As worker was climbing in to the flatbed of a truck ee felt a pop in left foot. EE continued to work and declined treatment at the time of injury. EE did not think it was too bad and thought it would get better. Pain gradually worsened over several days. On 6/11/17 ee was hurting bad and decided ee needed treatment.

Mostrando los 16Equipment was locked out while employees were flushing ice from sand screw. Later employee rolled the vee belts backward to unplug east screw. Sand screw motor was energized. Vee belts pulled employee's hand into belt guard, cutting EE's hand & breaking 2 fingers. Employees had removed their locks to bump the equipment when this occurred.
Slipped on a piece of rubber and fell. Sprain to left ankle.
Employee was taking a barge hatch off using an air impact to remove the bolts. When the hatch was pried open with the bar it came flying up from the internal pressure and struck his hand. He was diagnosed with a bruised wrist.
Employee stepped backward, tripped on a 4X4 and fell down injuring his back. He was diagnosed with a lumbar strain.
Employee was cutting rubber in #3 vibrating screen. His knife slipped and cut his right hand.
Employee was in an open space using a torch to cut up a barge. Old oils on the bottom were smoking and he continued to work. Started coughing and ended up with smoke inhalation.
Employee stated that the line on the capson head got too tight. When he went to take off the line it jumped and hit the palm of his left hand and jammed his wrist.
Employee stated that there were two possible causes. Either when he was installing a plate in a chute or when he was pushing a guard in place. Right side of ribcage sore.
EE was moving a shaft from skiff to the back of the pickup & slipped.
WORKER STATED HE WAS USING METAL LATHE TO CLEAR THE BIT OF STEEL WITH LATHE LOCKED OUT. THE PIECE OF METAL HE WAS REMOVING CAUGHT ON THE BIT AS HIS HAND WAS PULLING ON IT AND HE CUT HIS THUMB.
EMPLOYEE STATES HE WAS WORKING ON CENTER MAIN ENGINE EXHAUST, WRENCH SLIPPED AND STRUCK EMPLOYEEIN FACE.
EE WAS WORKING AROUND CO-WORKER WHO WAS WELDING AND GRINDING. A PARTICLE GOT INTO HIS EYE.
EE WAS KNEELING DOWN ON BOTH KNEES TO REMOVE THE BUCKET OF AN A MODEL. WHEN HE TRIED TO STAND UP, HIS KNEE LOCKED AND WOULD NOT STRAIGHTEN. HE WAS UNABLE TO PUT HIS WEIGHT ON IT.
EE WAS WALKING FROM CRAWLER CRANE AND STEPPED ONA NAIL IN ROADWAY PUNCTURING THE BOTTOM OF LEFT FOOT. HAUL ROAD WAS MUDDY AND HE DID NOT SEE THEHAZARD.
EE WAS TRYING TO START AN OUTBOARD MOTOR ON WORK SKIFF EQUIPED WITH MANUAL PULL CORD START. IN PROCESS, EE STRAINED LEFT WRIST.
