When employee was attempting to drive wedges out of rotor on the max cap, EE pinched EE's left pinky finger with the sledge hammer handle when swinging. this caused small fracture at last bone in pinky finger.

A Pomp's Tire Technician tried to remove a bur from a flange ring with a hammer and the piece of metal separated and imbedded in their cheek. Technician received medical attention at an ER. They received NO stitches and there was no lost time. Technician resumed normal duties.

While removing motor and motor mounts, the motor mounts attach to the catwalk, leaving a space approx 1.5'x1.5'. Employee was carrying 2'x2' piece of wood to place over space. Employee inadvertently stepped into the space scraping shin and thigh. After EE reported to supervisor EE refused medical treatment the date of this accident.

Part of a crew replacing the belt on a feed conveyor (C-397). He was working in a manlift. Attempted to push the conveyor belt into the center of the troughing roller. He felt a twinge/pull in his left shoulder. Pushed a second time on the belt, the pain was more severe and he had a hard time moving his left shoulder, i.e. the pain worsens with movement.

Operating a pit truck going to dump in hopper from muck pile, a 150-200# boulder fell off load when making turn to setup backing to hopper. Boulder was in travel portion of haul road. After dumping went to move boulder out of way manually, turned over twice and 3rd time his arm went pop, pop. Thought pulled muscle, MRI reveals tear in right bicep. Didn't report til shift end (6PM).

Repair work was being done on a tail-pulley at conveyor #C-390; while handling the tail-pulley the employee slip in some mud and the tail-pulley struck his right ring finger causing a crushing and lacerations injuries with tendon damage.

While working on bolts holding the TRAMAC Heavy Duty Excavator Style Rock Breaker Boom System (ID# P-152)from Primary Plant Rock Crusher Stationary Universal 5050 Jaw Crusher (ID# P-150) the unit P-152 was being held by Tadano TR400E Crane (UD# S-014) the load shifted pinning EE's right thumb between the railing and Chicago Pneumatic Air Wrench smashing the right thumb.

OWNER/OPERATOR OF SANDBLASTING CO. WAS SANDBLASTING AREAS OF THE NEWLY CONSTRUCTED PRODUCTION PLANT. INJURED WAS OPERATING A SELF-PROPELLED MANLIFT WEARING BOTH HIS PROTECTIVE HELMET AND RESPIRATOR. HIS HELMET MAY HAVE OBSTRUCTED HIS VISION, CAUSING A JUDGEMENT ERROR ON LOCATION OFMANLIFT'S BASKET. WHEN ADJUSTING THE BASKET, HE BECAME PINNED BETWEEN BASKET & I-BEAM, CRUSHING.
