Accident type, without injuries

While starting to drive a slope. The first shot was taken and bolted. After the first shot was bolted a second round was drilled and shot. The foreman noticed the brow working, it was decided to keep employees back and watch the brow the brow fell along with the bolts that had been installed.

An ignition occurred on the 9 West longwall face at #107 shield. A orange flame was observed at the mine roof that lasted about one minute around the tailgate shearer drum. The area was checked for methane which was .015% CH4. The shearer was stopped and power knocked. Cause may have been worn bits on the shearer drum.

Struck by falling object

The victim was leaning over a base lift jack housing. The molded casing catastrophically failed striking the victim in the chest from unknown/undetectable reasons. Cause was catastrophic failure of the housing. Compliance with rules/regs, protective items not a factor, miner proficiency may be a factor, mining equip/systems may be a factor.

Had left hand placed on long arm of drill was bringing drill back from the roof to add another steel. Finger got pinched between long arm and rotation hose fitting cutting tip of finger. Employee was struck by a track jacking bar lacerating the right side of his head, while trying to jack a mantrip rail bus back on the track. PPE was not a factor, mining equipment/system was not a factor, compliance rules/regs was a factor, miner proficiency was a factor.

Walking down long wall face and lower back gave out. PPE was not a factor, Compliance of rules/regs not a factor, mining equipment/system not a factor, Cause is not being aware of mine floor conditions when walking on long wall face.

EE foot slipped on the rail as purat moved forward his foot got caught in between the ramp structure.

Fall to the walkway or working surface

EE was assisting the Paurat as it was backing up the slope. Hoist rope got hung in the bridge conveyor when it broke free, hitting EE in the shin, flipped him up and onto the ground.

While helping fellow roof bolting partner, trying to bend a roof bolt he dropped the drill pot down and caught my right foot between mine floor and drill pot. PPE was not a factor, compliance rules/regs not a factor, mining systems/equipment was a factor, miner proficiency was a factor. This person worked after the incident and is now off 8/10/2011 last day worked.

Walking toward elevator slipped on uneven mine floor falling and putting pressure on right ankle. PPE was not a factor, compliance rules/regs was not a factor, mining equipment/system was not a factor, miner proficiency was a factor.

Over-exertion in wielding or throwing objects

Pounding clip ties on track rail felt sharp pain in my groin area. PPE was not a factor, compliance rules/regs not a factor,mining equipment/system not a factor, miner profiency was factor.

While taking drill steel out of roof bacon strap, it became separated from the first inby steel in the roof. When it came out of the hole and swung around striking him in the left ear and head. Employee stated he tripped while walking along a belt conveyor and as he reached out to catch himself, his left arm was caught between the belt and a roller.

Struck by falling object

Spotting headgate shearer drum bits rock fell near tip of shield bouncing off of shearer drum pushing his head down on drum causing him to bite tongue and put strain on neck. Compliances rules/regs not a factor, PPE was not a factor, mining equipment/system was not a factor, miner profiency was a factor.

Employee was lifting fourteen foot post over high side of shield car, his arm got caught under post and hit top side of shield car. PPE was not a factor, compliance of rules/regs were not a factor, mining equipment/system was not a factor, miner proficiency was a factor.

Pulling on super stucco hose on a long wall power move, the hose slipped out of hand and the end hose fitting hit left foot above meta-tarsal on boot. PPE was not a factor, compliance rules/regs was not a factor, mining equipment/system was not a factor, miner proficiency was a factor.

The injured was at 114 shield pulling in 115 shield, he heard rock fall between shields behind him. When he turned, the pontoon from 115 shield caught his right foot. It pushed on the side of his boot smashing the meta-tarsal to the top of the foot. PPE was not a factor, compliance rules/regs was not a factor, mining equipment/system was a factor, miner profiency was a factor.

Struck by falling object

While hoisting an accelerator jar, the chain at the end of the hoist cable slipped and allowed the accelerator to fall. The bottom end of the accelerator was protruding below the work deck. As it fell the bottom end was elevated and struck the ee on the right shoulder. The ee sustained a contusion to the right shoulder.

During a Part 50 audit, it was determined this injury was reportable. Employee twisted his back while working on a shield in a crunch position. Cause of this injury was proper body positioning. Compliance with rules/regs, mining equip/systems not a factor. PPE (Lack of Back support) and miner proficiency may be a factor.

