Employee was roof bolting on long wall face, dropped steels from roof the steels got stuck causing a bind causing the drill mast to jump. Put hand on steels while trying to remove steels, they jumped pinching fingers between mast and steel. Employee was disconnecting the draw bar on a lowboy that was coupled to a locomotive motor, finger got caught under the hitch pin and was mashed. PPE was not a factor, compliance rules/regs was not a factor, mining equipment/system was not a factor, miner proficiency may be a factor.

Employee attempting to align a drill steel to the other, he hit the wrong lever on the bolter (tilt lever) and pinched his finger between the two drill steels. Cause of this incident was the inattention of this employee and having the drill steel in the drill pod to align. Struck against stationary object

Pushing loaded lowboy through track switch, which was located at an overcast. As he was passing through watching the car, he misjudged the distance from the overcast. Striking his head and causing a laceration. PPE was not factor, compliance rules/regs was not factor, mining equipment/system was not factor, mine knowledge was a factor.

Drilling hole for roof bolt, he was lining up drill steels, he moved the feed lever rather than the rotation lever. The steels pushed together pinching index finger. PPE was not a factor, mining equip/system was a factor, compliance rules/regs was not a factor, miner proficiency was a factor.

Injury occurred while contract labor was watching trailing cable on a shuttle car. The operator of the shuttle car reeled the cable and caught the leg of the injured took his feet our from under him he landed striking the back of head. Injury occurred while this contractor was installing a top center belt roller on a belt line. While leaning over belt bed rail to install roller he felt a pop in left rib area. Cause of injury was improper position and overexertion. Employee was moving tailgate shields his hand was on top of panline rail. Rock fell when it slid off of top of shield striking middle finger on right hand. PPE was not a factor, compliance rules/regs was not a factor, mining equipment/systems was a factor, miner proficiency was a factor.

Employee was stacking belt structure in supply yard, there was a ram jack that was entangled in structure when trying to remove the ram jack out shoulder popped. Lost Time Injury.

Fall to the walkway or working surface

Employee tripped on a grade and fell on his knee. The results of the MRI along with a medical opinion dictated on 10/31/2012. The diagnosis was tearing of the meniscus and the employee is expecting to lose time.

Employee was removing a bent roof bolt out of mine roof. The employee reached back to drop drill head and moved the joystick for drill rotation. Bent roof bolt spun striking employee in left wrist. Over-exertion in lifting objects

Employee was unloading rock dust from track flat car into uni-hauler. He threw a bag of rock dust into the bed of the uni-hauler and felt pop in left shoulder. PPE was not a factor, mining equipment/system was not a factor, compliance rules/regs was not a factor, miner proficiency was a factor.

Over-exertion in lifting objects

Employee picked up a bag of rock dust put it on his shoulder and twisted back. PPE was not a factor, mining equipment/system was not a factor, compliance rules/regs was not a factor, miner proficiency was a factor. He became a lost time injury 9/16/2012 due to strain of back.

Fall down stairs

EE was cleaning Prep Plant floors and steps, started down a set of steps while cleaning with water, the third step from the bottom had a bend in the step on one side. He slipped on it, fell on left side. PPE was not a factor, mining equipment/system was not a factor, compliance rules/regs was not a factor, miner proficiency was a factor. Became a lost time injury on 9/13 2012.

Employee injured became a lost injury for this accident on 2/1/2013. Employee slipped when he was prying with a bar on brake car and fell both knees struck brake car. Employee was walking with cable during power move, Employee was going to put the end of the cable on a wooden skid. He slipped and fell over wooden skid fracturing his right leg. Employee was helping to build a block wall. The wall did not get wedged in as they were building the wall. The wall fell over as he stumbled backwards he tripped over the block that was used in building the wall, supposedly striking head on ground. Employee was barring over a relay bar when the bar slipped off an came back and landed on his left ankle. PPE was not a factor, mining equipment/system was not a factor, compliance rules/regs was not a factor, miner proficiency was a factor.

Unloading tailgate drive with a Petito mule went to adjust canopy and jack supporting it failed causing the canopy to drop striking his head, neck, and shoulders. Employee was helping to unload the stage loader concave, while unloading it he was standing in the red zone and got pinned when the concave slid on the scoop duck bill and put him against the rib. Alleged incident was to have occurred while this contract labor was walking to the section from the supply car area and claims to have stepped in a tire rut, twisting his ankle. Struck by flying object

Removing drive roller from belt drive, roller fell striking a bed rail that put it in flight which struck employee in jaw cutting face skin and fractured jaw. PPE was not factor, compliance rules/regs was not a factor, mining equipment/system was not a factor, miner knowledge was a factor.

