While two employees were working around a tail pulley of a conveyor belt, there was miscommunication between the guys on the ground and the plant operator. This resulted in the belt turning on and grabbing one of the employee's sleeves, pulling them into the tail pulley.

Weather conditions were cold. The 5 X 14 shaker wouldn't start; breaker was tripping. The employee checked for electrical shorts and continued by opening the electrical box to check the breakers. He removed the wires to attachment them directly to the starter. The wires arced 2 - 3 times burning his face and hands. The shaker caught fire.

Customer's driver was inside belly dump cleaning when the doors closed on his legs. This driver works for Guerrero & Sons Trucking company. He is not an employee of this company. One leg was broken and the other fractured. He did not place safety pins in place to insure doors would stay open.
Was installing a gear box. Gear box slipped out of hands and hit left knee causing bruising and swelling requiring medication and therapy.
Was cutting conveyor belt with a utility knife when it broke slipping and cutting top of right arm about 3 inches. Required seven stitches.
Replacing hammer wedges on universal. Twisted wrong in an enclosed area and strained upper back. Required prescription medication and therpy.

I HEARD EE YELL, I LOOKED AND SAW HIS HAND IN THE TAIL PULLEY, I RAN TO THE CONTROL PANEL, SHUT OFF CONVEYOR, BY THE TIME I SHUT OFF THE CONVEYOR, CO-WORKER WAS WALKING HIM TO THE TRUCK.
