Accident type, without injuries

A roof fall was found on a pre-shift exam. of the 4-1/2 in-active section. The fall was at the 2nd intersection of the #8 room of the #5 entry. Fall is approx. 20'L. by 30'W. and above the anchor point of the 6' resin bolts. Cause of this fall was clay veins & slickenslide roof. Compliance with rules/regs not a factor.

Accident type, without injuries

Mine examiner found a roof fall between No. 25 and 26 x-cut in the No. 1 entry of South Mains B return aircourse. Fall is approx. 15 feet long by 17 feet wide and 8 to 9 feet high. Cause of this fall was deterioration of the main roof due to weathering. Fall did not obstruct ventilation or escapeways and was re-supported for travel.

Accident type, without injuries

A new roof fall was found int he #1 entry of South Mains ""B"" return aircourse between 23 and 24 x-cut. Fall is approx. 18'wide by 8-9""high by 20-30'long. Area bolted with 8' Install III bolts with resin assisted and some floor to roof support. Cause of this fall was deterioration of the main roof due to weathering. Fall did not affect ventilation or escapeways.

Struck by falling object

Employee bent a roof bolt installing it in the roof and as he was attempting to get it out of the roof the bolt head came out of the drill head chuck striking him on the wrist. Cause of this injury was safety awareness, compliance with rules/regs was not a factor, miner proficiency and attitude was a factor, protective equip was not a factor.

Employee and another co-worker were changing out a bad top roller cradle on the SM ""D"" belt. It jammed up against the side of the bed rail and jarred his left shoulder. Cause of this injury was not having enough help and safety awareness. Caught in, under or between a moving and a stationary object

Employee was loading a 10 foot piece of 85-lb track rail onto a supply car. The rail shifted and he caught his finger between the rail and the edge of the supply car. Struck by flying object

Using a pick hammer to pound on a fish bolt plate to get it out, a piece of metal off the hammer head flew striking and lodging in his chin area. Cause of this injury was using the wrong tool for the job and safety awareness. A cement block fell from a wing wall and struck him on the side of the foot while seating in a chair at the waiting area of the slope bottom. Cause of this injury was safety awareness. Employee was traveling over a belt overcast when he fell through the top landing on the mine floor below. Cause of this injury was damage to the overcast occurred and no-one dangered off the area. Injured was walking down an overcast ramp, lost his footing, slid to the bottom and twisted his left ankle. Resulted in torn ligaments.

Reaching to get a roof bolt off the machine as his co-worker was moving the machine for the next row of installation. Victim's foot was under the tire and the machine ran over it. Supervisor was sawing a wood post to set and he cut the post crooked. He picked up the post to slam it down to break it off and he strained his back. Employee was pulling drill steel out of the top when he hit his right wrist on a bent down bacon strap causing a laceration to his wrist. Rules and regulations is not a factor. Employee was riding in a track mantrip and the mantrip jumped track at a curve, he claims to have hurt his neck. Cause of this injury was excessive speed.
