Employee on the ground was retrieving a water hose while employees in a manlift were cutting metal off a structure. Employee in the manlift threw a piece of metal from the manlift hitting the employee on the ground in the back of the head and back.

Mostrando 50 de 349Employee banged EE's left shoulder on a trunk duck at the three-way in the plant. EE then tried to pick up a hose that was laying on the floor and EE heard a pop and felt a tingling sensation and pain going down EE's right arm.
Employee was drilling a 5/16" hole in a 10" I-beam with a 1/2" drill. The drill bit caught twisting EE's left thumb.
Employee was going to turn on the clean coal filter drain pump. When they walked around 74 tail piece, the employee slipped on ice on the plant floor causing them to fall. Employee was placed in a knee brace.
The employee was having some type of medical issues and fell in between the D 8 Dozer tracks and the push arm.
Was stepping down out of the back of a truck twisting left ankle on a rock.
EE exited truck to clean the wiper blades. When EE went to get back in the truck, slipped and fell on the snow covered road and struck left elbow.
Employee was driving a steel wedge between 2 16" steel flanges to spread apart. The steel wedge came out striking employee above left eye.
A trailer loaded with hay bales was discovered on fire at the end of Red Oak road, approx, 200 yards from Refuse Fill #2 (ID#1211, MD 300-172-01). There were no injuries, no property loss. Local fire department dispatched but assistance was not necessary. This event was immediately reported.
Employee went to tram the cable hoist on the 11th floor of the prep plant to bring a load in. While tramming the hoist, the mechanism broke causing the pulley to fall and strike the employee in the safety glasses causing a laceration to the left eye lid requiring 5 stitches.
Employee was in a chute pushing the plastic chute liner back in place with a porta power. When an employee above IE lost EE's grip of the ram and it fell on IE's hard hat causing IE's neck to hurt.
An EE was handing a 5 gallon bucket full of concrete into the screen that they were patching underneath. When EE let go IE caught it twisting IE's left arm/shoulder.
EE lost balance while removing metal sheeting from inside 32B drive house, falling approx. 16' to the concrete floor below.
Was putting metal in a forklift basket from the ground and when the weight of the metal caused the basket to drop down and spring back up hitting the employee in the left side stomach/rib area.
Employee was cutting a section of chute out with a 6" grinder with a cutting wheel when the grinder became bound and kicked back cutting employee's left hand in between the pinky and ring finger.
EE was dismounting off of 49-K screen when foot slipped causing a pain in left knee. EE continued to work without restrictions. Due to Covid-19, surgery was postponed until 7-8-2021
Employee was walking down stairs with a grinder in hand,turned awkwardly onto last step and twisted right knee.
Employee was running a screw gun above EE's head installing sheeting and EE's right shoulder popped out of socket.
EE slipped and fell while walking up steps in prep plant and twisted knee. EE continued to work until an MRI could be scheduled. MRI on 5/4/20 revealed a meniscus tear and other damage and was taken off work until 5/11/20. EE returned to work full duty and is currently awaiting a surgery date. However, elective surgeries are not being conducted at this time due to COVID-19.
Employee was setting up to sandblast. EE set the sandblasting nozzle on the floor and when EE stood up EE hit left cheek on a 2" ball valve causing EE's 2nd molar to break off. EE found out that the inside of the tooth was bad.
Employee felt a pain in lower left back and left leg while attaching a hoist hook to a basket. Injury became a lost workday injury on Sept. 1 2020.
Employee was cutting a piece of 1/2" thick ceramic tile when the tile became wedged between the tile saw blade and the base.
Employee hurt left wrist/hand when the employee slipped and fell while carrying a sample bag of coal down a flight of steps at the Analyzer Building.
Employee was taking down some scaffolding and when lifting up to separate the two sections they smashed the right pinky between the scaffolding and unit 92 belt structure.
Employee said that EE was picking up a piece of concrete the size of a softball and right shoulder dislocated. EE was able to put it back into place by self.
