Resolution Mine: 4 recorded fatalities Metal/Non-Metal

A crew was conducting auger drilling operations. The assistant operator went to retrieve a 4.25" auger from the storage rack and was struck in the left hand when auger shifted in the rack, pinching EE's hand between the moving and a stationary auger. Impact resistant gloves were worn at the time of the injury. The injury was a laceration to the outer left hand requiring 8 sutures.

The employee caught a boot on the top of a short retaining wall and tripped and fell. The injured struck forehead on the forks of forklift causing a laceration that required three sutures to close.

EE was struck in the face with a long handle shovel after arguing with an employee over work duties resulting in a laceration.

The employee's finger was pinched between the sub-collar gate latching mechanism when a second employee assisted with opening the gate unexpectedly. The employee received stiches to close the wound.

Injured employee was assisting another employee tighten a drill water swivel when the pipe wrench slipped off the swivel and caused the employee to lose EE's balance and swing the wrench backward toward injured employee, striking the EE on the left cheek with the end of the pipe wrench handle. This caused a facial laceration that required wound closing sutures.

The main hoist would not pass the start of shift brake test, upon investigation the pinion brake disk was found to have oil on it. Oil was determined to have come from the adjacent hoist bearing.

At 10:51 the #9 hoist went down with a high temp reading (140° F) on #3 bearing. Employees were evacuated from the shaft using the Auxiliary hoist. The bearing cap was pulled at that time for inspection, during this time it was noticed that the oil level site glass was reading higher levels than what was in the gear box and it seemed to be air locked.

The No. 9 shaft auxiliary cage dog spring was found to be defective during the pre-shift inspection. Underground work was suspended until the new egress cage was commissioned on 5/5/2020.

TPL2 No. 9 shaft east drift pulled out existing ground support and fell. There were no persons in the area at the time the ground fell. 8' resin bolts, 6' splits sets and wire mesh failed due to working ground that started in the newly blasted face. The area had been barricaded previously due to the spalling unsupported ground.

The employee was drilling with a Jackleg when the drill steel shanked/broke. The employee received a laceration to the L-wrist when employee contacted the end of the broken steel that remained in the hole.

While working in the shaft the miner fell from height while attempting to access the bench to reattach the bail on the muck bucket. The miner landed on both feet on the muck pile and then rolled. Injuries included a hairline fracture to the left Tibia and a sprained ankle. The miner was released to light duty.

The mechanic's left thumb was pinched between the VSM bucket and the cylinder while working to install the retaining pin. The injury required sutures to close the wound.

The service hoist at the #9 shaft was placed out of service due to a Synchronize motor failure.

The main hoist PLC card failed causing the #9 shaft main hoist to trip out. The hoist was down approximately 4 hours.

The main hoist electrical drive failure caused the #9 main hoist to trip. The employees that were working underground were safely evacuated from the shaft using secondary escape systems.

The employee was welding in the shaft when employee noticed irritation in right eye. The employee received on site first aid and was taken to the Emergency room and diagnosed with a corneal abrasion. The employee received a prescription of antibiotics as a precaution to prevent a possible infection.

The #9 shaft Auxiliary Hoist UPS had an output short that was traced back to the master controller causing the system to trip out.

The #9 Service Hoist had a #1 motor tachometer failure that resulted in the loss of the hoist operation. There were no miners underground when this occurred. The Hoist was out of service for approximately 4.5 hours.

Janitorial contractor felt dizzy and nausea and rested in restroom. When standing up, EE fainted and struck head on wall. Based on this, initial 15 minute report made to MSHA Ref 01780594. Transported and treated at hospital for heat illness and head bump.

The #9 shaft Auxiliary hoist (secondary hoist) was not functioning properly due to electrical issues. The employees were hoisted out of the shaft using the primary hoist. Once repairs were completed on the Axillary hoist, the crew returned to work in the shaft.

The dywidag bolt driver fell from the recently installed bolt, deflected off of the drill, and hit the employee on the right side of the mouth. The drill helper sustained several broken teeth as a result of the impact.

