At approximately 8:10 a.m. on Thursday December 19th, 2024. The employee was attempting to troubleshoot the Screen #2 Feed located in the Allen-Bradley Cabinet, when an Arc Flash occurred resulting in burns to EE's face.

A crusher hand with approximately 3 months of experience broke two fingers while performing routine maintenance on crusher apron. EE was able to return to work the following day

RAM employee was in a feed chute relining it. EE was standing on a belt that was on a slight incline with dust and gravel, they tried to keep it clear but every liner removed would dump dust. While lifting a 80lb liner into place EE's left foot slipped putting strain on EE's left knee.

The employee was cutting belting with a utility knife and cut leg. The knife slipped and cut leg.

The employee was injured when an implement tool dropped onto his Right foot. There were several employees in the process of changing the implement tool when this happened.

Walking up steps to access fuel pump, missed footing on step and sprained left knee. Started physical therapy on 04/04/2011.

Operating loader, seat suspension failed causing seat to tilt and bottom out. Operator strained back and neck.

The employee was replacing screens on a crusher plant. As he tightened a bolt with an impact wrench he got his finger on the end of the bolt. As ee spun the bolt with the impact wrench, the head caused a laceration to the end of his finger.

Supervisor of crusher crew suffered a laceration to his left hand while unloading a conveyor belt from the back of a pick up truck. The laceration required 30 stitches to his hand.

A piece of electrical switch gear broke off and arced between B & C phases inside switch gear. This caused an arc flash which resulted in a first degree burn to the operators face and neck on the right side.

A rock was stuck between a conveyor pulley and the frame of the conveyor, as the employee removed the rock, the rock slipped and smashed left fifth finger.

while replacing cutting edge on a 988 cat loader operator pushed bolt into place with his hand rattle gun spun the bolt cutting through glove and lacerating his hand.

EMPLOYEE WAS REMOVING WORN OUT BUCKET TEETH ON 992 LOADER. HE STRUCK A TOOTH WITH A HAMMER AND A PIECE OF THE TOOTH BROKE OFF AND EMBEDDED IN HIS LEG.

DISMOUNTING CAT 777B TRUCK, MISSED LAST STEP AND FELL TO THE GROUND.

EE WAS POSITIONED TOO CLOSELY TO THE 773 HAUL TRUCKS HE WAS SPOTTING FOR IN THE FILL DUMP AREA. A ROCK FELL OFF THE LOADED MATERIAL IN A HAUL TRUCK WHEN THIS MATERIAL SHIFTED ONCE THE HAUL TRU CK DRIVER APPLIED THE BRAKES. THE ROCK HIT THE INJURED EE ON THE HEAD AND RIGHT SHOULDER.

EE WAS CUTTING A USED CONVEYOR BELT FOR SKIRTINGMATERIAL WITH A UTILITY KNIFE. WHEN THE UTILITY KNIFE GOT STUCK IN THE BELT, EE PULLED HARD ON THE KNIFE FREEING IT FORM THE BELT AND PUNCTURING HIS RIGHT LEG JUST ABOVE THE KNEE. EE WAS KNEELING ON THE BELT DIRECTLY IN LINE WITH THE CUT.

WHILE ATTEMPTING TO DISLODGE A SHOVEL FROM STACKER CONVEYOR, EE'S LEFT HAND MAD COMTACT WITH BELT & ROLLER ASSEMBLY CAUSING MODERATE SOFT TISSUE DAMAGE TO THREE FINGERS.

COMPRESSED HIS FINGER BETWEEN DRILL STEEL & STEEL GUIDE.

THE EMPLOYEE WAS TRYING TO PLACE AN UPPER SUPPORT BEAM ON A WING WALL OF THE JAW CRUSHER.THEY USED THE BOBCAT BUCKET TO BALANCE AND WORK OUT OF.ONE FOOT ON LOWER BEAM AND ONE IN THE BUCKET.THE BOBCAT OPERATOR LIFTED THE BUCKET PUSHING THE LEG INTO THE UPPER BEAM.THIS AFFECT PUT GREAT PRESSURE ON THE LEG AND RESULTED IN A BROKEN ANKLE.

WENT TO DR WITH SORE NECK AFTER DRIVING A773 HAUL TRUCK FOR 1 YEAR AND 16 WEEKS AT THE FLAMBEAU MINE HIS ONLY JOB WAS DRIVING TRUCK.

HIGH TEMP. THE DAY BEFORE WAS IN THE 40'S, DROPPING TO AROUND 20 AT NIGHT. CAUSED ICY SURFACE ON PARKING LOT. EMPLOYEE WAS REPORTING FOR WORK AND ALLEGEDLY SLIPPED ON THE ICE.

EMPLOYEE CLIMBED DOWN THE HILLSIDE TOT HE CRUSHER TRAP TO REMOVE A LARGE ROCK. WHILE CLIMBING BACK UP THE HILL A LOOSE ROCK ROLLED DOWN AND HIT HIM ON THE KNEE. HE RECEIVED A CONTUSIONTO THE RIGHT KNEE. DR PLACE DHIM ON LIGHT DUTY AND PRISCRIPBEDPHYSICAL TERAPY, FOR 1 WK.

EE JOB INCLUDES THE LIFTING OF SAMPLES WEIGHING 40-60-LBS IN THE PERFORMANCE OF HIS JOB ON 07-06-93 EE EXPERIENCED A POPPING SENSATION FROM THE GROIN AREA EE WAS TAKEN TO SEE A DOC & PLACED ON A LIFTING LIMITATION OF 10LBS FOR A PERIOD OF ONE WEEK SAMPLE WAS REDESIGNED TO THE BEST POSSIBLE ERGONOMIC DESIGN & REDUCED TO 15-30LBS IN WEIGHT
