Employee dropped EE's hard hat on the ground, picked it up and started walking and hit forehead on walkway support beam. Laceration to forehead.

Employee was attempting to rotate a conveyor tail pulley with a bar; the bar slipped from pulley causing employee to lunge forward hitting EE's chin on the side of conveyor frame.

An unplanned cave-in / release of material occurred underground during the second shift. During the second shift employees were loading shot rock and transporting to surface storage area when employees heard material falling and an accumulation of dust appeared in their area.

Employee was clearing blockage from a closed conveyor belt gate; employee attempted to clear blockage with hands, moving conveyor belt moved the blockage while employee had hand on it and hand became wedged between belt and blockage causing injury to fingers on left hand.

Employee was performing Pre-Shift inspection, checking fluid levels 0n 374F cat Excavator and mis-placed footing and fell. injured left shoulder catching themselves during the fall. Stepped off a narrow walkway That surrounds the inside of house into the hydraulic hose assemble area for the boom.

manlift basket was used improperly as a lifting device and the employee was pinched during the lifting process.

Employee was closing a shale gate. The gate did not fully close and EE used the handle lever (detachable square tubing) to strike against the gate. In doing so, EE hit EE's hand on the gate fracturing the tip of EE's left ring finger. Also lacerated the finger the required stitches.

WHILE EMPLOYEE WAS ASSISTING IN CLOSING THE SCREEN CHUTE GATE, EE PLACED RIGHT THUMB IN A PINCH POINT AND INJURED THE RIGHT THUMB. 3 STITCHES TO THE NAIL BED OF RIGHT THUMB.

Employee parked their personal vehicle in the scale house parking lot and slipped on ice and fell backwards as they were exiting their vehicle. Icy conditions contributed to injury.

Employee was performing belt Maintenance and lost EE's balance while on a 6ft step ladder and fell backwards to the ground. Right leg Fibula, Tibia, and ankle fractures.

Employee was stemming holes and when employee took a scoop of rock employee hit something hard in the stemming pile and hurt shoulder. I dug through the stemming pile and found a large rock.

Employee was installing new drive belts on a conveyor sheave. As employee was rolling the new belt on the sheave their right thumb was caught between sheave edge and belt causing a laceration to thumb.

Overheated material fell onto the clinker conveyor belts and caused the skirt rubbers to catch on fire. The resulting fire heat caused electrical damage to the e-stop causing the belts to stop. Employees tried extinguishing the fire with fire extinguishers. The employees called the fire department. No employees were injured, property damage will exceed $10,000.

Employee was cleaning inside of a Reed Gunite Pot and got left hand glove caught between feed wheel and rock shear. Severed tip of middle finger and index finger.

An employee went to take a scoop of stemming rock and pulled a muscle in upper right thigh.

Employee was attempting to move a metal portable folding explosive sign, when employee lifted the sign the legs folded together pinching the employees right middle finger.

Windy conditions stirred up dust and entered employee left eye. Left eye irritated that required medicated eye drops.

Employee slipped and fell on ice in parking lot and strained right shoulder while exiting ee personal vehicle.

Slipped and lost balance causing employee to stumble backwards and twist ankle.

Employee was retrieving equipment following a shot. Employee bent over to check a video camera before turning it off and felt an intense pain in employee's lower back. Employee reported pain while walking and getting in and out of vehicles.

Employee was struck by socket that came off an impact wrench while servicing equipment striking him on his front tooth

Removing rocks from cat walk on crusher, rock weighted around 150 lbs. pinky finger got between rock and handrail.

Adjusting and cleaning of a tail pulley on a 110' Masaba Conveyor LOTO was not performed by the EE before entering the tail pulley section of the conveyor. Belt was energized by the operator; EE was in tangled in the pulley from his feet to his mid section of his body. EE was fatally injured.

Employee was helping to lift a screen. Employee injured shoulder.

Employee was moving an impact hammer from a lower level to an upper level. Employee lifted the impact hammer and was unable to hold it and it dropped on his hand.

OVER A TWO MONTH TIME PERIOD THE EMPLOYEE'S ARM STARTED TO HURT. HE NOTIFIED US ON 9-8-03 AND THEN WENT TO A DOCTOR. UPPER ARM OR SHOULDER STRAIN.

EMPLOYEE WAS WALKING AROUND EQUIPMENT ON QUARRY LEDGE AND STEPPED ON A ROCK HURTING HIS LEFT KNEE.

TAKING WEDGES OUT OF HAMMER MILL, WAS HOLDING ON TO WEDGE WHILE OTHER EMPLOYEE HIT WEDGE. WEDGE DROPPED ON EMPLOYEE'S HAND.

TWO EMPLOYEES WERE MOVING A SCREEN WHEN IT SLIPPED OUT OF ONE'S HAND CUTTING HIS FINGER. HE NEEDED STITCHES ON HIS MIDDLE FINGER.

EE WAS USING A HAMMER WHEN HE HIT HIS FINGER. HEWAS WEARING TINTED SAFETY GLASSES IN A POORLY LIGHTED AREA. HE SUFFERED A CUT FINGER.

WALKING ON SOME ROCKS AND TWISTED KNEE. X-RAY WAS NORMAL AND EE RETURNED TO WORK MISSING ONE DAY.

EMPLOYEE WAS CLIMBING DOWN LADDER AND DIDN'T USETHE LAST COUPLE OF STEPS AND JUMPED TO THE GROUND.

EE WAS LIFTING A STEEL GRATE AND FELT A PAIN IN LOWER BACK.

THIS EE WAS LYING AN A CONVEYOR BELT/BIN, WATCHING EE CLEAN OUT ANOTHER BIN, THE EES POWERED UP THE BIN THEY HAD BE CLEANING WHEN THE CRUSHER OPERATOR HIT THE WRONG BUTTON CAUSING EE TO FALL O FF OF THE BELT TO THE GROUND. EE SUFFERED A COMPRESSED VERTEBRA IN THE MIDDLE BACK.

EE WAS WALKING AROUND THE DRILL AND HE SLIPPED ON THE MUDDY SURFACE. AS HE FELL HE TRIED TO BREAK HIS FALL WITH HIS HANDS. THIS CAUSED A FRACTURE TO HIS LEFT HAND.

THE FOREMAN WAS STEPPING OFF OF A LOADER THOUGHT HE TURNED HIS ANLE. CONTINUED TO WORK UNTIL HIS ANKLE BECAME PAINFUL.

EE WAS CLEANING OFF BELT WHEN IT STARTED CAUSINGHIS THUMB TO BE HIT.

EMPLOYEE WAS CLEANING OUT MUD IN AND AROUND THE TAIL PULLY AND THE GUARD WHEN HIS HAND SLIPPED AND HIS RIGHT FOREARM WAS CUT BY THE CORNER OF THE GAURD. PLANT WAS LOCKED OUT.

EE WAS WANTING TO GET DOWN TO A CONVEYOR BELT (5-6' BELOW). INSTEAD OF USING THE WALKWAY AND LADDER THAT WAS IN PLACE, HE DECIDED TO CLIMB DOWN THE FRONT OF THE CRUSHER WHEN HE SLIPPED, FALLIN G 5-6' TO THE CONVEYOR. THE EQUIPMENT WAS LOCKED OUT.
