Employee and 3 other men were picking up and pulling the miner electrical cable. Employee states "I bent over and it felt like a knife went into my back". Employee went down to his hands and knees; and complained of severe lower back pain. He was taken to the hospital ER and was discharged with a BACK STRAIN.

EE and another ee were removing bolts from a pump on the prep plant. EE was using a pull bar to break the bolts loose. As he strained, he felt a pain and burning to his lower abdomen and groin area. No visible injury: however he has had 2 prior hernia surgeries. He was taken to Urgent Care.

Contract coal hauling tandem axle truck left scale house road loaded and after entering haul road to his right continued to veer right driving into drainage ditch and turning truck on right side.

Two men were putting the rub rail on the continuous miner. The rub rail was dropped and struck this man's right foot. The foot was fractured, cut, bruised, and swollen. The cut required 10 stitches.

4 men were working in pairs of 2, driving pins out of the ripper chain on the miner. This man was holding the bar while the other was hitting with the sledge hammer. This man got hit in the forehead. His forehead was bruised, swollen, and cut. The cut required 7 stitches.

The Section Foreman, and 3 men were cleaning the conveyor belt rollers and structure. The belt was started and ee right hand and lower arm was mashed between the belt and a metal top roller. ee suffered a crushing injury to his lower right arm; no broken bones; bruising and swelling. An MRI was done on 3-11-14 to determine any damage to tendons and ligaments.

EE and co-worker were working on feeder jack. Co-worker threw a crib block toward the feeder; the crib block struck EE on his upper left leg causing him to fall onto the jack. EE had pain and bruising to left leg and left side. Transported to hospital for exam, x-rays, etc. No fractures. Discharged to return to full duty based on mobility on left leg.

Miner operator trammed miner into entry for first cut. He was sitting beside miner when a rock fell from between roof bolts; and struck the top and left side of his hard hat; causing an injury to his neck. His hard hat caused small scrap and swelling to the left side of his face between eye and ear. Seen at hospital and discharged day of injury.

EE was roof bolting. The glove on his left hand got caught in the chuck/drill pot. This cut his little finger (7 stitches) and fractured three fingers (little, ring, middle) on his left hand.

05/07/2012-2:00pm EE reported that he had hurt his back at 7:30am when he removed a rock from top of miner. He complained of pain in his lower back on right side; and he wanted to go to the doctor. Appointment was set for 05/09. After his appointment, he stated that he might have "pulled a muscle" and needed an MRI. Per Doctor, he returned on light duty on 05/11/2012.

EE(left handed)was using a 4lb hammer with both hands to drive a dump shaft onto mobile bridge(#3).The hammer glanced off shaft and hit sprocket; and a small piece of metal from sprocket flew into his right hand causing injury. The piece of metal entered between middle knuckles and embedded itself in top of his right hand. Puncture wound, laceration, & cut to vein; top of hand.

Prep plant lost power.Plant operator(certified electrician)found the breaker tripped at main transformer on top floor of plant.He checked area,phase monitor,etc.and reset breaker.As he was closing door, an electrical flash occurred causing slight and severe burns to left hand; slight burns to left side of face and neck; and right hand.(Locking pin fell from door onto power phase).

EE had a set of come a longs hooked to a roofbolt plate. The plate broke causing the come a longs to fall and strike him in the side of his head above his right ear. This caused an egg sized knot and cut that required stitches. No fractures or other injury.

Co-worker was operating the crane, to lift a 12x20 metal building, and set it in place. While employee was holding and steadying the building as it was being lowered, the building swung around and mashed/cut ring finger on left hand on leg of crane. 13 stitches.

Employee went to get tools out of a metal tool box near the section. He propped the lid open with a bar. He hit the bar and the lid fell onto his right hand. No broke bones; no cuts; Rt hand sore and swollen.

EE was putting last row of bolts (entry #5). He was working the levers with his right hand. He had his left hand on the drill pot. His middle finger on his left hand got pinched/cut. The tip of the finger - required 10 stitches and bone broken.

EE and two others were moving motors in the hallway of the shop/office building. As he was helping place one motor beside another, the tip of his index finger, left hand, got mashed and cut. Stitches were required.

An area of the roof fell approximately 12 feet long, 20 feet wide, and 3 feet high, in the belt entry on number 1 belt approximately 400 feet underground.

An unplanned roof fall onto the no.1 conveyor belt approx. 200 ft inby belt portail. The face was approx. 4 feet in length, 20 feet in width, and 3-5 feet in height.

EE and other ee were replacing elevator buckets on the prep plant. As he let off the comealongs, a metal elevator bucket slipped-knocking him against a metal angle brace. The metal angle brace struck his right shoulder and right side.

Employee and two others were on surface area of UG mine, loading cribs, boards and wedges into a scoop bucket. The scoop bucket was on a crib instead of the ground. When almost fully loaded the scoop bucket fell off the crib and the operator side edge of the bucket struck the top right foot of employee.

Injured employee and another miner were using a hydraulic drill drilling holes in the mine floor. The bit hung and the drill twisted, causing the metal handle to strike the inside of his left knee. Soreness, bruising and swelling, no broken bones.

EE was operating a scoop, from outside to the setion. As he was tramming thru the low top, He hit his head hard hat on the mine roof jamming his neck. He had pain to the left side of his neck.

EE was re-setting a breaker. An electrical flash occurred causing burns to his upper chest, face, and left side of his head.

EE was using a hammer to break loose a connecting link on spare piece of conveyor chain(miner). A piece of the hammer broke off, and went through skin into left wrist joint. Metal removed by Orthopedic Dr. on 7/18/07.

Fall was outby intersection for a distance of 25'long x 10' w x 5'high. Intersection had cribs and headers previous installed but had not indication of this fall outby cribbed area.

The cont miner operator was backing the miner. Inj EE was in front of the miner, moving the cable. He slipped & he caught himself by grabbing the cutter head with his right hand. His right hand got mashed between cutter head & mine roof. Broke his little finger.

He was walking with a 35lb can of oil in each hand toward the face equipment. He slipped, causing his back to twist and start hurting. He completed this shift and worked a full shift the next night. He went to the Dr. on Monday, 5/8/2006, because of continued pain in his lower back.

Employee was walking around a parked boom truck to go to the fines prep plant. He bent down to walk under the boom. He hit his head on the boom and he jerked backwards causing pain and injury to his back.

MECHANIC WAS TROUBLE SHOOTING AN ELECTRIC PROBLEM ON A SKID STEER LOADER. THE CAB WAS OPEN TO ACCESS WIRING DURING THE PROCESS OF CHECKING OPERATION OF CIRCUIT THE ENGINE WAS STARTED, VIBRATIO N CAUSED THE CAB TO CLOSE, WHICH HAD NOT BEEN LOCKED.

RUBBER MAT ON FLOOR OF CONTROL PANEL WAS TRIPPED ON AND EE SPRAINED ANKLE.

SLIPPED; LOST FOOTING WHILE GREASING TAIL BEARING. FELL WHILE STEPPING ON BEAM 36" OFF FLOOR.

THREE EMPLOYEES WERE INSTALLING A 25 HP ELECTRIC MOTOR ON THE PREP PLANT. THE MOTOR WAS BEING POSITIONED SO THAT IT COULD BE BOLTED DOWN. THIS EMPLOYEE HAD HIS RIGHT ARM BETWEEN THE MOTOR AND THE SHAFT. THE MOTOR STRUCK HIS RIGHT ARM BELOW THE ELBOW. CONTUSION, BRUISING AND SOME SWELLING RESULTED.
