Accident type, without injuries

A new roof fall occurred on the South Mains ""B"" beltline between 28 and 29 x-cut. The fall was approx. 8 to 9' high by 18' wide and 60' long. Area was bolted with 8' Install III bolts and resin, H-beamed with 4"" beams sitting on cribs. Area was cleaned up and re-supported. Fall did not obstruct ventilation or escapeways. Cause of this fall was weather deterioration.

Accident type, without injuries

A roof fall occurred on the South Mains ""A"" beltline at 69 x-cut. The fall is approx. 40 feet long by 18' wide and 9' high. Area was bolted with 8' Install 3 bolts and resioned. Other support that were installed were 4"" H-beams and cribs. Cause of this fall was deterioration of the main roof due to weathering.

Accident type, without injuries

A Taylor forklift parked by the hoist house drifted forward and ran into the hoist building knocking out the power to run the slope hoist. Equipment sat there for 1-1/2 hours before moving. Cause of this incident was not blocking wheels when vehicle is on a downhill slope and possibility of not setting the parking brake. Hoist was back into service around 4 pm.

Accident type, without injuries

A inactive hole (gas or oil) was intersected on the midnight LW shift at 21 Shield of the 12 East longwall section. No accumulation of methane was found but was liberating 2.6% over the hole. The hole was opened up by using non-sparking tools and plugged. THE OPERATOR IS FILING THIS REPORT IN PROTEST AS MSHA IS SAYING THERE WAS AN INUNDATION OF GAS. NO ACCUMULATION WAS FOUND.

Struck by falling object

Roof bolter was installing the second bolt of his first row when the roof inby the ATRS beam fell without warning. A piece of rock flew up striking his leg above the knee. Cause of this injury was slickenside material falling without warning. Employee was using a rockbar to slide off a shuttle car tire that was on a supply car. When the tire was at the edge of the car it moved suddenly and the bar struck him on the side of his face. Alledged incident was to have occurred while this employee was examining belts. He claims to have slipped in mud straining his knee. This incident was to have occurred while this employee was waiting for the shuttle car to pass him. He was standing in a x-cut and did not notice that the snub was near him and he got flipped by the cable snub. Cause of this injury was safety awarness. Examiner was coming down an aircourse and a rock was in the middle, he went to kick it out of the way and something popped in his right calf of his leg. Cause of this injury was safety awareness. Employee was moving the roof bolter and he went to get out of the cab, his SCSR hit the left tram lever and his left leg was out on the mine floor and the cab drug his foot under the edge of the cab.Cause of this injury was not shutting off machine. Employee was knocking a stuck cement block from the mine roof and he did not observe loose overhanging brow that came down striking him on his head and neck area. This questionable incident was to have occurred while this employee was shoveling a coal spill at the LW tail. He claims to had his foot slip off the shovel hyperextending his knee. Roof bolter was reaching across machine to untie wire that was holding roof bolts together when a piece of roof coal fell striking his arm. Cause of this injury was safety awareness. Over-exertion in lifting objects

Employee claimed to have hurt his back while lifting a 5x7 timber to build a 9-point crib. Cause of this injury was unsafe lifting habits. Employee was attempting to get the second drill steel extension into the drill chuck. The drill steel kicked back as he was raising the drill head and hit his wrist. Riding a mantrip into the mine the mantrip jumped track. He hit his shoulder against the inside compartment of the manbus injuring himself. Carrying pc of schduel 80 pipe with Case hoe. It started to swing. He was afraid it was going to hit the window. He stuck his hand out to catch it & caught his hand between window and pipe.

Employee was riding out of the mine on a mantrip and a small piece of rock fell from the top striking his fingers. Cause of this injury was not having a top over the seating compartments. Employee was using a torch to cut off a bit lug on a stamler feeder. The oxygen hose blew apart and caught his shirt on fire. He was burnt on his side of his chest. Roof bolter was attempting to put a roof bolt plate over a bolt and it slipped from his grip and the plate fell cutting his arm. Cause of this injury was safety awareness and being in a hurry. Employee was helping to change damaged bit inserts on a miner drum. When he hit the top of the bit insert a piece of slag flew striking him in the neck area. Supervisor and another co-worker were carrying a piece of metal. His co-worker dropped his end and caught the victims finger between the metal and the rib. Alledged incident was to have occurred while this employee was using a plasma cutter. He first though he got flash burned but later a piece of metal was found in his eye and removed.