The employee received a small laceration on the palm of left hand when the angle iron employee was holding unexpectedly slid downward.

Shotcrete dust entered the employee's right eye behind the foam lined safety glasses, while chipping dried shotcrete out of a concrete bucket. The employee was treated and released to full duty.

The employee received flash burns to both eyes while working in the vicinity of plasma cutting that was taking place in the shaft. The exact time of exposure was not known. The employee returned to full duty work, post treatment by an eye physician.

On the morning of 3/7/2018 the employee reported that the employee thought something was in eye. Although employee was not sure when the incident occurred employee believed it happened while working in the Shaft on 3/06/2018. The employee was treated and released by a local eye doctor.

While at the kitchen sink, an employee squashed a bee against a window pane with a sponge. The window pane broke and the employee cut the back of EE's left hand. EMT was immediately notified and evaluated the employee. Employee was transported to the hospital and received three stitches and a tetanus shot.

The injured miner reported pain to his right knee when the Auxiliary cage slowed rapidly.

The employee slipped and fell off of a ladder while descending it landing on his r-foot, left side. The employee worked without medical treatment until March 9th.

Grouting material entered the right eye around the safety glasses when the return line sprayed the employee in the face.

The injured ee was assisting the Jackleg driller by helping pull the machine out of previously drilled hole they were cleaning. The wrist of the helpers glove was caught momentarily in the rotating drill steel, pulling the glove off the L-hand. Tip of the middle finger was amputated as the glove wrapped around the drill steel and the injured pulled back to free his hand.

Employee was holding a length of 3/4" all thread with his left hand & operating the grinder with his right hand. The grinder kicked back and the cutting wheel contacted his left thumb, causing a 1/2 inch cut & severed the tendon on the back of his thumb.

While closing the elevator on the pipe the employees finger was pinched between elevator and pipe.

Laceration to little finger on the left hand due to contact between the bolt driver and the chuck on the Jackleg.

Driller was removing a piece of wood from a pallet with a sledge hammer. As he struck the pallet, a piece of wood came up and struck him in the mouth.

A galloway door dislodged from its hinges and fell 50 ft. striking the worker on the lower back.

The worker was rolling over a shaft guide. His finger was pinched between the guide and concrete floor. 4 sutures required to treat wound.

Worker stepped into a sump hole filled with water. Ankle sprain was diagnosed on 08/17/11. On 09/01/11 an avulsion fracture of the ankle was diagnosed rending the injury Reportable. This is medical only. No restricted duty or lost time.

The crew was in the process untying and moving the canopy when a sudden gust of wind violently blew the canopy over. The frame of the canopy struck EE in the left shoulder. He was taken to urgent care in gold canyon by a supervisor. EE was treated and released to return to work on light duty. He was diagnosed with a dislocated shoulder and a cracked rib.

A drill crew was doing a wet trip. The helper was using a bucket to prevent the mud from spraying out of the pipe. The bucket was positioned on the pipe that was hanging in the elevators. As the helper pushed the rod back to direct the mud away, he cut his arm on the pipe that was still in the foot clamps.

Worker lifted the handle to dump the trash hopper. The weight of the trash caused the hopper to dump quickly. The finger was pinched between the bottom of the hopper as it rotated past the handle.

Crew was pulling assembly into position when the load shifted and pinched employee's hand between pulley and the bracket.

Worker was removing a clamp from a vertical 8" pipe. Another vertical 8" pipe shifted, pinching the worker's small left finger between the pipes.

Employee's right hand middle finger tip was pinched between the verticle shaft mucker and the tower frame corner angle iron. Resulted in smashing his finger tip which required suturing.

Worker opened a 4 inch valve on a charged airline to bleed it off. The force of the air releasing out of an 11 degree elbow caused the end of the pipe to move and it struck the worker on the left side of the nose. The edge of the safety glass lens caused a lacertation to the nose.

Worker was drilling with a jackleg drill installing ground support. The air line gooseneck detached from the machine and struck the worker on the right side of the chin.

The failure of the hoist hydraulic brake system components that required alternative means of agress is an incident that is immediately reportable to MSHA as required by Part 50. The hoist was down for six days.

