Accident type, without injuries

A fire was discovered at the hourly bath-house trailers, smoke was seen coming out of the roof vents by a worker. Fire was investigated and found in the ceiling area, fire dept. was called and fire was put out at approx. 4:00 pm. Trailer is a total loss and no injuries were involved. Cause, electrical short in the wiring. Compliance/rules, PPE, Mining equip/systems not a factor.

Over-exertion (Not Elsewhere Classified)

Employee was helping load supplies stepped off of supply car on uneven bottom and sprained his right ankle. PPE was not a factor, mining equipment was not a factor, mining systems was not a factor, compliance rules/regs was not a factor, miner proficiency may have been a factor.

Fall from machine

The injured employee stated he was unloading roof bolts from a car when he slipped and fell off the car catching himself with his right arm causing pain to his right shoulder area.

Struck against a moving object

While putting scoop on charge employee came into contact with a belt cross over in an adjacent xcut.The cross over came into contact with the seat bending it forward, cause of injury was employees lack of attention to his surrondings.PPE/mining systems was not a factor, mining equipment was a factor, compliance rules/regs was not a factor, miner proficiency may have been a factor

Miner operator was pinned between standoff cable handler of a continuous miner and coal rib in #1 entry of 5 west development section. Cause of incident was miner operator not paying attention. Struck by powered moving object

Employee was standing in the middle of the offside of roof bolter, during bolter reposition his foot was struck by the front tire. Fall to the walkway or working surface

Employee was walking thru x-cut when he tripped and fell landing on his left side, accident occurred on 5/19/14 but didn't become a RLTA till 5/24/14. Struck by falling object

Employee was bolting in the face of 1 left, while installing inside bolt of first row the immediate mine roof fell with debris coming back toward the operator striking employee in the left lower leg. Employee was making a mechanical splice on belt, while swinging a 3lb hammer to drive required rivets he strain/sprained his shoulder. Employee was removing clip ties from track rail, while removing last clip tie track rail came together catching his left lower leg between the rail. Employee was removing a broken cutter shaft from the shearer; snap ring was removed causing hot oil to come into contact with the employees left cheek, hands, wrist and lower arms causing burns. Employee standing near #4 shield when a rock laying on the mine floor pinched his foot between the rock and the panline. Cause of the injury was the rock being near the movenment of the shield. Employee bent over to lift a battery cell and strained his back. Cause of injury improper lifting and not getting help. This incident became reportable as of 02/11/2015 due to employee receiving physical therapy.

Employee was attempting to reinstall an isolation block to the shield when it slipped from his hands striking him along the left side of his head. Employee was cutting belt structure stands when a piece of metal got into his eye. Employee did not seek medical attention until 8/04/2014 where he then received eye drops.