During a Part 50 audit, it was determined this injury was reportable. Door handle broke free of door when employee kept twisting it, hurt wrist. Cause was broken door handle and inattention to task. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

Contact with hot objects or substances

During Part 50 audit it was determined that this injury was reportable. Employee was removing a cutter shaft on shearer and hot fluid contacted his hands causing 2nd degree burns. Cause of the injury was not releasing all pressure. Compliance with company rules a factor, PPE a factor, miner proficiency may be a factor, mining equip/systems not a factor.

Hoisting grating in the Prep Plant using a rope and a sheave block, was trying to adjust sheave block and got his glove caught getting his fingers into the sheave. PPE was not a factor, compliance of rules/regs mot a factor, miner proficiency was a factor.

Picking metal out of section feeder, a rock fell down striking left small finger cutting the finger through the glove. PPE was not a factor, compliance of rules/regs not a factor, mining equipment/system not a factor, miner proficiency was a factor.

Flash burns (welding)

During a Part 50 audit, it was determined this injury to be reportable. Employee was standing beside someone welding and received a welding flash to his eyes. Cause of this injury was looking at the welding process. Compliance with rules/regs, mining equip/systems not a factor. PPE and miner proficiency may be a factor.

Fall to the walkway or working surface

During a Part 50 audit, it was determined that this injury was reportable. Employee was pulling cable off a shuttle car and slipped, injuring his back. Cause was mud and uneven bottom with inattention to task. Compliance with rules/reg., PPE, mining equip./systems not a factor, miner proficiency may be a factor.

Walking by headgate drive on the long wall a rock flew out the panline chain and struck him lacerating his lip on his face. PPE was not a factor, compliance of rules/regs not a factor, miner proficiency was a factor.

Struck against stationary object

Cutting hole in mine belt for winch cable, the knife blade broke. When the blade broke my left hand went forward cutting it on the broken blade left in the mine belt. PPE was not a factor, compliance rules/regs were not a factor, mining equipment/sytems was not a factor, miner proficiency was a factor.

Prying a loose rock down from mine roof, rock pushed back on bar and pinched his right hand between bar and steel beam. Lacerated hand between index finger and thumb. Pulling cut metal banding material from bacon straps, the end flipped out striking left forearm. Cut left arm. PPE was not a factor, rules/regs not a factor, mining equipment/system not a factor, miner proficiency was a factor.

Moving a fan from air compressor, while sliding fan into Bobcat bucket, fingered got pinched between bucket and fan housing. Lacerated middle finger. Cause of the injury was safety awareness of the employee, compliance with rules/regs not a factor, ppe was not a factor, mining equipment/system was not a factor, miner proficiency was a factor.

Yellow wire holding bacon strap on ATRS pad broke causing bacon strap to hit fore head above right eye. Cutting fore head. PPE was not a factor, compliance rules/regs not a factor, mining equipment/system was not a factor, miner proficiency was a factor.

Contact with hot objects or substances

During a part 50 audit, it was determined that this injury was reportable due to 2nd degree burns. Employee was loosing a snap ring on a cutter shaft and hot oil blew out onto his hand/wrist area. Cause of this injury was not releasing stored energy. Compliance with company rules, PPE (Gloves) and miner proficiency a factor, mining equip/systems not a factor.

Looking over top crib on belt line and crib fell down smashing left index finger. Compliance rules/regs not a factor, PPE was not a factor, Mining equipment/system was not a factor, miner profiency was a factor.

Putting on winch cable on scoop while winding on cable glove got caught in crosby clamp and cable, lacerating left small finger. PPE was not a factor. Compliances rules/regs was not a factor, mining equipment was not a factor, miner profiency was a factor.

Struck by flying object

During a part 50 audit, it was determined that this injury was reportable due to embedded object. Employee was changing bits and a piece of the hammer (sliver) flew off embedding into his forehead above his eye. Cause of this injury was using pick hammer as a tool. Compliance with rules/regs, mining equip/systems not a factor, PPE (Faceshield), miner proficiency may be a factor.

EE was taking off choker from I Beam for rail supports. Frozen choker snapped from beam edge when EE moved it. It hit him in the mouth and chipped tooth.

Unhooking lowboy from locomotive while pulling pin from draw bar the pin moved pinching the end of left ring finger. Cause of this injury was safety awareness of the employee. Compliance with rules/regs not a factor, ppe was worn so not a factor, miner proficiency a factor.

Installing roof bolt, roof bolt drill wrench slipped off bolt head pinching finger between wrench and roof bolt plate. Cause of the injury was safety awareness of the employee.
