An employee was operating the 2012 Cat 993K loader #551 loading overburden in the 11B pit. At approximately 3:42 AM flames were noticed coming from the rear of the engine compartment near the radiator. The on-board fire suppression activated as the operator exited the cab but could not contain the fire within 30 minutes of discovery.
Employee was assigned to conduct cleaning activities on the Poca 9 from the Horsepen level, at this time highwall materials loosed up at the 12 C level and fell away from the highwall. The material travelled across two safety benches that are approximately 30 feet wide each. The material struck the Excavator, including the operators compartment.
Employee was assigned to operate Caterpillar 777C #648 truck. EE was traveling down a haul road loaded, when EE stated that EE struck a hole in the road, jamming head and neck. EE suffered pain in neck and upper back.
An idle Dozer was found to be on fire at 5:45 PM, crews tried to extinguish the fire but were unsuccessful. Since it was not being operated at the time and no one was near the machine and the fire damage was extensive, an indication as to what started the fire can not be determined at this time.
Employee slipped and fell on saw, cutting EE's hand.
The Tech was performing a removal of a Y-PIN on a 993k Wheel Loader. They were pulling the Y-PIN with the PIN tool specified in the CAT SIS system. When pressure was applied the puller rod pulled the threads out of the end of the Y-PIN. When they moved the apparatus the bell slid off of the puller rod causing the weight of the rod to flip back and strike the tech in the face.
EE was struck in the face with a fragment of steel that flew off a mounting pin. The pin was struck by a sledgehammer to drive into place on a Pan hard Bar for a Caterpillar rock truck. EE received a laceration that required 3 stitches to close the wound.
EE had removed right side of the cap and started loosening bolts on the left side. Was removing the 3rd of 4 bolts when the cap assembly shifted. EE instinctively raised right hand to block the cap assembly from hitting them. The cap assembly hit EE's hand and punctured glove taking hand with the cap assembly causing the cut to right thumb/palm.
Employee was stemming holes when EE experienced low back strain/pain.
EE, at the beginning of the shift was climbing the mounting ladder on the 789C Rock truck and as EE went to grab the grab rail on the front of the cab, missed the rail with hand and as a result fell backwards and landed on back & right hip.
EE was part of a crew assigned to install a hard-bar pin in the D9T dozer on the mud dump parking area. The pin was aligned and placed to be driven in place with a sledgehammer. When the pin was struck with a hammer a fragment of the pin fractured off and struck EE in the face. EE received a laceration that required stitches.
Employee was climbing up the access steps of the 777C rock truck to clean the mirrors. EE lost footing and fell backwards to the ground. Employee landed on back and began experiencing pain in the neck, back, and hips as a result of the fall.
Stemming drilled holes using hand shovel to push stemming into hole when employee experienced pain in lower back radiating down into right leg.
EE was using a sledgehammer to drive the coupling latches down on the highwall miner push beams. As EE attempted to swing the hammer to secure one of the latches, EE felt left shoulder popped. EE experienced severe pain in left shoulder forcing EE to stop work assignment.
EE was dismounting the D11T cat dozer when EE lost footing and step to the ground twisting the left knee.
Hydraulic tank cover was removed with crane. The cover was suspended with lifting brackets. The technician mistakenly removed the retainer bolts for the sight glass guard allowing the cover to fall onto EE's left. The cover impacted the employees foot just behind the steel toe of EE's boot.
Part 90 Miner per MSHA District 12. "This miner has participated in a medical exam authorized by the MSHA of 1977 or the earlier 1969 Coal Act. This medical examination indicates that this miner has sufficient degree of Pneumoconiosis (black lung)."
Employee was laying on their back replacing a fuel filter on a pick up truck. They loosed the line and gas spilled out onto their face. A small amount of gas got under their glasses and into the left eye. The eye was flushed & washed, they suffered no damage to the eye. Employee was prescribed medication that makes this event reportable.
Employee slipped and fell from a service truck as ee was unloading wear strips for a dozer blade. EE suffered a laceration to left hand that required stiches.
Employee was using a 1/2" drive impact to remove the 18 MM bolts. The socket used was a swivel version and the swivel joint has a small pin that backed out of the retainer. This pin caught the employee's index finger through the leather glove and EE suffered a laceration that required 7 stitches.
Employee was changing a battery terminal on a 777 Water Truck. EE was removing the insulation from the conductor with a knife. The knife slipped and cut EE's hand.
Stepping down out of the truck, slipped & fell to the ground, hurt back & elbow

Employee was installing a bearing while attempting to strike the bearing with a sledge hammer the employee missed the bearing smashing employee's finger between the hammer handle and the frame of the truck.
Working on an enclosed generator engine replacing fuel injectors and turbocharger. Work area was confined inside generator enclosure and was in awkward position to perform task.
EE was ascending onto a dozer, once stepping upward the EE felt a pull in the calf (leg) muscle.

Team member lost grip on a pneumatic torque wrench causing it to slip in hand. When it did it caused the team member's right thumb to hit the side of the wheel that the member was working on.

EE was climbing down the ladder of the rock truck, when ee missed a step, causing the weight shift to right ankle on the following step.
Employee was walking towards mobile piece of equipment, with hard hat in hand, when employee tripped and fell forward striking forehead on the frame of the mobile equipment.
Employee was removing a hydraulic hose from a threaded coupler joint, when the hose sprung back striking the employee in the right maxillary central incisor tooth, making the tooth loose.
Employee stated that while getting on the excavator "on employee's first step" while pulling self up, employee felt a pain in employee's left shoulder.

EE was walking from pressure truck washing area across the road to the fuel barrels. As he was walking across the road, he was struck by a bulk truck that was traveling in reverse. The truck was owned /operated by contract powder company (Austin Powder Company)
