During installation of new screens in the screening plant, one screen did not set in properly. The employee was adjusting the screen and cut EE's finger on the new screen edge causing a laceration. The laceration required 3 stitches.

EE FAILING TO USE PROPER METHOD WHILE DISMOUNTING EQUIPMENT. NOT TAKING HIS TIME. EE FELL STRIKING THE GROUND THIS RESULTED IN INJURY TO HIS LOWER BACK AND MILD CASE OF SCIATICA.
WET/MUDDY GROUND. EE WAS DISMOUNTING THE ENDLOADER WHEN HE SLIPPED IN THE MUD AND LANDED WRONG ON HIS ANKLE. THIS CAUSED THE EE TO STRAIN ANKLE.

HE WAS SEEN BY A DR ON THE 7TH, RESTRICTED ON W
