A roof fall was found in the return aircourse of South Mains ""B"". Fall is approx. 40' long by 18' wide and 8' high. Area was bolted with 8' Install III bolts and resin with bacon straps and wire screen. Air not affected. Location of fall is between 25-A xcut and 26 xcut. Cause of fall was deterioration of beam created by roof bolts due to weathering of the roof.

The elevator quit working due to going up into the overtravel stops. The PLC would not allow the elevator to back out of this top stop. It was found that dirt got in a contactor. The elevator was done for more than 30 minutes but had no damage. Elevator was back running at 3:30 pm.
ROOF FALL HAS OCCURRED AT THE #43 Xcut ON THE SOUTH MAINS ""A"" BELTLINE. FALL IS APPROX. 25 FT L X 48 FT W BY 8 FT H. THE AREA WAS BOLTED WITH 8 FT INSTALL II BOLTS & RESIN W/METAL STRAPS & WIRE SCREEN. FALL DID NOT AFFECT ESCAPEWAYS/VENT. CAUSE WAS FAILURE OF BEAM CREATED BY ROOF BOLTS. THE ELEVATOR WAS OUT OF SERVICE FOR MORE THAN 30 MINUTES DUE TO BOTTOM DOOR SWITCH HAD GROUNDED WIRE & TOP BACKUP SLOW DOWN SWITCH HAD AN EXPOSED LEAD IN CONDUIT. CAUSE OF THIS WAS USEAGE OF ELEVATOR. THIS WAS NOT AN ACCIDENT BUT PROBLEM WITH THE ELEVATOR USE FOR MORE THAN 30 MINUTES. BACK IN OPERATION AT 4:15AM ON 2/9/05.