Worker was placing a steel tie under rails. A rail fell off a track jack and landed on the tie pinching the workers fingers underneath it.

Worker was installing resin cartridge in a drill hole. He lost balance and slipped and his left hand contacted a sharp protruding piece of wire mesh screen.

Worker attempted to pick up an oxygen bottle. He lost his balance and pinched his hand between the bottle and locomotive frame

While moving brush, employee caught his left pinky finger between two branches twisting his finger.

Worker was operating small air tugger to hoist hoses from shaft bench. Hoses hung up on shaft form. Worker placed hand on cable 6 inches in front of drum and shook it to free hoses. The cable took up slack and rotated slowly pinching worker's index finger between cable and drum.

Worker was attempting to drive a split set bolt in the back. The jackleg drill slipped and the worker lost his balance and his forehead contacted the rockbolt plate hanging on the split set bolt. Laceration requiring sutures resulted.

Water truck driver was task training another employee on the 8000 Gal Water Truck. He was trying to situate himself on the passenger's seat. He put his hand on the door jamb. When the trainee shut the door he closed it on the operator's thumb. He needed stitches on his thumb and also chipped the tip of his thumb bone.

Worker was drilling with jackleg drill to install rock bolts. While pulling drill back out of a hole, the pressurized air line disconnected from the drill and struck the worker on the forehead.

It was a windy day and saw dust and debris went into left eye of employee. Employee was wearing safety glasses at the time of incident. Eye had abrasions on it.

Worker was moving drill cores in the core warehouse and felt pain to his right leg. The next morning the leg was swollen. Employee went to the hospital and it was identified as an insect bite, was prescribed antibiotics for the reaction and returned to work full duty.

Employee was logging core data into computer and needed to break core sample to view inside. Worker broke the core over an angle iron tray. When doing so, broken jagged end punctured his right hand between fore finger and thumb causing a laceration. Injury required 6 stitches to close.

Public Electric utility crew was performing maintenance on electric transmission tower. Tower was located in remote area separate from mine. While descending the hill on which the tower is located, Employee slipped and fell down steeply inclinded surface 45 ft.

Driller helper was attempting to tighten water swivel on rod string when driller engaged lever with incorrect selector switch setting. Driller rotated rods, forcing 36" pipe wrench into helpers hand and compressing forearm between wrench handle and mast support strut. Report is late due to discussion with MSHA regarding proper jurisdiction over drill site.

THE INJURED EMPLOYEE WAS LOWERING A STEEL GRATING SECTION WEIGHING 70 TO 90 POUNDS.

Employee was lowering a sheet of tin off sampling building. Tin slipped from employee's grip and when he went to regain control of the sheet, the corner cut his left forearm.

Employee was stepping off of a trailer, upon descending he stepped on a 2 1/2" penny nail, causing puncture wound. The employee placed the board down when he stepped on the exposed nail.

EMPLOYEE STATES WHILE MOVING PIPE ON DERRICK, HE GRABBED JOINT OF PIPE AND LACERATED HIS LEFT RING FINGER ON THE ROUGH, SHARP EDGES.

THE HELPER WAS CUTTING A GATORADE PLASTIC BOTTLE WITH A RAZOR KNIFE WHEN HE CAME TO THE NECK OF THE BOTTLE HE NEEDED MORE FORCE TO CUT THE NECK. HE WAS PULLING THE KNIFE TOWARDS HIMSELF WHEN I T BROKE THROUGH HE PUSHED THE KNIFE INTO HIS LEFT LEG.

A TEMPERATURE SENSOR SWITCH ON THE #9 HOIST DRUM BEARING LOOSENED UP, CAUSING THE HOIST TO SHUTDOWN. THE HOIST WAS DOWN FOR 1 HOUR DUE TO ELECTRICIAN CALLOUT. THE SWITCH WAS RESET AND RETIGHTE NED. THE LOOSE SENSOR SWITCH CAUSED THE SYSTEM TO HAVE A FALSE READING CAUSING THE SHUTDOWN OF THE HOIST.

