Several unplanned roof falls has occurred in the Nos. 2,5,6,7,and 8 intersections one crosscut inby survey spad No. 375 the mine is in BA status and no one working underground only weekly examinations being conducted. All access to the falls will be dangered off and cribs built to prevent entry of persons.

A roof fall occurred in the return air course, N07 entry, one crosscut outby survey spad 193, above the anchorage, which did not affect ventilation or block passage of men.

There has been a roof fall outby the 001-0 and 002-0 working section. This fall is located in the No.3 entry at spad No.68, this fall measures 17'x80'x4'. The fall does not impede travel or effect the course of air. This fall will be temporarily dangered off and cleaned up at a later date.

employee was spot bolting the roof, when his glove got caught on a burr on the drill steel being used, twisting his right hand and wrist around the drill steel causing strain type injuries to the wrist and a slight fracture to the right thumb.
