While an employee was using a torch to thaw a water line, a Rock came off the plant and struck the employees right hand.

Employee was using a chipping motion with a bar to knock ice off of a screen deck prior to running. While doing this, employee's hand made contact with the plant causing a "right fifth metacarpal fracture"

Employee was using sledge hammer to knock a shaft out of a water pump for rebuild. While swinging the hammer, employee felt a pop in EE's left side.

DRIVER WEIGHED AT THE SCALES FOR DELIVERY AND WAS OVERWEIGHT. DRIVER PULLED OFF TO OUR DUMP OFF BIN TO DUMP SOME MATERIAL BEFORE BEING WEIGHED AGAIN. WHILE CLIMBING DOWN TO UNLOCK TAILGATE, DRIVER SLIPPED ON STEPS AND TRIED TO CATCH SELF ON THE MUFFLER COVER. THE SHARP EDGES ON THE MUFFLER COVER CUT RIGHT RING AND MIDDLE FINGERS.

Employee was pressing a sleeve out of crusher with a port a power. After pressing it out, employee began disassembling port a power. During this time the steel cross bar that was holding the port a power fell and struck EE's finger.

Employee reached up through a guard to check belts shortly after the machine turned off. The machine was still in motion and the belts took the employee's fingers through the pulley.

Employee was walking to car at the end of day when employee lost balance and fell.

Employee was loosening bolt on a screen. While trying to keep the nut secure, the nut turned and pinched EE's thumb resulting in a small laceration.

Employee was performing maintenance to an articulating truck. As they were loosening the caliper bolts, employee was pushing down on a breaker bar. The bolt came loose and the employee fell forward into the truck, causing a laceration to the eye.

Employee was getting bag out of Pickup truck. Employee stepped on tire to reach bag. Upon lowering back to the ground, the employees leg was injured.

Employee was assisting repair in a conveyor belt that broke. While untwisting the belt a rock that was twisted up in the belt came loose and fell, bounced off the beam of the take up pulley and struck the employee.

Performing engine repair on 992C Wheel Loader. Cleaning air filter with sanding pad attachment. Piece of metal flew into Left eye. Eye became irritated next day. Given Rx and referred to ophthalmologist 7/13.

While climbing down a ladder on the plant, the employee slipped and fell into the ladder on the right side.

Employee was unloading a box of welding rods from a truck. The box slipped and while trying to maintain control of the box the employee felt a pull in their shoulder/arm.

Employee was using a cutting wheel to cut a bearing out of a crusher. While cutting the bearing the wheel kicked back and cut the employees right arm.

Employee(#1) was installing an accumulator on a CAT 980H Loader with the assistance of a Quarry employee (#2). #2 was holding accumulator while #1 was removing bolts. #1 saw #2 struggling with holding up the part and came down to assist. When #1 took hold of part, #2 let go of part and the unexpected transfer of weight caused #1 to strain upper back muscles.

Employee was cleaning out a feeder shoot on the plant. While clearing out a tree root, employee brushed up against the shoot and cut arm.

Employee was carrying a container of welding rods and cut fingers on the container.

Employee fell from a ladder on the plant at the remco 3000 crusher at the back-end.

After employee finished fueling the drill, employee was putting hoses away. While walking to the truck to hang the nozzle, employee tripped over track marks left in the ground from the drill.

Employee slipped on ice while climbing down from a loader. EE caught self with EE's arm and felt a pull in EE's shoulder.

Employee was climbing down a ladder after making repairs to a 992D Loader, when the ladder slipped out from underneath him causing him to fall. while falling he made contact with the tire and injured his back/neck.

Employee was climbing Ladder of haul truck when the employee felt his knee give out.

Employee was dusting off the windshield of a CAT 773 Haul Truck with a rag prior to his shift. While dusting it, his hand swung down and hit the bar below the windshield. (this bar is used for climbing up the truck) This resulted in a dislocated pinky finger on his right hand.

Employee was pulling the torque wrench while standing on a set of stairs. As the employee was pulling, hit weight shifted causing the stairs to slide. The employee's knee twisted to the right and his body fell to the left. His leg went forward and hit the frame of the truck.

The maintenance crew was moving a conveyor up the ramp to the top of the tunnel stockpile area. A Kobelco LK 300A wheel loader was used to move the conveyor up the ramp. The chain used to lift the one end slipped through the eye in the bucket, causing the loader to overturn on the employee who was walking on the left side of the machine.

The truck owner was replacing a bolt in a pony axle. The owner had a bottle jack under one axle tire and a wooden block under the axle tire beside it. The owner then lowered the axles from the cab, positioned himself on a creeper between the 2 raised axle tires. The jack came out from under one axle tire; crushing the owner.