EE SLIPPED AND FELL, STRIKING HIS LEFT ELBOW ON THE RIB.

EMP WAS BOLTING THE BACK WHEN A SMALL ROCK FELLFROM THE BACK. THE ROCK STRUCK EMP'S RIGHT FOREARM CAUSING A LACERATION THAT REQUIRED 15 STITCHES.

A FALL OF GROUND OCCURRED IN THE 3710 STOPE PANEL 20 NORTH. THE AREA AFFECTED WAS APPROX. 11' WIDE AND 30' LONG.

EE WAS RIDING ON THE LAST CAR OF THE TRAIN, HE WAS DISTRACTED AND STRUCK HIS HEAD ON A OVERHEAD PIPE.

WHILE EE WAS REMOVING POUR VALVES FROM SAND LINE, ONE OF THE KIND NIPPLES STRUCK HIM ON THE LEFT FOREARM CAUSING A SNAG CUT THAT REQUIRED 3 STITCHES.

A ROCK FELL AND STRUCK EMPLOYEE'S LEFT HAND AS HE WAS PULLING DRILL BACK FROM A HOLE HE HAD JUST DRILLED.

THE STEEL STRAP AROUND A PALLET OF SPLIT SETS WAS CUT AND LEFT IN PLACE WHILE EMP WAS UNLOADING THE SETS. WHILE PERFORMING THIS TASK, THE EMP'S RT LEG WAS CUT WHEN HE CAME IN CONTACT WITH THE SHARP EDGE.

JACK FAILED AND VEHICLE CAME DOWN ON VICTIM.

FELL WHILE BREAKING CASING JOINT AND LANDED ON PIPE WELD. FRACTURED RIGHT RIB & BRUISED RIGHT LUNG.

DURING A POUR OF A PANEL, THE LOWER RIGHT SIDE OF THE FILTER BROKE CAUSING BACKFILL MIXTURE TO BE SPLASHED INTO EMPLOYEES EYES.

EMPLOYEE WAS PICKING UP ROOF BOLTS AND EXPERIENCED BACK PAIN.

EMPLOYEE WAS STEPPING OFF AN LHD AND TWISTED HIS AKNLE.

WALDEN MINE PUMP FELL OVER AND HIT EMPLOYEE IN BACK OF HEAD & NECK & SHOULDER AREA AS EMPLOYEE WAS PICKING COUPLING OFF THE GROUND.

EMPLOYEE WAS DRILLING A BENCH ROUND USING AN 8FTSTEEL AS HE PULLED THE DRILL OUT OF THE HOLE HE FELT PAIN IN HIS LOWER BACK

EE WAS CUTTING HOSE WITH A UTILITY KNIFE. KNIFE SLIPPED OFF HOSE AND CUT BACK OF EE'S LEFT HAND.

EE WAS CLIMBING A LADDER AT THE 3700 LOAD STATION WHEN HE FELT PAIN IN HIS RIGHT KNEE. ON 9-12-95, EE WENT BACK TO DR. AND HAD SURGERY WHICH MAD THIS INCIDENT A REPORTABLE ACCIDENT.

RIDING ON BACK OF ELE MOTOR WHEN TRAIN DERAILED SLAMMING EMPLOYEE INTO BACK OF MOTOR.

EMPLOYEE WAS EMPTYING AN ARE CAR AND CAUGHT HIS FINGER BETWEEN WIRE ROPE AND ARE CAR CAUSING FINGER TO BE AMPUTATED.

EMPLOYEE WAS SAWING A PIPE WITH A HACKSAW THE SAW SLIPPED OFF THE PIPE & CUT EMPLOYEES FINGER REQUIRING (5) STITCHES

AN ELECTRIC COIL BURNED OUT CAUSING THE NO 9 HOIST TO BE SHUT DOWN FOR ABOUT 3 HOURS

EMPLOYEE WAS GOING TO WORK, TWISTED HIS ANKLE IN THE PARKING LOT.

EE WAS BREAKING BOULDER ON THE GRIZZLY WHEN HE LOST HIS FOOTING CAUSING HIM TO FALL, STRADDLING THE GRIZZLY I-BEAM.

