Mostrando los 13Miner was cutting plastic with a box cutter, and the box cutter slipped out/off of the plastic, hitting the miner's hand and causing a cut on the knuckle of a finger. Miner went to Urgent Care for treatment.
The miner was hitting a steel bearing with a hammer when the bearing broke and a small piece of steel shot out and stuck into his leg.
Employee was watching a cable being moved. Cabled whipped, catching the employee around his ankles and knocking him off his feet. Employee fell, landing on right side, fracturing his right shoulder, hurting right wrist, laceration to the right side of his head, and right ear.
per employee, "Changing screen on gravel plant. The screen slid on my thumb and bent it way back."
This report is late because we were told at first it was a re-injury. It has been established that it is a new injury. The employee was "working on dry screen of wash deck" and was "pulling on ratchet in awkward position". He has tendonitis and possible rotator cuff injury to the right shoulder.
Per employee "getting sand sample - put sand in bucket and put 1 1/2 pipe into bucket - dump out sand - keep what is in pipe - that's the sample - when I stood up I felt it pop" (lower back). Employee has pain in lower back that resonates down right leg.
per witness / other participant "taking rock out of hopper with excavator, turn engine speed down just started take hydraulic out heard him scream."
Per employee, injury occurred "in Greenville Pit southwest side of pit dry screening. Stood up too early in screen and hit head." Injury - minor, superficial laceration to scalp. Dissolvable stitches used. No follow up with provider required. Employee returned to work the same day.
EE was shoveling out under gravel plant. He got halfway done & stopped to take a break. He felt muscle in lower back tighten up like a knot. He dropped down to his knees & finished shoveling out the plant.
While using a loader, a clump of dirt fell out and wedged in the front part of the loader. Employee got out to remove it and when it broke free it hit his right leg, scraping it.
Running loader, it hit a bump when turning, banging his left elbow on metal frame.
CARRYING UP NEW M0NITOR, TRIPPED AND FELL WITH MONITOR 8 FT TO GROUND.
