While discarding obsolete hydraulic pipe into metal skip, set one end down to grab other end of hydraulic pipe with both hands and caught right hand pinky finger between pipe and edge of skip.

EE was working on housekeeping tasks. They bent down to pick up an old roller from a conveyor belt and felt a pain in their right wrist. Strain/Sprain

At approximately 11:55am, the Line 3 scrubber drop out box ruptured releasing large volumes of water onto the floor. The employee was likely north of the launder when the water swept them down the inclined walkway wedging them beneath the kiln pier stairs.

When the bit caught a piece of steel, the hand drill twisted in the employee's hand striking the employee in the face with the battery of the cordless drill.

Employee felt pain in their lower back and numbness in their hands after a shovel dropped a bucket of material into their production truck box.

Injured & EE's partner were removing pan skirts. The injured was installing a swivel eye lifting device while EE's partner partially lifted the liner with a lining bar so EE could install the 3/4" nut underneath. The liner slid down the lining bar, pinching the injured's right middle and pointer fingers between the liner/nut and the pan feeder flights

Walking out of the training office employee tripped and fell down the stairs. Employees boot caught the second step and fell onto their knee 2 steps down pinning their left leg underneath.

The employee was pushing a scrap skip weighing approx. 2200 pounds across the Filter floor on a pallet jack when the employee felt a pop in their right calf.

MTM was lifting Milwaukee battery operated come-along that was in the case from the floor to EE's plant kat

Employee was finishing up a belt discharge on 72-5. When EE was closing the doors on the crusher EE pinched EE's palm between the door and the lock post causing a 2-inch laceration

Employee was performing a pre-shift on the shovel. The employee was trying to open the door with their right hand. The door was stuck, and the employee felt a pop in their right shoulder.

Employee driving the 585 was injured during loading at the 18 shovel. A large rock hit the inside of the box causing the operator to feel pain in the neck and hip area. THIS INCIDENT BECAME A RECORDABLE ON 1/6/26 WHEN EMPLOYEE RECEIVED RESTRICTIONS

Injured MTM was installing a drive sheave on a pump with a partner. The MTM's partner was installing the sheave while the injured MTM was pushing on the guard. The injured MTM's right pointer finger was in the pinch point of the guard hinge. When the injured MTM felt the pain from the guard pinching their finger, they pulled it out while it was still pinched.

The employee's left leg slid out and the right foot stayed planted. The employee caught self, but felt a pop in their right knee and maneuvered to the ground until emergency response was called.

EE was pushing a sheet of stainless into position to be welded, as they were pushing the sheet they felt a sharp pain in left shoulder. This resulted in a strain/sprain type of injury.

Employee felt soreness in left shoulder after using a 4' bar and 8 lb. sledgehammer. THIS INCIDENT BECAME A RECORDABLE ON 12/8/25 WHEN EMPLOYEE RECEIVED RESTRICTIONS.

Employee was demoing an out of service AC unit. Employee unbolted and removed the first section of the AC frame, set it on the floor, and reached for an impact. When they reached for the impact, they tripped and fell down to their left knee. Their left knee and shin landed on the piece of demo that was on the floor and cut their shin.

Employee was changing out return rollers on a belt. Employee was lifting return roller on the one side into place on bracket. The roller slipped and dropped to the floor 12 inches smashing the right ring finger fingertip between the floor and edge of roller causing multiple fractures and laceration to fingertip. Employee was sent to VRMC and returned to work with restrictions

Employee became entrapped in the Step 1, 2 Agglomerator basement elevator. The main gate was notified at 852 AM and the employee was able to safely exit the elevator 927 AM.

EE was working in the Kiln. They moved to the side and left knee buckled. this resulted in a strain/sprain type of injury.

While taking apart a connection for conduit, two MTEs were trying to loosen the connection with channel lock type pliers. The channel lock type slipped, unsure if the conduit or tool contacted the injured left side of the chin.

Employee was stuck in the Step 1, 2 Agglomerator basement elevator. The employee became trapped at 902 AM. Security and safety were eventually notified. First responders arrived shortly after 930 and were able to get the employee out at 940 AM

Employee was pushing up on a conveyor belt trying to get additional clearance to remove a roller. After releasing the belt employee felt a popping and burning sensation in their hand

Employee felt pain in the left arm when helping move a window a/c unit. THIS INCIDENT BECAME A RECORDABLE ON 3/13/26 WHEN THE EMPLOYEE RECEIVED RESTRICTIONS.

Employee was busting up rocks with a sledgehammer and putting pieces on the belt to clean the tunnel. Reported to manager that they felt a twinge in abdomen after they finished the job. Before starting their next shift, employee was seen by telemedicine. Hernia.

043-06-1 conveyor belt where employees were doing work to the belt and changing rollers. Employee was helping clean up and pulling out a return roller from under the belt at which time EE felt pain in EE's left bicep. Other employees were also present at the time of the incident. Employee said EE is unable to straighten EE's left arm

MTM was changing all-thread to level the thickener rake. When employee was finished, EE felt pain in EE's shoulder. Employee climbed down from the job and reported the incident to supervisor

The employee was guiding the belt in along the west side walkway of the 27 conveyor when EE stepped onto some cross bracing where grating was removed and fell approximately 4' to the ground striking EE's face on the conveyor structure cutting EE's left cheek and damaging some teeth.