EMPLOYEE WAS STRUCK ON THE HEAD WHEN A ROCK FELL FROM THE BACK, WHILE HE WAS INSTALLING ROOF BOLTS.

EMPLOYEE CAME IN CONTACT WITH SHOTCRETE MATEIAL WHICH CAUSED CHEMICAL BURNS.

EE WAS STEPPING BACKWARD FROM ONE DECK LEVEL TO A LOWER DECK. HIS LEFT FOOT CAUGHT THE EDGE OF THE LOWER DECK AND HE FELL OFF THE LOWER DECK AND CAUGHT HIS LEFT LEG UNDER HIMSELF. THE LOWER DE CK WAS 14" HIGH.

EMPLOYEE WAS LIFTING A POST AND FELT PAIN IN GROIN AREA.

EMPLOYEE REPORTED THIS ACCIDENT ON 4-25-95 AND STATED HE WAS USING A BLOWPIPE AND GOT SOMETHINGIN HIS EYE.

81 WINZE HOIST ROPE WENT SLACK 10 TO 15' BELOW 3200 LEVEL, HOIST HOPE & SHAFT INSPECTED. HOIST PUT BACK INTO OPERATION AT 10:00 AM. NO EMPLOYEES ON CAGE WHEN THIS OCCURRED.

SMOKE ENCOUNTERED IN NORTH COUNTRY DRIFT, MINE EVACUATED & MINE RESCUE TEAM ACTIVATED. MINE RESCUE TEAM FOUND THAT A CONTRACTOR'S JACKET HAD CAUGHT ON FIRE [LITE CIGARETTE SUSPECTED] FIRE WAS OUT WHEN TEAM FOUND JACKET.

EE TRIPPED OVER THE CEMENT SLAB AT THE DRY DOORWAY.

WHILE DRIVING NAILS, EMPLOYEE STRUCK LEFT THUMB RESULTING IN A FRACTURE.

WORKER WAS UNLOADING NP DRILL RODS 10FT) FROM A RAIL FLAT CAR TO THE DRILL DOCK WHEN HE SLIPPED & FELL AGAINST THE END OF THE RODS

WHILE MOVING A DRILL MACHINE EMPLOYEE LOST HIS BALANCE & FELL CAUSING THE MACHINE TO FALL ON HIM INJURING HIS LOWER LEFT LEG.

EMPLOYEE WAS WALKING, SLIPED AND FELL ON SELF RESCUER AND INJURED HIS BACK. INJURY DIDN'T BECOME REPORTABLE UNTIL THE REPORTING DATE BECAUSE OF LOST TIME.

BLASTING GAS INUNDATED THE 3700 2XC DRIFT WITH CO WHICH WAS PICKED UP BY THE CO MONITORING SYSTEM IN THE MINE. CREWS WERE WITH HELD UNTIL DRIFT WAS PROPERLY VENTILATED AND AREA INSPECTED BY TH E MINE RESCUE TEAM AND MSHA.

EE WAS DRILLING WITH JACKLEG, THE MACHINE MADE A SUDDEN MOVE WHICH PULLED HIS RIGHT ARM AND SHOULDER.

WHILE SHOTCRETING EMPLOYEE FAILED TO WEAR HIS RESPIRATOR AND THE CEMENT DUST BURNT HIS NASAL PASSAGES.

EMPLOYEE WAS DRILLING OVERHEAD HOLES WHEN SOME MUD CAME OFF OF DRILL STRIKING HIM IN THE RIGHT EYE.

DUST BLEW DUST IN EMPLOYEE'S EYES.

EE WAS RIDING ON THE BACK OF A MOTOR. HE LOOKED DOWN AND THE AIR DOOR FRAME STRUCK HIM ON THE HEAD.

A WIRE STUCK THROUGH HIS BOOT, WHICH ALLOWED SLIME WATER TO RUN INTO BOOT CAUSING A BURN.

EMPLOYEE WAS INSTALLING A CAP DUST BLEW INTO HISEYES AND A FOREIGN OBJECT BECAME IMBEDDED.

