EE GOT DEBRIS IN HIS RIGHT EYE WHILE HELPING REPAIR A DOZER. AT THAT TIME HE WENT TO THE EMERGENCY ROOM. HIS EYE WAS CLEANED OUT & HE WAS GIVEN A PRESCRIPTION FOR TOBRADEX.

Mostrando los 17SURVEYERS WERE GOING TO TEST A WATER WELL OFF OF ALLEY ROAD. ONE PICKUP TRUCK GOT STUCK IN THE MUD. THE HEAT FROM THE TRUCK CAUGHT THE GRASS ON FIRE. BECAUSE THE GRASS WAS TALL AND THICK, THE FIRE SPREAD VERY QUIKLY. FIRE SPREAD AROUND THE TRUCK, BUT THERE WERE NO INJURIES. HE COULD NOT STOP IT.
WORKING NIGHT SHIFT, EE WAS CHANGING TRACK LINKS ON A BACKHOE, USING A 4" AIR GRINDER AND A TORCH. HE HAD ON SAFETY GLASSES AND A FULL FACE SHIELD. HE WAS NOT AWARE THAT HE HAD GOTTEN ANYTHING IN HIS EYE UNTIL HE GOT HOME AND HIS EYE WAS BOTHERING HIM. HE WENT TO THE DR AND GOT A PRESCRIPTION FOR EYE DROPS.
EE WAS WORKING UNDER THE FLOOR PLATE OF A D11 DOZER, BREAKING A FITTING LOOSE ON THE HYDRAULIC CONTROL VALVE. WHEN THE FITTING BROKE LOOSE, HIS HEAD HIT A CLIP HOLDING AN ELECTRICAL CONNECTION , CUTTING THE BACK OF HIS HEAD. HE WENT TO THE ER ABD 3 STAPLES WERE USED TO CLOSE THE LACERATION. THE COMPANY WAS NOT AWARE THAT THE EMPLOYEE RECEIVED STAPLES UNTIL 3/5/01.
EE HAD BEEN INSTALLING A TRANSMISSION ON AN END DUMP & WAS GETTING DOWN OFF OF THE TRUCK. HE SLIPPED & FELL APPROX 6 FT. HE PUT HIS HANDS OUT TOBREAK THE FALL & HURT HIS LEFT FOREARM. HE WAS T AKEN TO E.R. & A SPLINT WAS PUT ON HIS FOREARM
EE WAS PUTTING RAILS ON THE BUCKETWHEEL CONVEYOR SYSTEM. HE WAS HOLDING A PIN WITH ONE HAND & HITTING IT WITH THE OTHER. THE HAND HOLDING THE PIN SLIPPED DOWN AS HE WAS SWINGING, THE PIN TURNE D AND HE HIT HIS FINGER CUTTING IT. 4 STITCHES.
EE WAS OPERATING WATER WAGON (W004) NEAR AUXILIARY STOCKPILE. STOP ENGINE LIGHT WAS ON & ENGINE STOPPED. SMOKE & FIRE WERE COMING THRU THE FLOOR. HE ACTIVATED FIRE SUPRESSION SYSTEM FROM CAB & EXITED. FIRE CAUSED TOTAL DAMAGE TO THE CAB & ALL HOSES & ELEC. WIRING DESTROYED, TIRES DAMAGED.MAINT. DEPT. INVEST: HYD. LINE ON STEERING CYLINER BURST, HYD. OIL ON TURBOCHARGER CAUSED FIRE.
EE WAS OPERATING A WATER TRUCK NEAR THE AUXILIARY STOCKPILE. HE NOTICED THE STOP ENGINE LIGHT WAS ON AND THE ENGINE STOPPED. HE NOTICED SMOKE AND FIRE COMING THRU THE FLOOR, ACTIVATED THE FIRE SUPPRESSION SYSTEM FROM THE CAB AND EXITED THE MACHINE. HE WAS SEEN 1/2 CLIMBING, 1/2 JUMPING DOWN THE LADDER ON THE FRONT OF THE CAB AND HE ROLLED ON THE GROUND.