Injured MTM was sitting in a spare 3rd stage mainframe in the zinc shop installing a horse collar liner. EE was operating the overhead crane with EE's right hand and repositioning a horse collar liner for final placement with EE's left hand when EE's left fingertips became pinched between the bottom of the tub and the liner. This resulted in a fractured left middle fingertip

Employee was moving breaker from waist level off of plant kat onto plastic cart which was at knee height and approximately 2 feet away from plant kat. Breaker weighed approximately 120 pounds; the lift was assisted with another employee. The employee felt pain in their left bicep while moving the breaker

EE was replacing a valve. EE was using a torch to remove flange bolts, once EE was down to the last bolt, water that was trapped in the line started to spray out from around the flange. Not wanting to get wet, EE took a step backwards and tripped over a curb. EE put EE's hands out to catch self and bent EE's right wrist backwards, this resulted in a fracture of EE's wrist.

MTMs were changing rollers on a belt. They were dragging a roller with a rope and decided to each carry on one end. The roller weighs about 150#. The injured carried the back and felt a pain go through their left elbow area. The employee reported the incident to their supervisor at the start of shift the next day (9-12-25).

Employee was pushing a full oil cart through the PM stall next to a loader. The cart shifted to the right and pinched the employee's right middle finger between the cart handle and the rim of the loader.

The employee was installing reflectors into berms and adding orange tape. While stepping back, the employee tripped over a rock and fell, landing on their left elbow, which resulted in a laceration.

2 MTMS were removing the outboard bearing on a dust collector. To free the bearing from the taper lock, the injured was using a 3lb. hammer while EE's partner held a chisel in a chisel holder against the bearing. During this process a piece of the chisel broke off and struck the injured resulting in a laceration to the left forearm.

Individual mis stepped while walking down a small flight of stairs and rolled an ankle resulting in a sprain/strain injury

EMPLOYEE WAS USING A UTILITY TO CUT A CONVEYOR BELT WHEN THE KNIFE SLIPPED AND CUT EE'S LEFT LEG

Employee was traveling to sample pellets from line 4 and about the climb stairway from basement level, when they slipped on the pellets and mud at the base of stairway and twisted their left knee.

Employee was lifting a box weighing approximately 35LBS putting it into a pickup and felt a pop in left bicep

61 was filling A fuel truck. They felt their back getting warm and saw that there was a fire. They exited the truck and ran a safe distance. Called the onsite fire department and also had the guard shack call the fire department. There were no injuries, and the truck was a total loss. Fire reported at 7:52 and MSHA was notified at 8:17 am.

Employee was changing the 40-04-1 belt to an air tugger in step 2 Agglomerator. Employee was kneeling down while rigging up the 40 belt. Employee felt a pop in their right knee when they stood back up. EMS was contacted

Employee was changing a belt on a drive motor and pinched EE's finger

At 8:30 pm the driver on a water truck was watering a road and hit rumble strips. The truck hit the strips and the employee was thrown forward and then pulled them back in the seat causing pain in the neck. EMS was called went to the ER for further eval. The truck was taken out of service so seatbelt could be checked and the road was graded. became recordable 6/10 with restrictions

A tree blew over into the power lines near the Basin Shops building at approx. 11:27 AM next to the road. Basin employees expelled 4 extinguishers and called Keetac Fire to respond. Keewatin fire department was called and the DNR and Nashwauk fire department. Basin cat was used to assist in cutting access. Fire was extinguished before 3 PM. MSHA was notified at approx. 11:56 am.

EE was using a cart to haul boiler tubes. The surface was smooth/flat concrete, slag/ash had fallen off the tubes while loading them on the cart. slag/ash prevented the wheels from freely moving in the intended direction while being pushed. This resulted in the cart tipping to one side. the EE stopped the cart from tipping, in doing so the EE strained the muscles of EE's left arm.

Employee was walking from Line 4 performing routine rounds when they slipped on pellets. The employee caught themselves & jaw came down and locked inflicting pain to lower jaw area. The employee notified the supervisor and EMS was called. The employee was then transported to telemedicine and later returned to work with a sore jaw and will be followed up on next shift per EMS

EE was removing steel grating from a floor launder for the purpose of cleaning the launder. As they were pulling the grating they experienced a pain in lower back on the right side. This incident resulted in the EE having a sprained lumbar.

Rolling a pin into a skid steer bucket and felt slight pain in EE's lower back.

EE was grinding/welding as part of bull gear repair that was taking place. They stopped grinding to visually check for high spots. When they flipped up their face shield, they felt something enter right eye. it was determined that the EE had a Corneal abrasion as a result of a small piece of grinding debris entering their eye.

A Crusher employee became entrapped in the Step 3 Fines Crusher elevator between the first and second floors. Our safety department became aware of the issue at 12:05 AM. The employee was originally trapped sometime after 11:30 PM on Saturday, April 19th. The employee was able to leave the elevator unharmed after the fire department responded and mechanically opened the doors