EMPLOYEE WAS WETTING DOWN A MUCK PILE, WATER SPLASHED INTO HIS EYE AND A FOREIGN OBJECT BECAME IMBEDDED.

EMPLOYEE WAS BOLTING THE BACK, A SLAB FELL AND STRUCK EMPLOYEE ON FOOT.

EMPLOYEE WAS BOLTING WIRE TO THE BACK, A ROCK BROKE LOOSE FROM THE BACK STRIKING HIM ON THE HEAD.

EMPLOYEE UNCOUPLED A RAIL CAR, IT ROLLED FORWARD AND STRUCK HIS FOOT.

A FALL OF GROUND OCCURED IN THE 3833-C STOPE AREA AT THE CROSSCUT SECTION AT 3833 RAMP.

EMPLOYEE WAS WALKING DOWN 3700 RAMP, SLIPPED & SPRINED HIS LEFT ANKLE.

EE WAS MOVING A LHD TIRE, HE LOST CONTROL OF THE TIRE WHICH FELL ON HIS FOOT.

EE EXPERIENCED PAIN IN HIS BACK WHILE DRILLING WITH A JACKLEG.

EMPLOYEE WAS PULLING A 14' BOOM FROM THE FACE WHEN BOOM FELL STRIKING HIS RIGHT INDEX FINGER REQUIRING STITCHES.

EE STRAINED HIS RIGHT SHOULDER WHEN HE TRIED TO KEEP DRILL MACHINE FROM FALLING OVER.

YGROUND FALL IN THE 3590-C STOPE, 35-W PANEL AND THE NORHT SOUTH CROSSCUT ON 11-2-94 ABOUT 6PM.

INJURED WAS WALKING IN DRIFT, FELT HHIS KNEE LOCK UP AND FELL.

EE WAS OPERATING A LHD IN THE 3722-D STOPE AREA, PANEL 14S, WHEN A ROCK FELL FROM THE BACK STRIKING EE ON THE LEFT INDEX FINGER RESULTING IN A FRACTURE.

EE INJURED HIS RIGHT WRIST WHEN THE SHOVEL HE WAS USING HIT A LARGE ROCK. THIS ACCIDENT HAPPENED IN THE 3644-C AREA OF THE MINE.

HAULING ORE TO DUMP, MOTOR JUMPED TRACK ADN CAUSED EMPLOYEE TO JUMP AROUND I MOTOR, HURTING HIS BACK.

APPROX 30'BY8' OF SANFILLED BACK CAVED INT EH 3744-32 NORTH PANEL. NO INJURIES RESULTED FROM THIS FALL.

WHILE DRILLING ROCK BOLT HOLES, EE WAS POSITIONING FOR LEG AND TWISTED HIS LEFT KNEE.

YEMPLOYEE DRIVING RAIL SPIKES FELT NUMBNESS IN LEFT HAND. RESULTING NJURY WAS HAIRLINE FRACTURE IN LEFT ARM.

APPROXIMATELY 20 LINEAR FEET OF SANDFILL CAVED 5 TO 7 FEET IN THE 3744-C 22 NORTH PANEL. NO ADDITIONAL FALL OF GROUND SINCE INITIAL INCIDENT. INCIDENT OCCURRED NEAR ROCK/SANDFILL SEAM.

EE SLIPPED WHILE MOVEING TOWARD FACE, BENT RIGHT KNEE IN AWKWARD POSITION.

AT APPROX 6:30 PM ON 9-8-94, A FALL OF GROUND WAS DISCOVERED IN THE 3680 STOPE, PANEL 1S. THE MUCK PILE ENCOMPASSED AN 8' X 12' AREA, APPROX. 5' HIGH. THE MUCK PILE WAS OBSERVE TO CONTAIN SEVE RAL SPLIT SETS. EES WERE KEPT CLEAR OF THE AREA.BY 9"00 PM, ADDITIONBAL GROUND HAS FALLEN IN THE AREA.

EMPLOYEE FELT PAIN IN STOMACH AREA WHILE LIFTING SECTION OF 4" PIPE.