EE WAS DISASSEMBLING THE FMC MF800 VIBRATORY FEEDER. THE EE WAS INT HE PROCESS OF REMOVING THE FASTNERS FOR THE SECOND COUNTERWEIGHT WHEN THE FIRST COUNTERWEIGHT SPUN ON THE SHAFT. THE EE S RI GHT THUMB WAS PINCHED BETWEEN THE TWO COUNTERWEIGHTS.

EMPLOYEE WAS WORKING WITH EMET GUTIERREZ CHANGING A HOIST CABLE ON DB-04C AN 80D NORTHWEST CRANE. THE EMPLOYEE WAS ATTEMPTING TO HOLD THE 1 1/8"CABLE WHILE IT WAS BEING INSTALLED IN THE HOIST DRUM OF THE CRANE. IT CAME FREE STRIKING THE EE IN THE FACE.

EMPLOYEE WAS EXITING THE CAB OF THE FUEL TRUCK FT-03C, WITH AN UNPROTECTED RAZOR BLADE IN HIS RIGHT HAND. AS HE BEGAN TO CLIMB OUT OF THE CAB, HIS RIGHT HAND STRUCK HIS LEFT HAND WITH THE RAZO R BLADE.

EMPLOYEE WAS REMOVEING A 4 LB. HAMMER WHICH WAS USED AS A BLOCKING DEVICE ON THE 6'X14' SCREEN DECK. THE EMPLOYEE'S LEFT MIDDLE FINGER WAS PINCHED BETWEEN THE SCREEN DECK FRAME AND THE HAMMER HANDLE.

EE WAS RECEIVING HIS OPERATOR TRAINING ON THE WATER TRUCK. THE ACCIDENT OCCURRED NEAT THE DUST COLLECTOR AND 21A PILE INTERSECTION. INSTRUCTOR WAS WEARING THE CAB SEAT BELT AND EE WAS WEARING A SAFETY HARNESS TIED OFF TO THE SEAT FRAME AND THE ROPS CAB. THE TRUCK STOPPED AT THE INTERSECTION. WHEN THE TRUCK RESUMED ITS TRIP, IT ENTERED A DIP IN THE ROAD. EE WAS THROWN FROM CAB AND

WHEN EE BACKED THE TRUCK UP TO THE WHEEL LOADER BUCKET, SHE HAD HER HANDS ON THE STEERING WHEEL AT THE 0900 AND 1200 POSITIONS. WHEN THE STEERING WHEEL SUDDENLY PULLED OUT OF HER HAND, HER RIG HT HAND BECAME LODGE BETWEEN THE STEERING WHEEL AND THE HAND VALVE CONTROL.

EE WAS INSTALLING A PLANETARY DRIVE AXLE IN L-25L A 980C CATERPILLAR WHEEL LOADER. HE WAS USING HIS RIGHT HAND TO PUSH THE AXLE IN WHEN IT MOVED SUDDENLY. THE PALM OF HIS RIGHT HAND WAS PINCHE D BETWEEN THE SUN GEAR AND THE AXLE SNAPRING.

EE WAS WORKING WITH OTHER CREW MEMBERS REMOVING A BLOCKAGE AT THE 8 BELT TRANSFER POINT. THE EE WAS ATTEMPTING TO AMANUALLY OPERATE THE 8 CONVEYOR BY TURNING THE MOTOR V BELTS. EE STARTED THE 8 CONVEYOR BELT FROM THE POWER SWITCH LOCATED ATTHE BASE OF THE 2400 AC CRUSHER.

EMPOLOYEE WAS DISMOUNTING FROM THE RIGHT SIDE OF DZ-37C. A D-7H CATERPILLER BULLDOZER, SUING THREE POINTS OF CONTACT. AS HE PLACED HIS RIGHT FOOT ON THE BLADE ARM STEP, HISS FOOT SLIPPED CAUS ING HIS RIGHT KNEE TO BE WEDGEED BETWEEN THE BLADE ARM AND THE TRACK FRAME.

EE WAS WORKING WITH OTHER CREW MEMEBERS REMOVINGTHE GRIZZLY FROM THE PUGMILL TRANSFER BOX. THE 6000 LB GRIZZLY WAS LIFTED STRAIGHT UP BY THE CRANE OPERATOR. AS IT WAS LIFTED IT SWUNG TOWARDS THE CONTROL HOUSE. EE ATTEMPTED TO GUIDE THE GRIZZLY AWAY FROM THE CONTROL HOUSE TO PREVENT DAMAGE TO IT. IN DOING SO, HIS RIGHT INDEX FINGER WAS CRUSHED BY GRIZZLY AGAINST A HANDRAIL

EMP WAS LIFTING ONE END OF A CUTTING EDGE WHICH WEIGHED APPROX 200 LBS. AS HE WAS LIFTING THE ONE END UP HIGH ENOUGH TO SET IT ON A SHORT TABLE,HE EXPERIENCED LOWER BACK PAIN.

