Three men attempting to remove a large rock from raw coal belt. Two were lifting the rock, one was using a slate bar to pry. Slate bar slipped from beneath the rock striking one of the lifters in the left eye.

On 9/18/2019 a fire was discovered at the clean coal bin. This was an off shift and no employees were on property. Mingo 1 Plant only operates on the day shift. There was a security guard on site at the time the fire was discovered. Fire department was called and the fire was extinguished. no persons were injured.
Employee stated that EE was attempting to unlatch EE's seat belt when the seat belt buckle struck EE in the mouth chipping EE's left front tooth. The employee had EE's tooth fixed the next day, but has not missed any scheduled work due to the incident.

Employee was removing a bearing from a centrifugal dryer when the bearing slipped, employee caught the bearing resulting in a cut that required stitches.
Injured employee was removing the chute from a coal dryer and twisted his right knee.
Injured employee was operating an excavator on the slate. He swung the bucket around and caught a rock and jerked his lower back.
The driver backed up in stockpile to dump coal and turned over.

EE FEELS AS IF HE DID NOT COMPLETELY OPEN TOGGLE SWITCH AND THAT THE VIBRATION OF THE BUILDING CAUSED THE SWTICH TO BECOME CLOSED, THEREBY CAUSING THE CONVEYOR TO REENERGIZE AND HIM TO BE PUT THRU THE CHUTE AND ONTO THE RAW COAL SCREEN. THEEE WAS SEVERELY BRUISED BUT SHOULD NOT HAVE ANY PERMANENT IMPAIRMENT.
ON 3/10/90 EMPL WASINJURED HOWEVER HE DID NOT LEAVE WORK UNTIL 5/17/90. REACHING TO TURN OFF WATER VALVE & SPRAINED BACK.

AT THE END OF T2E SHIFT OTHER AND I NAME HAD JUST FINISHED SERVICING THE DOZER. HE WAS STANDING ON THE FUELING PLATFORM AND AS THE DOZER BEGAN TO PULL OUT HE STEPPED DOWN FROM THE PLATFORM. AS HE STEPPED DOW3 HIS LEG BECAME INTAGLED IN A PULL CABLE ON FUEL TANK THIS CABLE ACCIDENTLY BECAME ATTACHED TO DOZER GROUSER THUS BREAKING HIS LEG HE EMPLOYEED BY CARTER BZ5.
