Mostrando los 25EE pulled up to quarry office to sign in for the day. As EE stepped out of truck onto the shoulder of road EE slipped on ice exiting the truck. EE tried to grab door but fell to ground with shoulder/right arm pain and was taken to urgent care.
Cut left hand while breaking down boxes
EE was prying on a mechanical lacing clip. EE lost grip of bar, the bar then bounced off of the belt coming towards EE and hit EE in the mouth resulting in a chipped left central incisor.
Employee was trimming the skirt board on C-06 with the grinder and lost control of the grinder resulting in a cut to hand.
Two millwrights were trying to remove a metal plate from the conveyor by inserting pry bars. As the part broke free, EE's hand was pulled with the pry bar and pinched between the bar and the conveyor roller assembly causing a fracture of the distal phalanx of the left little finger.
EMPLOYEE WAS STEPPING DOWN OFF THE BUMPER OF THE MAINTENANCE TRUCK ON TO LEVEL GROUND AND SPRAINED RIGHT ANKLE.
(1st day of Treatment 1/13/23) Bending over to tie in the shot and felt low back give out.
Employee was chiseling nordback from crusher with a hammer and chisel. As the employee struck the chisel with the hammer, the chisel came out of hand and struck a nearby employee in the mouth chipping two front lower teeth and cutting upper lip.
The employee claims no specific incident, but has pain in EE's back from years of driving a haul truck.
The employee was moving an 8'x19'x1/2" sheet of steel assisted by a telehandler. The employee tried to slide the sheet of the forks in a pulling motion then felt a pop in the left bicep.
Miner was installing an impact roller on C-12, Miner was applying force to the roller with right hand and bracing self with left hand. When Miner released right hand the roller came in contact with the Miner's left hand causing a break to thumb and hand to become bruised. The Miner was wearing all required PPE including gloves per Company Policy.
Employee was repairing fuel truck under carriage, left job after working on it for 1hour, he returned to complete job, then felt soreness in his right shoulder and had problem lifting arm to side. He was working off a creeper on the floor. From floor to working area is about 22 inches
Employee was guiding the bottom of a staircase into place, when the handrail he was using to guide broke at spot weld and struck him on the left side of the head. This injury required stitches right above his left temple.
EE fell from a standing position to the ground causing a laceration to his head and a fracture to a rib.
EE was working in corn fields on the mine property, a tick got under his clothing and bit EE on the R side of his stomach below the rib cage. EE discovered the tick in his skin, went to ER to have tick removed and sent to lab to check for Lyme disease. He was given a shot of lidocaine in the area of the bite and was sent back to work.
Pulling 1 1/2" water hose line when employee felt pain in the right shoulder.
Employee was changing the hammer on the drill with another ee. When they started to lower it down employee felt something pull in his left shoulder.
Elective surgery for Carpal Tunnel Release. Surgery performed on 1/4/07 on left wrist. First diagnosed as CTS on 3/4/06. EE states previous injuries in 1991 and prior may have contributed. EE alleges injury to be caused by driving a loader for 38 years.

EE EXITED HIS VEHICLE IN THE EE PARKING LOT AND SLIPPED ON A PATCH OF ICE THAT WAS LOCATED BETWEEN HIS CAR AND ANOTHER CAR. WHEN THE EE SLIPPED, HIS LEFT ELBOW CONTACTED THE ADJACENT CAR IN THE RIGHT HAND CORNER PANEL & HIS RIGHT FOOT CONTACTED HIS VEHICLES LEFT TIRE WHICH RESULTED IN THE EE CATCHING HIMSELF WITH OUT CONTACTING GROUND.
EE WAS ENTERING THE LADDER ENCLOSURE, WENT TO TAKE FIRST STEP ON RAILINE, LOOKED UP & HIT HIS NOSE ON THE ENCLOSURE CAUSING AN INJURY.
EE (TRUCK DRIVER) CLIMBING INTO TRUCK WHEN DOOR HANDLE DISLODGED FROM DOOR AND LEFT KNEE STRUCK RUNG OF LADDER. EE DID NOT INFORM US HE NEEDED MEDICAL ATTENTION WE FOUND OUT AFTER HE WENT TO MEDICAL HELP ON ABOUT 8-26-03 WHEN BILLS ARRIVEDAT OFFICE.

EE WAS ALLEGEDLY HANDING LINER PLATE TO ANOTHER EE, WHEN HE FELT PAIN IN HIS LOWER SIDE. EE DID NOT REPORT ACCIDENT UNTIL NEXT DAY AT 5:30PM.