EE WAS DISMOUNTING FROM THE OPERATOR'S CAB OF A WATER TANKER, USING THREE POINTS OF CONTACT. HE PLACE HIS LEFT FOOT ON SOME SURGE STONE. AS HE PUT THE WEIGHT OF HIS BODY ON HIS LEFT FOOT, HIS FOOT TURNED OVER.

EE WAS MOVING A TRANSFORMER FRAMEWORK WHICH WEIGHED APPROX 250 LBS. AS HE WAS REPOSITIONING THE FRAMEWORK, HE LOST HIS FOOTING, AND THE FRAMEWORK, PINCHED HIS RIGHT KNEE BETWEEN IT ANE AND THE FABRICATION TABLE

EE WAS WALKING OUT OF THE OPERATOR'S CAB OF SH-17C, A KOBELCO K-975 FACE SHOVEL. HE CAUGHT HIS RT FOOT ON SOME MACHINE PARTS THAT WERE IN THE WALKWAY, TWISTING HIS RT KNEE.

EMPLOYEE WAS STANDING ON THE LADDER OF FT-03C DIRECTION L-19C, A CATERPILLAR 992B WHEEL LOADER, TO THE BACK OF THE FUEL TRUCK TO BE FUELED. THE FUEL TRUCK OPERATOR TUNRED AWAY FROM THE LOAD ER TO ZERO THE FUEL METER. THE LAODER OPERATOR COPNTINUED TO BACK UP WHILE HE WAS WATCHING THE FRONT OF THE LOADER SO HE WOULDN'T HIT THE FUEL TRUCK. HE PENNED THE FUEL TRUCK OEPRATOR BETWEE

EE WAS CHANGING A HOIST CABLE ON CR-05L, A NORTHWEST MODEL 6 CRANE. THE EE WAS HOLDING THE CABLE DOWN ON THE HOIST DRUM WITH BOTH HANDS WHILE OTHER EE WAS PULLING ON THE CABLE. THE CABLE SLIPP ED OUT FROM THE HOIST DRUM AND HIT HIM ON THE RT CHEEK.

EE WAS CUTTING STRAPS THAT HELD DUCTWORK. WHEN HE CUT ONE OF THE REMAINING STRAPS, THE WEIGHT OF THE DUCTWORK FORCED THE EE TO HOLD THE WORK, CAUSING THE LADDER TO SLIP AND FALL. THE EE FELL 5 -6 FT INTO AN OPEN BARREL WITH ONE LEG IN AND THE OTHER LEFT OUT.

EE WAS CLIMBING UP THE STEPS TO THE 1 BELT, 2 BELT TRANSFER BOX. HE WAS CARRYING HIS BREAKFAST IN HIS LEFT HAND AND TOOLS WEIGHING 50 & LBS. IN HIS RIGHT HAND. HE PROCEEDED TO STEP UP ON THE F IRST STEP WITHOUT THE USE OF HAND RAILS. WHEN HEDID HE FELT A SHARP PAIN IN HIS LOWER BACK.

EMPLOYEE WAS DISMOUNTING FROM THE CAB OF A K914 BACKHOE, USING THREE POINTS OF CONTACT. HE PLACED HIS LEFT FOOT ON A PILE OF LOOSE GRAVEL WHICH HAD BUILT UP AROUND THE TRACKS. AS HE PUT THE WE IGHT OF HIS BODY ON HIS LEFT FOOT, THE PILE GAVE WAY CAUSING HIS FOOT TO TURN OVER.

THE EE WAS HOLDING A CABLE IN HIS HANDS WHICH WAS USED TO UNSCREW THE TELESMITH 44FC CRUSHER. THE LOADER OPERATOR TIGHTENED THE CABLE WITHOUT THE EE S KNOWLEDGE.

WHILE TIGHTENING BOLTS ON CRUSHER WITH HAMMER AND WRENCH, 1ST PARTY HOLDING WRENCH, 2ND PARTY SWINGING HAMMER, THE HANDLE HIT OFF HIS BODY SENDINGTHE HAMMER INTO VICTIM'S HAND, RESULTING IN AV ULSED FINGER TIP.

VICTIM WAS CHECKING DOOR ALIGNMENT AFTER INSTALLING NEW HINGES AND LATCHES. HE WAS CLOSING DOOR TO CHECK ALIGNMENT WHILE RESTING HIS HAND ON EQUIPMENT TO SUPPORT HIMSELF. DOOR CAUGHT TIP OF 3R D FINGER, 3RD JOINT.