Alleged injury was to have occurred while this contract labor was lifting waterline and he attempted to turn while he was bent over, exerting pressure on his back. Cause of this injury was not proper lifting techniques and enough help. While lifting a belt roller to load in a uni-hauler, turned felt pain or a pop in lower back. He worked his next two scheduled days when scheduled to come back to work on the May 12th he called stating he could not work because of the injury. Mining a sixty to the right, while cutting the roof, the miner bits caught a bacon strap taking out a couple roof bolts allowing the bacon strap to be thrown back towards the miner operator striking him at the bridge of the nose and above the eye. Alleged injury was to have occurred while this contract labor was building a block wall while standing on a ladder, the wall started to fall over, knocking him off the ladder. Cause of injury was not securing wall with wedges as being built. Installing track and had track jacked up with a track jack, while they were pounding clip ties, rail slipped off jack striking legs. PPE was not a factor, mining equipment/system was not a factor, compliance rules/regs was a factor, miner proficiency was a factor.

Alleged injury was to have occurred while this contract labor was placing a bag of rock dust on the mine floor, felt pain in his back and a burning sensation. Unloading block from scoop bucket, threw one out of bucket felt sharp pain in right shoulder. PPE not a factor, compliance rules/regs not a factor, mining equipment/system was not a factor, miner concentration was a factor.

Over-exertion in lifting objects

Employee was unloading a distribution box off of a utility belt ride by himself, felt pain in groin area. Cause is improper lifting procedures and not getting assistance, PPE was not a factor, mining equipment/system was not a factor, miner proficiency was a factor. This person worked after the incident and now is off work for the injury as of 3/17/2012.

Employee was roof bolting in face, went to install second drill steel to finish drilling hole, rock fell striking index finger against drill pot, cutting finger. Employee stated he was holding a shuttle car cable to cut the outer jacket off when the knife slipped and cut his left ring and small fingers.

Cause of this injury was lack of communication with other workers in area. Foreman was measuring the tip height on No. 132 shield, when a piece of roof coal fell between the shield tip and roof bolt approximately six feet long, 3 feet wide and 13 inches thick. During part 50 audit, it was determined this injury was reportable. Employee was removing a hose from inside shield, used hammer to break loose fitting, hit thumb instead of fitting. Cause of this injury was inattention. Compliance with rules/regs. mining equip/systems not a factor, PPE and miner proficiency may be a factor.

During part 50 audit, it was determined this injury as reportable. Employee was reaching for the winch cable when co-worker reversed lever on scoop pinching his hand between the bucket and ram. Cause of this injury was poor communications. Compliance with rules a factor, PPE a factor, and miner proficiency. Mining/equipment systems not a factor.

While hanging high voltage cable on wire screen a rock fell down and hit upper lip cutting lip. PPE was not a factor, compliance rules/regs was not a factor, mining equip/system was not a factor, miner proficiency was a factor.

The employee was underneath piece of equipment performing maintenance. As he was removing the belly pan underneath the cribbing, he experienced an intense pain in his lower back. On 11/2/2012, the MRI confirmed a lower lumbar strain of the back.

Employee was driving a golf cart along side of a parked track shield car. Left front tire caught part of the track shield car which spun steering wheel on golf cart catching little finger and dislocated the finger. During part 50 audit, it was determined that this injury was reportable. Employee was struck by a piece of rock that fell between bolts at the stage loader area. Cause of this injury was not pulling down loose top. Compliance with rules/regs. and miner proficiency could be a factor, mining equip/systems, PPE was not a factor.

During part 50 audit, this injury was determined reportable. Employee was changing cutter shaft on shearer, did not relieve hyd. pressure, hot fluid (2nd degree burns) sprayed onto his face/neck. Cause was not relieving PSI. Compliance with rules and miner proficiency a factor, PPE and mining equip/systems not a factor.

Employee was loading roof bolts on the roof bolter machine and cut his arm on the corner of a roof bolt plate. PPE was not a factor, mining equipment/system was not a factor, compliance rules/regs was not a factor, miner proficiency was a factor.

Employee was walking toward section power center slipped and fell on the ground. When he fell on the ground the employee landed on an old piece of a razor knife cutting his left hand. Alleged incident was to have occurred while this contract labor was setting a post, a piece of rock fell striking him alongside of his head. When he got ready to strike the pin the hammer slipped out of his hand striking the victim in face breaking his two front teeth. Employee was crossing a make shift x-over out of 5x7"" posts, a pile of coal came along and knocked the posts out from under him falling onto the belt. Cause was unsecured x-over. Absorption of radiations, caustics, toxic and noxious substances

Alleged injury was to have occurred while this contract labor was working in water at bottom of an airshaft. He noticed skin burns around calf area of both legs (cement). Cause of this injury was lack of knowledge of what cement can do to skin.
