Employee was loading bolts into the tray when they felt a pain in their back. Employee returned to work on 10/26/18, but was taken off work by a physician on 10/29/18 awaiting an MRI.

***We tried to report this Injury on 10/23/2018 but got caught kicked out of the MSHA site under a specific login*** Employee was loading cinder blocks into a scoop and another employee put blocks into the scoop and the blocks smashed injured employee's right middle finger.

A fall occurred at the mine on the MB #2 Panel in an intersection between the #2 and #3 entries inby the section tailpiece.

Employee stated that ee was attempting to hook a chain to the scoop bucket when the operator of the scoop inadvertently lowered the scoop bucket onto left leg just above knee.

Employee stated that as EE was kneeling near the rib on the #1 Section during the last lift of pillaring on the solid block in the #1 entry, a section of rib fell out and struck EE in the lower back.

The employee stated EE was attempting to make a belt splice when EE struck left middle finger with a hammer causing a laceration to the finger.

Employee stated that employee was attempting to lift a shield on the continuous miner when the shield slipped and struck employee on employee's left middle finger causing a fracture to the finger.

Employee stated they were attempting to replace a wheel bearing, striking the race with a hammer when a sliver of metal struck them in the left ring finger. The metal was removed later that day. Employee did not miss any scheduled work days.

Employee stated that on 10/2/2017 EE was attempting to lift a 5 gallon can of hydraulic oil when EE felt a pain in the back. The employee finished the shift on that day and did not report the incident to the supervisor. On 10/3/2017 the employee reported it to the supervisor at approximately 10:25 a.m.

Employee stated that EE was moving the miner in the outby direction. EE stopped the miner to move the CM cable. EE positioned self over top of the cable, when EE picked up on the cable, EE felt a sharp pain in groin area and lower back.

Employee stated that EE was attempting to stand up after placing crib blocks under the section feeder and felt a pain in the left knee.

EE was walking bent over (~4' coal) to the section. When ee turned head to signal oncoming equipment ee felt a strain in neck. EE developed a headache shortly thereafter. EE was taken to the hospital by ambulance.

Employee stated they had stopped the Mac-12 Mantrip near a scoop in the #4 entry to attempt to pull a cable using the mantrip when the mantrip rolled into the scoop pinching their left leg. This injury resulted in a fractured left femur.

A roof fall occurred in the Left Return on the #5 Belt line inby SS# 832 in the #1 Entry and SS# 831 in the #2 Entry.

Employee stated that as employee was walking to the #2 entry face in the last line of breaks employee stopped in the intersection at SS# 1400 to pick up a pop can. At this time a section of the inby rib sloughed off striking employee's upper back.

Employee stated that while making up bolts just outby the drill station of the bolt machine, preparing to install the 3rd row of bolts in a cut, a section of top fell out striking EE on top of EE's head/upper back.

Employee stated that as employee was tramming back toward the section feeder the shuttle car slid toward the rib where there was a crib block. The crib block slid into the shuttle car and struck the employee on employee's left lower abdomen.

Employee stated that after they had moved the bolt machine up to set up a new row of bolts, a section of draw rock fell and struck employee on the left side and left leg.

Employee stated ee was attempting to load supplies onto the RB tray when ee felt a pop in back.

Employee was attempting to put a fork back onto the CAT 930H Forklift that had fallen off. The fork was not in place on the bar where it slides and when employee attempted to shove the fork into place it fell off the bracket and struck the employee's left foot.

The employee stated ee was attempting to loosen a bolt on a CM reducer. While standing overtop of the ratchet apply downward pressure, ee hand slipped off the ratchet causing the ratchet handle to strike ee in the forehead. The laceration required 3 sutures.

Employee stated there was a shuttle car parked in the heading on the #1 Section in the #4 entry. While the employee attempted to walk between the rib and the shuttle car, a section of rib fell out and struck employee on the left ankle.

A FALL OF ROOF WAS DISCOVERED WHILE MAKING WEEKLY AIRWAYS EXAM. THE FALL WAS OBSERVED IN NUMBER 8 ENTRY IN A DANGERED OFF AREA. THE FALL IS APPROXIMATELY 20 FEET WIDE, 90 FEET LONG, AND 20 FEET WIDE. ROOF FALL WAS IN A DANGERED OFF AREA LOCATED AT SPAD 410.

MANTRIP(RAILRIDE) GOING THROUGH DOOR, RAILRIDE CAUGHT DOOR KNOCKING SUPPORTS LOOSE. CINDER BLOCKS OVER DOOR HEAD FELL STRIKING INJURED ON HEAD.

