Klein Quarry Metal/Non-Metal

Employee was using cutoff tool on sheet metal project. Work was performed outside with favorable conditions. Cutoff tool was inadvertently powered on while connecting hose to tool, making contact with left thigh causing laceration requiring 5 stitches.

EE slipped while exiting a CAT 980 end loader and caught self with the right hand causing a tear of the left bicep.

Splicing a conveyor belt and skiving the ends when the blade broke on the skiving tool. The blade stuck in the belt and EE struck it with their right hand.

A Pomp's Tire Technician tried to remove a bur from a flange ring with a hammer and the piece of metal separated and imbedded in their cheek. Technician received medical attention at an ER. They received NO stitches and there was no lost time. Technician resumed normal duties.

While removing motor and motor mounts, the motor mounts attach to the catwalk, leaving a space approx 1.5'x1.5'. Employee was carrying 2'x2' piece of wood to place over space. Employee inadvertently stepped into the space scraping shin and thigh. After EE reported to supervisor EE refused medical treatment the date of this accident.

In the process of final fabrication of heavy metal object, object tipped, striking employee's foot causing a compression injury to the right foot.

Employee was walking to the truck line from the shop building, slipped on the snow covered ground EE was walking on and fell. EE injured their lower back and has pain in the right shoulder.

Employee was replacing hoses on the dust control system on the plant and while cutting a hose the utility knife being used slipped and caused a laceration requiring 4 stitches

Employee was torching the legs off a stand EE fabricated and a glob of slag dripped onto EE's hand. EE received 2nd degree burn on EE's hand where the slag landed.

The employee was climbing up ladder in front of haul truck cab, hand slipped causing ee to lose 3 points of contact and fell to the ground landing on feet.

Employee was guiding himself up to a bin in a manlift. He said he was distracted or was not paying attention and caught his left hand and the controller between the manlift and the bin, cutting his left pinky finger.

While tightening a bolt on the drifter of the rock drill, the operator pulled the 24" box wrench toward himself. He felt a pop in his neck, resulting in severe pain when attempting to move his head.

Part of a crew replacing the belt on a feed conveyor (C-397). He was working in a manlift. Attempted to push the conveyor belt into the center of the troughing roller. He felt a twinge/pull in his left shoulder. Pushed a second time on the belt, the pain was more severe and he had a hard time moving his left shoulder, i.e. the pain worsens with movement.

Operating a pit truck going to dump in hopper from muck pile, a 150-200# boulder fell off load when making turn to setup backing to hopper. Boulder was in travel portion of haul road. After dumping went to move boulder out of way manually, turned over twice and 3rd time his arm went pop, pop. Thought pulled muscle, MRI reveals tear in right bicep. Didn't report til shift end (6PM).

Repair work was being done on a tail-pulley at conveyor #C-390; while handling the tail-pulley the employee slip in some mud and the tail-pulley struck his right ring finger causing a crushing and lacerations injuries with tendon damage.

While doing maintenance on a Euclid R-50 Hual Truck and using a 1'x1'x1' (+/-) block of wood as a step turned to step off/down from the block and rolled right ankle resulting in a sprain.

While working on bolts holding the TRAMAC Heavy Duty Excavator Style Rock Breaker Boom System (ID# P-152)from Primary Plant Rock Crusher Stationary Universal 5050 Jaw Crusher (ID# P-150) the unit P-152 was being held by Tadano TR400E Crane (UD# S-014) the load shifted pinning EE's right thumb between the railing and Chicago Pneumatic Air Wrench smashing the right thumb.

While working on replacing an electric motor at the Primary Crusher the employee apparently grabbed or pulled on the Come-A-Long causing it to fall off a beam and struck the employee in the nose, breaking it.

While performing maintenance on a conveyor and cutting flashing with a box knife, pulling the knife in the direction of his body, the employee stabbed himself inflicting a laceration to his lower left leg.

While performing maintenance work on a Euclid R-50, prior to Quarry startup, Company ID #H-09, the employee was using a grinder to prep metal for welding of a crack in the body of the above mentioned Euclid R-50, when a metal flake entered his left eye (cornea).

Conveyor Belt/chute plugged with 8"-10" material. While attempting to clear the blockage the material shifted causing a rock to strike the third finger of our mine supervisor's right hand. This resulted in a laceration to the finger requiring sutures.

WHILE MOUNTING EX550 BACKHOE EE SLIPPED ON MUD STRIKING LEFT SHIN ON EQUIPMENT STEP.

WHILE CLEANING CONVEYOR, REPLACING CLEANOUT DOOR HIS 5TH FINGER ON LEFT HAND WAS PINCHED BY THE CLEANOUT DOOR DURING REPLACEMENT. THE FINGER WAS CRUSHED (TUFT FRACTURE REQUIRING SPLINT).

OWNER/OPERATOR OF SANDBLASTING CO. WAS SANDBLASTING AREAS OF THE NEWLY CONSTRUCTED PRODUCTION PLANT. INJURED WAS OPERATING A SELF-PROPELLED MANLIFT WEARING BOTH HIS PROTECTIVE HELMET AND RESPIRATOR. HIS HELMET MAY HAVE OBSTRUCTED HIS VISION, CAUSING A JUDGEMENT ERROR ON LOCATION OFMANLIFT'S BASKET. WHEN ADJUSTING THE BASKET, HE BECAME PINNED BETWEEN BASKET & I-BEAM, CRUSHING.
