Employee was loading ram jack onto shearer, while repositioning the ram jack it shifted catching finger between the shearer and ram. Lack of employee concentration was the cause of this incident. EE was using 4lb hammer to make belt splice on 12/4/16,during work activities EE states they strained wrist. EE worked w/o restriction until seeking med attn on 2/28/17 becoming a RLT on 3/1/17.Cause of the incident was lack of EE concentration. While EE was bending over to pick up water hose they strained their lower back. EE worked 8/31/16 and became a RLT after seeking medical attention on 9/1/16. Lack of proper lifting technique was the cause of this incident. Employee was removing dollies from monorail on a previously worked out panel, while removing dolly from end of monorail, finger was caught between dolly and monorail.