EE WAS GOUGING KEEPERS OFF OF A DRAGLINE BUCKET. A BALL OF HOT METAL GOT INTO A CREASE OF HIS JEANS, BURNED THROUGH AND FELL INTO HIS BOOT, BURNING HIS LEFT FOOT. THE BURN GOT INFECTED AND HE WAS TREATED FOR THE INFECTION.
HE & A WITNESS (NWR EMPLOYEE) WERE TAKING IDLERS OUT OF A RACK AND PUTTING THEM ON A TABLE TO REBUILD. A LINK THAT HOLDS THE IDLER CAN TOGETHER BROKE, CAUSING BOTH EMPLOYEES TO FALL BACKWARDS . INJURED TWISTED HIS KNEE AT THE SAME TIME THE HALF OF THE GARLAND HIT THE INSIDE OF THE KNEE HE TWISTED. KNEE IS IMMOBILIZED DUE TO SPRAIN.
EMPLOYEE WAS OPERATING A COAL HAULER COMING UP THE DX RAMP. THE TRAILER HITCH BROKE LOOSE AND THE TRUCK REARED UP. WHEN THE TRUCK CAME BACK DOWN, IT JARRED HIM. HE HAS BEEN SEEING A DOCTOR SIN CE THEN AND ON 10-6-00, HE MISSED HIS FIRST DAY OF WORK DUE TO THE INCIDENT.
EE WAS AIR ARCING ON A DRAGLINE EQUALIZER BAR AND GOT SOMETHING IN HIS RIGHT EYE. (HE WAS WEARING A WELDING HOOD.) HE WENT TO THE EMERGENCY ROOM AND WAS REFERRED TO AN EYE DOCTOR WHO REMOVED M ETAL FROM HIS RIGHT EYE. HE GOT A PRESCRIPTION OF GENTAC (ANTIBIOTIC EYE DROPS.)
THE INJURY INVOLVED NO ASPECT OF REGULATORY COMPLIANCE. THE INJURY INVOLVED EQUIPMENT 4011 THE INJURY OCCURRED DUE TO AN INTERNAL WELD OF THE TRAILER'S NOSE CONE FAILING.
EMPLOYEE WAS GRINDING ON A HAULER DOOR (ONE ON A SPARE CAB WHICH WAS SITTING ON A STAND) AND TRASH GOT IN HIS GLASSES AND IN HIS EYES. HE WENT TO THE DOCTOR WHO REMOVED A PIECE OF METAL FRO M HIS RIGHT EYE.
EE WAS BUFFING AROUND BOLT HOLES. THE BUFFING WHEEL CAUGHT ON THE BOLT HOLE, CAUSING THE GRINDER TO KICK BACK INTO HIS LEG JUST ABOVE THE KNEE.
EE WAS DOING SOME PAPERWORK AND STARTED FEELING LIGHTHEADED. SHE WENT OUTSIDE FOR SOME AIR AND STARTED FEELING DIZZY. EMT CAME BY AND SHE ASKED HIM TO CHECK HER BLOOD PRESSURE. HER BLOOD PRESS URE WAS HIGH, SO EMT TOOK HER TO FIRST AID ROOM WHERE SHE BLACKED OUT. AN AMBULANCE WAS SUMMONED AND SHE WAS TRANSPORTED TO THE HOSPITAL. THIS IS A MEDICAL CONDITION.
EE WAS MOVING A DOOR IN THE DRAGLINE REVOLVING FRAME. HIS FOOT SLIPPED ON AN INSPECTION RING COVER IN THE FLOOR, HE LOST HIS BALANCE AND THE DOOR SLIPPED AS HE WAS GOING THROUGH THE BULKHEAD. HE PINCHED HIS FINGER BETWEEN THE BULKHEAD AND THE DOOR, CUTTING IT. THE LACERATION REQUIRED FIVE STITICHES.
