4 recordedEmployee was found on the ground in front of a Cat 777D water truck. It appears that the employee fell to the ground from the equipment. There were no witnesses to the event. Autopsy report is pending.
EMPLOYEE AND PARTNER WERE WORKING ON THE 3570 LEVEL. EE WAS FATALLY INJURED WHEN A ROCK SLAB FELL FROM THE BACK ADN UPPER RIB OF THE RAISE STATION EXCAVATION. HE AND HIS PARTNER WERE PLACING ONE END OF A STEELL CAP ON TOP OF THE SUPPORT POSTS AT THE TIME OF THE GROUND FALL.
EMP WAS LOADING ORE CARS FROM THE PONYSET FLOOR APPROX 10' ABOVE TRACK LEVEL. UPON COMPLETION OFTHE TASK THE TRAIN WAS GIVEN A "HIGH BALL" LIGHTSIGNAL TO LEAVE THE PANEL AND PROCEED TO THE DUM P. WITHIN SECONDS ANOTHER SINGLE "STOP" LIGHT SIGNAL WAS GIVEN AND THIS STOPPED TRAIN. EMPS PARTNER FOUND HIM LAYING ALOND TRAIN. SPECULATED THAT EMP SLIPPED OFF LADDER, FELL AGAINST ORE CAR.
