Accident type, without injuries

A roof fall occurred in 1 west bleeder at 84xc. Was approx. 8' H, 18' W and 25' L. Area was bolted with 8' point anchor resin assisted bolts, 12' CC cable bolts in pattern along with roof screen and bacon straps. Fall did not obstruct vent. or escapeways. Cause of fall was deterioration due to weathering. Floor/Roof support will be set as reported to prevent future occurrences.

Contact with electrical current

Alleged incident-EE knocked #9cm breaker, was removing the cathead and received an electrical shock while their neck was against the cable. Incident is still under investigation. Cause is unknown at this time. Compliance with rules/regs at this time is not factor, incident involved equipment, job skills not a factor, miner proficiency may be a factor, PPE not a factor at this time.

Over-exertion (Not Elsewhere Classified)

While walking thru last open x-cut on at the longwall employee stepped in a hole twisting his right knee, employee began missing work on 12/17/15 as a result of this injury. PPE was not a factor, compliance rules/regs was not a factor, mining equipment/mining systems was not a factor, miner proficiency was a factor.

Fall from ladders

Employee was climbing down a ladder to unhook ladder when he lost his footing causing a fracture to his lower leg. Cause of the injury was miner not paying attention to the task at hand. PPE was not a factor, compliance rules/regs was not a factor, mining equipment/systems was not a factor, miner proficiency may have been a factor.

Injured employee was carrying 8"" pipe with coworker when the coworker stopped and backed up causing the pipe to strike him in the face causing contusions.

Fall onto or against objects

Grating measuring 20""x42"" had been removed from floor to perform scheduled maintenance,injured employee was walking back to area to perform work,he stepped into hole and struck his ribs against grating. Cause of injury was employee's lack of communication. PPE was not a factor,mining equipment/systems was not a factor,compliance rules/regs was a factor,miner profiency was a factor.

Employee was using a drill when the drill bit caught and spun the drill out of his hand striking the back of his right hand. Cause of incident was employee lacking to hold onto drill. While drilling a 12' hole for a torque bolt employee was struck in the left ring finger by drill steel. Cause of the injury was employee not holding drill steel in the top when adding additional steel. While doing rehab work employee was running miner when a rock was thrown 5' past miner striking victim in the left foot. Cause of the accident was 8' bolt caught bit lug and threw rock rearward. PPE was not a factor, mining equipment/systems was a factor, compliance rules/regs was not a factor, miner proficiency may have been a factor.

Injured employee stated he was moving out of the way of a scoop car when he tripped on some blocks causing him to fall and strain his groin.

Employee was getting out of a scoop on south A track. The employee slipped and fell catching himself with his left hand resulting in a fractured wrist. The cause of the accident was the employee's lack of awareness to his surroundings. PPE was not a factor, mining systems may have been a factor, compliance rules/regs was not a factor, miner proficiency may have been a factor.

Employee was lifting feeder cable to be hung after feeder was advanced in preparation for a belt move. Cause of accident was a small cut in cable and employee had damp gloves on. PPE may have been a factor, compliance rules/regs was a factor, mining equipment/systems was a factor, miner awareness was a factor.

Absorption of radiations, caustics, toxic and noxious substances

Employee was hand spreading potassium permanganate onto pond for treatment when he slipped and fell on rocks getting some of this potassium permanganate into his eyes. EE seeked medical attention and received eye drops. PPE was not a factor, Compliance with rules/regulations was not a factor, Mining equipment/mining systems was not a factor, Miner proficiency was a factor.

Employee was handling belt stand during belt and power move when a bottom roller fell over striking his hand. Employee's lack of attention was the cause of this incident. PPE was not a factor, mining equipment/systems was not a factor, compliance rules/regs was not a factor, miner proficiency was a factor.

Employee was using a bench grinder when his thumb came into contact with the grinding wheel. Employee's lack of awareness was the cause of the incident. PPE was not a factor, mining equipment/systems was not a factor, compliance rules/regs was not a factor, miner proficiency was a factor.

Employee was assisting another employee while changing a vertical pin when a piece of metal broke free striking him in the wrist. Body placement was the cause of the incident. EE was assisting in the unloading bolter supplies when came into contact with a metal banding strap. EE seeked medical attention next day due to the appearance of infection. Employee pulled golf cart into x-cut and came into contact with a bacon strap resulting in a laceration to his forehead. Cause of injury was not paying attention to surroundings. Employee was installing bolt next to rib. Vibration of drill caused rock to separate and fall striking left lower arm resulting in a small laceration. Cause of the injury was body placement. While entering the long wall face from over the tailgate sail backwards. He turned around, a piece of screen hanging down between shields caught employees glasses and scratched left eye. Cause of accident was screen between two shields. PPE may have been a factor, compliance rules/regs was a factor, mining equipment/systems was not a factor, miner awarness was a factor.

Employee was building a wall and dropped a block. The block struck the end of a shovel causing the handle to strike employee in the right ear resulting in a laceration. Employee was lowering jack, in the process of re-railing a mantrip. Employee failed to keep firm grip on jack bar as instructed. Resulting in jack bar coming out of jack striking employee behind right ear resulting in laceration. Employee was checking sensor at a pond site when he slipped and fell, attempting to catch himself he injured his shoulder. Employee began missing work on 6/29/15 as a result of his injury. Lack of awareness was the cause of the accident. PPE was not a factor, compliance rules/regs was not a factor, mining equipment was not a factor, miner proficiency was a factor.
