EE was changing screens at the washplant and was cutting a piece of rubber using a utility knife. During the cutting motion the knife slipped past the rubber and stabbed the EEs thumb.

When the canal was filled (pressurized) the valve that feeds water to the plant was in an open position to prevent water from freezing over the winter months. With the canal being pressurized and the valve in the open position, water started to flow into the pond causing the damn embankment to fail.
While cleaning in the wash plant tunnel, employee slipped on aggregate material on the floor and hurt EE's knee. EE requested to be evaluated at the occupational clinic where they prescribed restrictions along with medication for the knee injury.
EE was rotating the sand screws manually by grabbing a hold of the V-belts, as the EE was rotating the sand screws the EE's middle and ring finger was caught between the V-belt and Sheeve causing a Turf Fracture to the tip of the ring finger.
Employee slipped on snowpack, fell from same level fracturing EE's right pinky when EE landed

While employee was cleaning under the wash plant with a mini skid steer EE backed into the H beam which forced EE's arm and wrist into the boom lift lever which injured the employees wrist.
The employee was walking around EE's service truck walking on uneven surface throwing away trash, flashing, and old bearing housing that were in the back of the service truck, when the employee felt a pop in EE's left knee.
EE was adjusting screen rail using a wrench. wrench slipped and hit them in the mouth. EE was referred to dentist for extraction and grafting.
Employee was changing the motor on feed conveyor, using grinder to cut off an old bolt when grinder shifted and made contact with left index finger
EE was using water truck to wet down area when loader operator who was back dragging roads backed into the water truck EE's chest struck the steering wheel. EE has some strained muscles in chest.
EE was standing on ladder to check progress of the Maintenance work being completed when a co-worker pulled out the screen unaware of EE's hand placement on the ladder. The screen caught EE's finger pulling back EE's left pointer finger nail
Employee was walking on the catwalk and tripped over their feet causing them to fall on the same level striking their right knee causing a contusion.
EE was Hammering a head pulley bearing Gear box on west screen feed, when a small sliver piece of metal broke off and lodged into the right side of EE's chin.

EE was hosing out bottom pan of sand screen using fire hose standing on A frame ladder. ladder came out from under them causing them to fall and strike left shin on ladder. left shin received stitches
The crew was installing mobile crusher. The crew was locking the machine out and to start unfolding catwalks. Half of the catwalk stuck open. When EE had made it up top of the machine, EE started changing screens without closing the catwalk first. When EE moved to the right, EE fell through the open part of the catwalk. Employee received closed fracture of two ribs on right side
EE, working for Sync Aggregate, was using a grinder and it kicked back and cut EE's left forearm. Taken to hospital by co-worker. Received stitches
EE was using a razor knife to cut a piece of hose to use as a piece of material to cover wiring. EE applied too much pressure and the blade went through the hose and into EE's left hand that EE was using to hold the material in place. EE received stitches to EE's LFT hand at the clinic and was released back to work with attention not to aggravate the same day.
EE was using marking paint to create a new berm that runs along the high wall. As the employee was marking the area along the high wall, this is when the EE fell from the high wall approximately 14 feet from the top of the high wall to the point where the EE laid at rest at the bottom of the high wall.
EE was cutting rubber mold when the knife slipped and cut EE's hand.

April 19, 2023, Employee was doing maintenance on a Hydraulic ram when the ram slipped when attaching the ram striking the employees left hand causing injury.
Fracture to 4th digit on right hand
Employee was walking to the maintenance truck to begin running welding leads to the work at the belt hopper. The employee tripped and fell on left hand, fracturing left wrist.
Employee cutting a hose when the the cutting blade slipped and cut the employee's left leg.
Employee removed a bolt from a wash plant pinching EE's finger causing a laceration to EE's right little finger requiring stitches.
EE was exiting surge tunnel and took off hard hat to scratch head and while turning, struck a piece of metal located above, causing a small laceration requiring 4 stitches to close.
EE was removing a bearing from a tail pully, and while striking the bearing race with a hammer, a tiny piece of fragmented steel came off, striking the EE just above the safety glasses in the eye brow. The fragmented piece embedded in the bone and had to be removed.
EE was assisting in prepping a machine for transport, and while cleaning off the decks, EE started feeling discomfort in EE's right eye. First Aid was given by washing out the eye with eye wash, but was unsuccessful. EE was brought to the local clinic in Twin falls, and the clinic had to remove a piece of gravel from the eye. EE was wearing safety glasses.
The employee was last seen on top of the tower and determined to be missing. The employee was found in a pipe traveling from the cyclone discharge box to the thickener tank. Investigation has revealed that the employee traveled outside the provided safe access to the cyclone discharge box. It is not yet determined how the employee entered the pipe.
EE was tasked to clean out a screen box adjacent to the #2 screen, that got overloaded with materials. As they finished, they climbed down the ladder after cleaning off screen component and felt light discomfort in the right knee.
Miner was using a oxy-acetylene torch when debris blew into an eye past the safety glasses
While cleaning screens the prybar slipped and smashed employee's finger (employee was wearing gloves) first aid was administered at scene but further aid was needed and employee was taken to Work Med
While removing an attachment to a piece of equipment the employees arm slipped and was cut on a sharp sliver of metal on the equipment.
Employee was attempting to perform fluid samples on machine and was needing to take the coolant sample. While the employee was attempting to pull the coolant sample, the employee placed hand on the hood to get off the machine and to hand the coolant sample to the operator and the hood fell down on top of the employee's leg and caused fracture in 2 places near ankle.
Employee was dismounting a conveyor frame and stepped onto a rock twisting EE's ankle.
As the miner was stepping off of a 980 CAT Loader,ee stepped onto uneven, Snow covered ground causing ee to fall and break left Ankle.

Another employee was assisting in bumping the belt to rotate the pulley, and the initial employee removing the belting got EE's arm pulled into the conveyor, causing significant injury to include fractures, and skin lacerations to the right arm.
EE was operating a wheeled loader when EE encountered a bump in the road and states EE bottomed out seat causing pain to lower back and neck.
Employee was using a 5# sledge hammer to pound on a steel pin and a shard of steel came off the pin striking the employee in the stomach.
Employee was descending a set of stairs to the scale house and as EE stepped to the second to last step, EE lost balance after stepping into a small indentation located on the step, this causing EE to fall forward down on EE's ankle. EE was brought to the clinic, treated for a right ankle sprain and x-rays showed a slight chip to the bone on the top of EE's right foot.
The KeyPlate wedges against the JawDie to hold it in place. A piece of angle iron was welded to the KeyPlate used as lifting eye. Using a come-along to remove the plate. The come-along applied tension to the plate the EE was hitting it with a hammer to jar it loose. Weld on the lifting eye broke, striking EE in the chin causing a cut requiring 6 stitches
Employee was repairing the lower step on front end loader. EE was using a utility knife to cut a section of conveyor belting to the proper size for repair. During the process of cutting the employee was using the utility knife in a drawing motion towards body. The knife slipped in the cutting process and in doing so created a puncture wound to the right thigh of the employee.
Employee was cutting conveyor belt with razor knife. The employee was cutting the belt perpendicular to himself and as the employee was cutting the belt, the knife slipped while applying pressure. Puncturing his lower right thigh. Resulting in 8 stitches to close.
Employee was forming urethane flashing for a belt conveyor with a hand grinder and started not feeling very good during the task. It was thought the employee may have been feeling ill from some of the fumes from the cutting process, but when brought to the clinic it was diagnosed with the flu and fumes may have contributed to accelerating his flu symptoms.