A FALL OF ROOF HAS OCCURRED 1 BRK INBY SPAD 1100. FALL WAS APPROXIMATELY 40 FEET IN LENGTH IN NUMBER 5 ENTRY.

INJURED WAS CHANGING GUIDE CHAINS ON A ROOF BOLTER. THE POT SLID MASHING EE'S RIGHT INDEX FINGER BETWEEN THE POT AND FRAME.

A roof fall occurred approx. 20 feet outby spad No. 399 in right intake entry.

Roof fall in neutral entries between No.3 & No.4 entries at break No.18 on the 3rd West Submain Bleeder panel.

A NON-INJURY ROOF FALL FELL IN THE INTERSECTION OF NUMBER TWO HEADING (SPAD#1012) AND EXTENDED FOR 50 FEET TOWARDS NUMBER ONE HEADING. FALL WAS APPROX. 6.5 FEET HIGH. NO EQUIPMENT WAS INVOLVED.

INJURED WAS INSTALLING BELT STRUCTURE OVER A BELT DRIVE. EE PLACED ONE END OF A RAIL IN THE SPREADER AND WAS CLIMBING DOWN TO PLACE THE OTHER END OF RAIL. EE PUT HAND ON THE MOTOR TO CLIMB DOWN AND THE RAIL FELL STRIKING HAND.

A roof fall occurred, while pillaring, and extended beyond the anticipated fall line and fell onto the MRS units being used for support and to break the fall.

Employee was putting belt clamps on top belt and EE felt a "pop" in right shoulder. Employee was transported by ambulance to Logan Regional Medical Center. X-rays and CT scan was performed and no damage indicated.

Employee claimed injured back while moving rail on 5/13/16. Supervisor or coworkers were not informed nor was anything reported until 5/15/16 at which time the accident investigation began.

Employee was walking to the No. 1 entry to get dinner bucket. After a short period of time, another employee went to get employee and the victim was found lying face down in the No. 1 entry. Employee had no pulse and was not breathing when found. Employee was treated an transferred by ambulance to the hospital.

INJURED WAS OPERATING MINER IN 4 LEFT. EE WAS SETTING MINER OVER FOR CLEAN-UP. EE WAS BACK OUT OF RED ZONE AT 4 HEADING FACE WHEN A SECTION OF RIB FELL STRIKING RIGHT SHIN. INJURED HAD A COMPOUND FRACTURE AT SHIN AREA.

INJURED WAS FOUND LYING ON THE FLOOR OF THE RESTROOM NEAR THE DOOR. NO OBVIOUS REASON WAS OBSERVED AS TO WHY EMPLOYEE WAS LYING ON FLOOR. INJURED WAS TREATED ON SCENE BY EMT'S, GIVEN OXYGEN AND PLACED ON BACKBOARD. WHEN ASKED WHAT WAS WRONG, NEVER STATED ANYTHING . EMPLOYEE WAS THEN TRANSFERRED TO LOGAN REGIONAL MEDICAL CENTER BY AMBULANCE.

INJURED HAD CLEANED GOB OUT OF UNCUT FACE. HE SET THE MINER OVER TO GET HIS INITIAL CUT. A PIECE OF ROCK FELL OFF OF RIB, STRIKING HIM IN THE LEG. THE WITNESS GOT THE ROCK OFF OF INJUREDS LEG.HIS LEFT LEG WAS BROKEN IN TWO PLACES.

INJURED STEPPED OUTBY TO RETRIEVE A SECTION OF LINE CURTAIN. WHEN HE BENT OVER TO PICK THE CURTAIN UP, A SECTION OF RIB 4" LONG, 5" THICK AND 9" WIDE STRUCK HIM ON HIS RIGHT SHOULDER. THIS LEFT REDNESS AND SMALL SCRATCHES TO HIS SHOULDER AREA.

INJURED WAS MOVING TRACKING CABLE UP. SETTING UP THE SPOOL OF CABLE, THE INJURED USED HIS FOOT TO CLEAR AWAY SOME RIB FLUFF. HE PUT THE SPOOL IN PLACE, AND A SECTION OF THE RIB ROLLED OUT PUSHING INJURED AND SPOOL OVER. STRIKING INJURED IN RIGHT KNEE.

EE stepped off truck - fell after and hurt back.

Injured was working on reel on shuttle car. The hinged lid has a prop to hold lid up. When he put prop in place and let go of lid, the lid fell and mashed his right hand, braking bone or bones in his hand. Injured was able to return to work on his next scheduled shift.

Injured was making a splice in a portable trailing cable. While splitting the insulation on a lead, his knife slipped and cut through his clothes into his right leg. The cut went through the skin to the edge of the muscle. Injured was not scheduled for work until after the release date of 08/02/2015. Injured was able to return on his next scheduled shift.
