Mostrando los 9Employee stated he had an injury, that allegedly happened on 11/19/10. We were notified at 4:20 pm in a phone conversation on 12/07/2010. Employee has been off work since 11/23/10 on a non-work related health issue, results from some of his testing while in the hospital indicated 2 fractures in his vertebra, 1 old and 1 recent? Using a sledge hammer tapping grates into place?
Employee was assisting in setting a newly constructed loadout building onto the back of the boom truck. Rigging came loose on hoist causing the loadout building to shift causing the employee to fall to the ground. The employee fell approx. 54 inches resulting in a broken right wrist and a fractured little toe on the left foot.
Sample man was walking down muddy river bank when he slipped and fell bruising his right forearm.
EE along with two co-workers were in the process of loosening the winch cable rachet to loosen hard rigging to separate barges. EE & co-worker were pushing on rachet handle to get slack when rachet slipped striking employee. (This was reported to & filed with Dept of Labor Longshoreman because it occurred on the US waterway (river).
employee and co-worker were dumping 55 gallon drums of garbage. He lifted on the barrel and pulled a muscle in his lower back
Employee was sampling/truck topping coal truck. He positioned his hand on the truck railing and his hand was punctured by a metal burr sticking up on railing.
THE CRUSHER HAD BRIDGED-OVER AND EE'S WERE TRYING TO DISLODGE LUMPS OF COAL USING A SPUD BAR. ONE EE WAS USING THE BAR & DIDN'T SEE THE OTHER EE(INJURED) LOOK INTO THE CRUSHER, SUDDENLY THE COAL BROKE FREE & THE BAR STRUCK EE ON THE NOSE.
EE WAS SECURING RIVER BARGES WHEN A LODGED WIRE ROPE CABLE SLIPPED STRIKING EE IN MOUTH CAUSING A "REPORTABLE ACCIDENT" COMPLETED SHIFT AND REPORTED TO LOCAL DENTIST OFFICE.
