entry's in the return on North East Main #4. It fell in the intersection of both entry's and traveled out by in #2 entry to the next cross cut.
A roof fall at spad #106+10 in the intersection of #5 entry on the belt line. The area has been danger off.
The EE was putting a new feeder chain in the section feeder. EE was striking a link of the chain with a hammer to separate it from the chain. When EE struck the link it chipped a piece off and the piece of metal struck EE in the right jaw.
A roof fall occurred at spad #40+40 in #7, #7-8 and #8 entry's in the #2 North East Main intake. The area was previously bolted with 6 ft torque tension bolts and roof screen.
EE was climbing down a step ladder and turned to put EE's foot on ground and felt EE's knee pop.
EE was spraying De-icer onto the scalping tower belt. When EE turned off the nozzle the hose split spraying De-icer into EE's eyes.
The EE was using a cutting torch while working on a S/C conveyor chain. The torch slipped out of EE's hand. As EE went to pick it up EE's knee contacted the hose and spun the torch head causing it to go across EE's right thumb.
A roof fall at spad #39+00 in the intersection.The area was bolted with 6ft Torque tension and screen.It is contained in that intersection.
Employee was hooking up a suction hose to water pump and slipped in the mud causing employee to twist knee.
Roof fall located on our non producing MMU 002. At spad #16+40. Approximately 20 to 25ft in length by 18ft wide. This fall was in between the pillars.
The EE was cutting a notch into a section of belt so that a splice could be identified that needed replaced on a idle shift. The EE stated that as they were cutting the notch, the knife slipped and cut their left index finger.
EE was holding the end of a grease hose on to a grease fitting when grease shot out and went through EE's glove and into the end of EE's finger. EE received 2 stitches in the finger where the doctor lanced it open to clean the wound.
Operator was shoving dirt out with a D11T Bull Dozer and noticed smoke coming from engine compartment, EE jumped out and used the fire extinguisher on the fire but it was out of control so EE got off the machine and walked away.
EE was cutting a rock box off of an idle tail piece. As EE was adjusting position in the rock box the flame of the torch went across right hand. Leaving EE with a burn on right hand.
An examiner found a roof fall in the #5 entry. The fall was called into the MSHA hotline at 8:45 pm.
A roof fall was found in the #4 entry neutral of Unit 002 (Idle). The fall was reported to MSHA on 6-20-19
EE was changing oil on a rock truck and was retrieving a ladder from under the truck. EE picked up the ladder and turned and hit the side of their head on the bumper.
EE was carrying a piece of roof screen beside the roof bolter. EE lost footing and twisted knee.
Operator was loading trucks and noticed that high idle quit working then am/fm radio quit then CB quit, when they looked back they saw smoke coming from the engine compartment and tried to shut the machine off. The wire going to the engine shut off had burnt into also so they could not shut it down. Operator got off the machine and walked away
The EE was moving a full piece of 60lb rail to cut a section off of it. When EE slid it to the side it fell down smashing the end of EE's left middle finger.
EE was walking past a battery for a scoop. As EE slid a leg past the battery it contacted the ear on the battery, causing a small splinter of metal to go into EE's leg.
The EE was trouble shooting a hydraulic hose on the foot of the ATRS. EE had swung the off side bolter head out so that ee could get in between it and the ATRS. Another EE was coming up the side of the RB hanging cable and started the machine and swung the head in out of way to hang cable. Which in turn pinched the first EE in between the ATRS and the bolter head.
EE was standing on a D-11 Track pulling on a Hydraulic hose that is on a hose reel on our lube truck, when EE pulled on it the reel did not lock in place and it jerked the hose back and caused the EE to lose balance and slip and fall onto the push arm. Received 4 cracked ribs.
