While walking from vehicle to mine office, blacked out and fell to the ground.

The roof fall was discovered during the weekly examination of the return airways.

This was a rock fall on the intake No.6 entry. The fall measured 18'x50'x10'. The fall will not be cleaned up and did not effect escape way or airways.

A roof fall occurred in the #7 intake entry approx 350' outby the working section. Measured 20 X 50 X 8. The fall was timbered off and will not be cleaned up. No equipment or personel were involved.

While preparing to begin bolting, a rock fell from mine roof striking the victim on his right shoulder and arm.

THE FALL WAS DISCOVERED ON THE PRE-SHIFT. THE FALL MEASURING 18'X 12'X 35' WAS LOCATED IN THE LAST OPEN CROSSCUT IN THE NO.6 ENTRY.

The fall occured in the no.2 entry approx. 250' out of the face. measuring approx. 8'x20'x25'. the intersection was in a fault area.

The roof fall occurred on the 001 section during an idle weekend. It was located in the last open crosscut in the no. 5 intersection, measuring 8' x 18' x 45'.

THE EMPLOYEE WAS OPERATING THE OFF STD 10SC SHUTTLE CAR WHEN HE HIT A POT HOLE THEREFORE BOUNCING HIM INTO THE CANOPY. HE THEN COMPLAINED WITH NECK PAINS

A roof fall occured in the no.4 entry measuring 8'x18'x30. The fall was discovered during weekly exam.

THE FALL OCCURRED DUE TO A SLIP. MEASURING 8' X 18' X 50'. THE AREA HAD ALREADY BEEN CRIBBED AND DANGERED OFF BEFORE FALL OCCURRED.

A ROOF FALL MEASURING 60' L X 40' W X 8' H WAS DISCOVERED ON THE ABANDONED NO.2 MAINS.

A roof fall was discovered at S.S. 128 and extending 70' inby No. 2 mains No. 7 entry.

Employee was tramming the roof bolter from the deck and had his foot hanging out of the bolter when he got his foot caught between the roof bolter and the rib in the number 5 entry.

Employee was making a belt splice on the surface belt when he slipped and fell off of the belt hitting on his right side. Employee fell approximately (4) four feet.

Employee was drilling the top installing a bolt when his drill steel hung. As he was reaching for a wrench to loosen the bolt it came unloosed and fell. The ee had his hand lying on the top of the drill pot and the bolt hit his middle finger on his right hand. Caused a fracture to finger.

A ROOF FALL WAS DISCOVERED APPROX 60' INBY THE NO. 2 BELT PORTAL. THE FALL MEASURED 5' HIGH X 18' WIDE X 25'.

A ROOF FALL WAS DISCOVERED AT STATION 92. THE FALL WAS IN THE NO. 1 ENTRY RETURN OF THE NO.2 MAINS 1200' OUTBY THE ACTIVE 001 SECTION. THE FALL MEAUSRED APPROX. 6' HIGH 20' WIDE AND 80' IN LEN GTH.

A ROCK FALL WAS DISCOVERED AT SURVEY STATION #60. THE FALL MEASURED APPROX. 4 FEET HIGH BY 40 FEET LONG BY 20 FEET WIDE. THE FALL OCCURRED IN ANINTERSECTION. FALL WAS IN THE NEUTRAL ENTRY. THE FALL CAUGHT THE HIGH VOLTAGE CABLE AND WAS BOLTED WITH 42 INCH RESIN BOLTS.

EMPLOYEE WAS LAYING DOWN PUTTING A GREASE PLUG IN THE SECTION BELTTAIL ROLLER WHEN A PIECE OF ROCK MEASURING APPROX 24 INCHES BY 30 INCHES, 3-4INCHES THICK FELL AND HIT HIM ON THE RIGHT LEG.

EE WAS BOLTING TOP IN THE #3 HEADING WHEN 3O" PIECE OF DRILL STEEL HE WAS USING BROKE & HIT HIM IN THE MOUTH AREA