Driver was hooking toe strap back to D ring on back of Dozer and sprained left wrist.
Assisting a co-worker turn 80-c cross flow and got hand caught between cross flow and beam.
Employee was drilling a 3/8" hole in a beam to give the torch a place to start cutting from and a piece of drill shavings went into left eye.
Employee was letting some rags tied to a rope down from a catwalk under u-34 belt and caught left arm in between the bottom return roll and the belt.
EE was installing floor drain boxes into the ceiling of the 8th floor of the preparation plant. Said EE fell off the scaffolding falling 5' hitting left shoulder.
EE was walking towards the thermal dryer from the shop when another worker hit EE with the 1994 f-250 truck from behind hitting EE on right back and right elbow.
Hit head on an X-brace beam with hard hat causing neck pain.
EE was lifting a sieve up from inside the screen to spin it because of wear when EE heard a pop and felt pain in EE's right elbow.
Employee was lifting a 3 ton chain hoist (Roughneck Brand from northern tool + equipment -weighs 48 lbs.) out of the hole we were working when employee felt something pull on the left side of stomach area.
Employee was doing preparation plant maintenance repair and utilizing a hammer while extracting a retainer pin when EE miss-struck the pin with the hammer, and incurred a puncture wound injury to the top of left hand.
The employee was descending down a metal permanent ladder backwards and slipped on the second rung from the bottom landing on EE's feet twisting EE's right knee in the process.
Employee was taking well water samples using a 1" bailer. When the bailer exited the well casing it stuck the employee in forehead causing a laceration that required medical treatment, (2) sutures.
Removing bad 4" steel channel from the side of the thermal dryer to install new siding to be installed. EE was holding the opposite end that was being cut free when it was cut loose it seesawed and smashed EE's right hand thumb up against another piece of metal.
Cutting the tape off of the cord that was keeping the cord rolled up to use it and when EE went to pull the tape off after it had been cut EE forgot to close the folding razor knife and stabbed self in the chest.
He said that he was shoveling dirt off of the floor and something got into his eye. He thinks it was something from the other guys that were working behind him at the time.
Crawling between the top and bottom belts on V-98 train 10 about conveyor to remove 2 (berthold) ash analyzers that were not in service anymore and twisted his neck wrong in the process of removing them.
Driver was getting out of truck to use bathroom at wide spot top of 10 on private road. As he was exiting truck missed bottom step & fell to ground pull hid groin muscle.

He was coming down the steep incline of the catwalk from the twin silos sliding his hand down the hand railing and caught a piece of metal between his pinky and ring finger on his right hand, The metal did not cut him, it spread his knuckles apart between his fingers.
Employee found a wood board stuck in belt chute. The employee jerked on the board in an attempt to remove from the belt chute and felt a pop in his left shoulder. Employee continue to work from date of accident until January 25, 2016 when he went off work for surgery to repair left rotator cuff injury.
Snagged pant leg on a piece of metal plate in the shop and tripped while carrying a piece of 1/2"x8"x48" flat bar and fell to the floor landing/catching himself on/with his right hand. Per phone call he twisted/sprained his hand.
Driver was inbound loaded with coal came around turn #4 on private haul road. Lost control of trailer & tractor sliding into ditch and over turned truck & trailer.
Was driving truck 2242 coming onto Mettikis scale struck pole with left front steer tire causing steering wheel to spin catching right ring finger in wheel.
Employee was helping install beam when beam slipped he had a hold of and said he felt pain in lower back - Employee said he didn't want to fill out accident report - which was done - employee went to a clinic that evening & said no medical treatment was giving & recommended he have x-ray - on other page.
I am filing this report because employee called me on the 18th of December stating he had to have emergency surgery on the 17th of December stating that he thought he had come down with the flu on the 8th of December and had to take off work until the 11th of Dec & returned back to work still feeling ill & worked 4 days until they found a hernia in his stomach.
