Employee was attempting to dislodge a stone in a jaw crusher with a long steel rod. As EE used downward pressure to strike the stone, EE struck EE's right hand on the edge of a steel plate that surrounded the crusher.

Employee was doing maintenance on a conveyor belt. Employee was cutting rubber for a skirtboard with a utility knife. Employee cut themselves in the left hand through a leather glove. Employee required sutures to close the wound.

WHILE MOVING A CRUSHER LINER, RIGGING SLIPPED & CRUSHED EMPLOYEE LEFT FIRST & 2ND TOES

Lead Technician was on their second day working alongside Field Technician to complete field assembly of an HD605-8. Work is being performed inside the equipment maintenance shop. Technician finished welding the left side tire guard on. Technician was using a cloth and solvent to wipe the welded area when they noticed something had entered left eye from behind safety glasses.

Middle finger hit with sledgehammer causing finger fracture & lacerations.

EE drove the loader down the hill to remove material from the stackers and the parking brake came on causing whiplash which resulted in EE hitting head on the windshield.

Operator of an excavator climbed up onto the machine to enter the cab. While standing on the track to enter the cab the operator fell backwards to the ground. operator dislocated their left shoulder, fractured their left hip and cut their left elbow requiring stitches.

Employee was walking across pit floor and twisted ankle, resulting in a fracture of left foot.

A piece of steel was cut from a chute with a torch and slid down and pinched and burned the hand of the employee performing the work.

EE was climbing down from 773D haul truck and EE's hand slipped off the handrail and fell approximately 10' to the ground injuring right wrist and back.

Metal backplate crushed left index finger

Two EEs were using tanker bars to adjust a radiator on a pallet to prepare it for shipping. As the two were both pushing on the bars, the injured EE lost control of bar and slipped off the radiator causing EE to fall to the ground. When EE fell, right knee impacted the concrete floor of the shop. EE immediately felt a pain in lower back, side, a leg.

Employee was grinding the edges of a 4 inch by 4 inch piece of metal using a stand grinder when the metal got lodged between the grinding stone and the safety guard pinching EE's finger between them. EE sustained a broken bone and the loss of the finger nail on right hand.

Stone chute was lying on conveyor #10A, tried to lift chute with one hand when EE felt a pop and then fingers tinging.

Employee was moving a heavy duty jack stand when EE lost EE's balance and fell to the concrete floor in the shop. The jack stand landed on EE's right hand causing a laceration that required 7 stitches to close.

Standard threshold shift on annual hearing test. Retest validated the STS.

EE was not feeling well & sat down on a picnic table within the quarry shop. Shortly after sitting down, EE experienced a medical episode, believed to have been related to diabetes, which caused EE to faint & hit head on the picnic table. Incident was not work related but due to a medical episode. EE received staples to back of head that was cut when EE fainted & hit head on table.

Employee was removing a chain that slipped off the lower drive sprocket of a stacking conveyor motor. After locking out the drive motor, employee was removing chain on "slack" side of upper sprocket. As employee did this, the tension on the opposite side, pulled the chain and employee's hand and pinched it between the chain and the sprocket.

Miner was attempting to remove a rock that was in the collection box at the top of conveyor #3 at the primary before plant start-up. Employee was prying a rock with a bar that was about 6" that was wedged against other material. While the miner was pulling the bar, the rock became free and the bar pinched EE's pinky finger on the left hand between the top of the box and the bar.

Employee's right hand came in contact with weed trimmer blade causing a laceration to Right wrist (Stitches).

Employee was changing screen cloth on the #1 screen when EE lost EE's balance and fell hurting EE's right knee and left shoulder. The employee continued working without restrictions until lay off when EE had surgery to repair EE's knee. Was originally first aid.

Injured employee slipped on the ice fracturing left leg.

Employee slipped on icy surface hurting right shoulder, continued to feel pain and was found to have shoulder problems requiring surgery on 04/11/2019, was originally reported as 1st Aid. Is now lost time.

Employee was removing a chain on a skid steer and cut right thumb

Field Service Technician performing work on a piece of heavy equipment. Just removed a strap from truck crane to hydraulic cylinder. When trying to remove strap completely from cylinder, the cylinder dropped and pinched right wrist.

Fingers caught in heater fan

While operating loader employee felt pain in right knee. This injury was 1st Aid until ee had surgery on 2/23/2018.

Hand rail broke causing employee to fall 4 feet

Employee slipped and struck knee with a wrench causing a small cut. Injury became reportable on 9/14/2016 when employee went for medical treatment and was give restrictions and prescription medication for infection.

Bearing on head pulley slipped pinching finger.

Employee tripped over wheel chock onto concrete floor.

Employee was using chipping hammer while doing maintenance on stone plant Deister screen. Employee's hands were sweaty and slipped down shaft of chipping hammer, hitting screen and causing small laceration, bruise and swelling. On 7/7/2016 employee sought medical attention for the swelling that had not receded. Diagnosis was broken bone in left hand.

Employee felt pain in shoulder over the last few years and decided to get it looked at this year - Repetitive had surgery on 8/4/2016

Pulling gate closed at the end of the day and felt pain in left shoulder, was originally reported as 1st aid. Employee is now treating with physical therapy as prescribed by physician making this re-portable.

Cutting steel got hot slag in ear, was originally reported as first aid.

Employee was pulling on a wrench when he felt pain in his shoulder. Employee went to the Chiropractor on 10/28/14, this is the date the injury became reportable.

Employee got his right middle finger pinched between screen panel and screen frame.

Employee twisted his knee climbing out of quarry truck. Accident was originally 1st Aid and became reportable on 8/4/14.

Cut finger on sharp bolt while changing cutting edge.

EMPLOYEE USING UTILITY KNIFE TO CUT CONVEYOR BELT. KNIFE SLIPPED, CUTTING THROUGH SAFETY GLOVE, LACERATING LEFT INDEX FINGER.

Hauling stone from pit to Primary Crusher, Truck went over highwall.

Employee bumped right knee on steel channel.

Checking for oil leak in hose, when oil squirted employee moved his head striking lip on sharp object (2 Stitches).

EE was lifting the welding trailer off the hitch of a pickup truck and felt soreness in his lower back.
