Employee struck left hand with a hammer while performing winter plant maintenance.

Employee slipped and fell while walking around equipment during fueling. Employee's left wrist was injured breaking the fall.

After welding on the Omni Short Head crusher, employee was grinding down the weld when a piece of metal slag flew behind EE's safety glasses and landed in the right eye.

Employee cut right thumb through glove while changing worn out screen cloth.

Employee strained EE's lower back lifting 4 inch steel square stock tubing off the ground to place onto a block.

Employee was using an impact wrench with a 1 1/4 inch socket when the socket came free from the wrench and struck the employee in the mouth breaking a top, front tooth.

Employee's left pinky finger caught between cobble stone and bin chute. Bruise/Sprain, restricted duty.

Employee was exiting stairs, lost balance and slipped to the next step down causing a sharp pain in their lower back.

Employee was changing a front-end loader cutting-edge when a sharp burr on a bolt caught employee's right index finger causing a laceration that required 3 stitches to close.

The employee was making repairs to an excavator bucket, removing excess welds and slag with a hammer and chisel. When struck, the chisel broke free from it's holder and struck the employee in the mouth cutting EE's upper lip and breaking EE's right front tooth in half.

Employee was removing a large nut from a cone crusher tramp cylinder with a large air gun and heavy socket that fits the nut. When the nut came to the end, the socket fell over onto the employee's finger.

Employees were installing and seating top ring of cone crusher when employee had EE's right hand struck by a sledge hammer, cutting and breaking EE's middle finger, and crushing EE's top knuckle.

Employee was sitting on bench in break area before shift started. Employee stood up to punch in and turned wrong causing a strain to back. Employee stated that employee sustained an injury to back in the military a few years ago and this aggravated that old injury.

Employee stood up after working on short head crusher. Started walking around the catwalk carrying a wrench in right hand. Right ankle gave out and fell to catwalk causing sprain to right ankle. Walkway was dry and clear of obstruction. There were no slip/trip hazards.

Employee needed to clear a rock that was trapped in the tailgate after dumping a load. EE used EE's hand and pulled on the side pin of the tailgate to open it. The rock cleared sooner than expected and the pin cut EE's finger as the tailgate returned to the closed position. EE sustained a laceration that required stitches.

Employee stated was changing screens at the 8x24 location in the plant and felt something in the back as EE was maneuvering the screens into position. * Please note this injury was not reported until July 7, 2017.

Employee exited loader after completing snow plowing task during active storm. Employee slipped and fell while walking across yard shortly after exiting the loader, fracturing right leg.

Employee was dismounting from loader to refuel at the end of the day. He thinks his foot slipped as he stepped onto the last step (approximately 18 inches from the ground), lost his balance and fell to the ground.

Employee was raising dump body of dump truck to empty load. Body was approximately 75% raised when hydraulic line burst and dump body slammed back down causing jolt to employee sitting inside the cab.

Employee was unloading steel plate from forklift when he felt discomfort in lower back during the task.

Employee reached into heat duct inside truck cab to warm hand and was struck by turning heat duct blower fan.

Employee was lifting and removing rocks from feed hopper grate. A heavy rock fell on foot causing contusion.

Employee was holding pin punch by handle while second employee used sledge hammer to strike pin to remove master pin from spare excavator track. Employee felt foreign object break skin and enter left forearm when pin punch was struck.

Tripped over floor mat while weighing truck at the counter

The miner was either entering or exiting the cab of a Terex TA-30 articulated end dump when he either slipped or lost his grip causing him to fall backwards from the truck striking his back on a concrete barrier then landing on the ground.

The miner was kneeling on the ground using a sheet rock knife to cut a piece of new skirt board rubber. The blade slipped along the rubber striking the front of his right thigh causing a cut, which required four sutures to close. The next morning, we disccused what would have prevented the accident and what he could have done to protect himself against being cut.

PUSHING ON GENERATOR GUARD, HE INJURED HIS LEFT SIDE.

CLIMBING DOWN FROM BULLDOZER, HIS FOOT SLIPPED OFF OF PUSH ARM; HE FELL ON MUFFLER HE HAD REMOVED.

WHILE DRIVING EUCLID IN QUARRY, WENT AROUND CORNER GOT PAIN IN BACK AND RIGHT LEG.

I INSTRUCTED HIM TO GO TO COLARUSSO'S QUARRY TO LOG OFF TREES THAT HAD BEEN CUT DOWN ON A SMALL AREA OF LAND THAT WILL BE MINED. HE WAS INSTRUCTED TO US A JOHN DEERE 350 DOZER WITH CHAIN TO AC COMPLISH THIS TASK. AT APPROX 8:15 AM THAT SAME MORNING I CAME INTO COLARUSSOS QUARRY WITH A SERVICE TRUCK TO FUEL UP THE DOZER THAT HE WAS GOING TO BE USING. I THEN LEFT AND GOT A PHONE CALL

WHILE GETTING BACK INTO EUCLID, FOOT SLIPPED OFF OF STEP, CAUSING TO FALL ON GROUND. EMPLOYEE HAS PREVIOUSLY INJURED LEFT WRIST, PRIOR TO EMPLOYMENT.

PUTTING NEW SPRAY BAR ON WASH SCREEN FINGER CUT ON SHARP EDGE

WHILE PUTTING CONVEYORS TOGETHER, THUMB GOT WEDGED BETWEEN CONVEYORS, WHERE THEY JOIN.

WHILE GRINDING ON GRINDING WHEEL, PLATE FELL OFFOF WORK BENCH, LANDING ON LEFT FOOT, INJURING LITTLE TOE.

WHILE GRINDING ON AXLE WITH HAND GRINDER, SOMETHING FLEW OVER SAFETY GLASSES INTO RIGHT EYE.

WHILE SHOVELING HIT BOLT STICKING OUT OF GROUND JAMBED LEFT SHOULDER.

USING PORT A POWER, THE PISTON KICKED OUT AND RELEASED PRESSURE ON THE HANDLE, CATCHING FINGERIN HANDLE.

16359 CLEANING SCREENS ON CRUSHER,PULLED MUSCLE IN BACK BETWEEN SHOULDER BLADES.

WHILE EMPTYING WATER FROM 55 GAL DRUM LEFT HAND SLIPPED RIGHT HAND TOOK WEIGHT OF DRUM

WHILE REPLACING PLANKS ON CATWALK. STANDING ON RAILING, SLIPPED, FELL TO GROUND.

WHILE LOOSENING FITTING WHEN FITTING CAME LOOSE BENT THUMB

LIFTING HERNIA POSSIBLE.

EMPLOYEE JUMPED OFF OF BOTTOM STEP OF PAYLOADER, TWISTING LEFT ANKLE.

EMPLOYEE WENT TO DR 12/28/88 WAS OPERATED ON 1/10/89.
