Accident type, without injuries

On 10/9/06 at 11:50 in the am the Rodeo hoist went down due to a power loss. The electrical department was notified and began trouble shooting the system. During the investigation of the power failure it lead the group to discover that the transformer feeding this system had failed.

A lead person was jamming an undercut rib for rehabilitation efforts in a secondary stope, he noticed trickling of rock from the right rib and backed out of the area. A ground fall of 25 feet long by 17 feet height by 6 feet wide came down from the sill to the middle of the arch.

There was a ground fall at the 3920 intersection. A slab approximately 3'x15' blocked the main ramp at the 3920. An electrical cable fell over the bucket of a loader (mucker). All personnel on Rodeo side from the 4100 to the 3620 and on Meikle side 8040 haulage to 1600 were evacuated.

A ground fall occurred at teh 4150-5740 heading. The fall of ground was approximately 22 feet high and 15 feet wide and buried the front deck of a MacLean bolter. The bolter operator had completed installing 3 rows of 12 feet roof bolts in the secondary heading and was in the cab to back the machine out when the rock fall occurred. The operator was not injured in this incident.

Fell approx. 15-20' to the ground from cab of overturned haul truck. EE comments to EMT's indicated he experienced a physical episode affecting his consciousness or orientation prior to event. Incident under investigation.6 broken ribs, punctured lungs, 2 compression fractures of L1/L4, strain to right arm, lacerations to face/head, and damaged tailbone.

Employee was carrying drill foot clamp up a set of stairs, lost his balance, tried to recover and subsequently experienced a hernia. Treatment was not rendered until 1/2/2007.*Surgery to Repair*

Employee was walking on slippery surface, rolled his ankle and felt intense pain. X rays were negative for fracture, employee was given a brace and released to full duty, however was unable to return to work for two shifts due to pain in ankle.

Emp doing a preop inspection of truck and hit lower right leg on piece of timber. Reported on 5 point card, did not seek medical treatment. Wound got infected and employee sought treatment and was admitted to hospital for IV antibiotics.

Employee was loading damaged 2 inch pipe onto a scissor lift truck. He was loading the first pipe and had his left little finger over the end of the pipe. The employee looking back towards the pipe and miss-judged the back of the truck. His finger was crushed between the pipe and the back of the truck. The severity of injury required immediate surgery to repair the finger.

Employee was riding on hot change bus. Bus hit a hole and threw the employee around in the seat. He was transported to NNRH ER and admitted to the hospital after a series of episodes involving his heart and blood pressure. He was diagnosed with back strain, hip strain, and cardiac bruising.

Injured employee walking up the walkway to #1 east neutralization. Called supervor. Another employee was walking beside injured employee when the accident occurred - other employee said injured employee tripped but did not fall and after that injured employee said he felt he had just broken his ankle.

Employee was pulling toolbox around truck to begin work, heard and felt a pop in his left shoulder and felt immediate pain. Placed on restricted duty pending MRI. Diagnosed with tear to rotator cuff, biceps tendon, labrum.

Employee was removing/installing a grid motor cover. The lid got stuck while removing/re-adjusting. The employee pulled on the lid and felt pull in shoulder.

Fall to the walkway or working surface

Employee was getting off piece of equipment, slipped and fell between two tires, landing on left elbow and hip. Diagnosed with bruised elbow, neck, shoulder, and hip, left side. Released in a sling, worked 2 shifts restricted duty.

Employee stepped in a hole on deck of bolter and twisted his left knee and ankle. He was originally released to full duty, but was unable to perform all the essential functions of his job, so was placed on restricted duty on 12/08/06.

Employee was walking around the dig face, stepped on a rock and bruised his foot. X rays were negative for fracture, employee had MRI on 11/21/06. Employee was placed in a walking boot and on restricted duty pending the outcome of the MRI.

Emp was working with another employee pulling cable. As the coworker was backing up the scissor deck, the injured employee was pulling cable onto the deck. He felt a pull and ripping sensation in his groin area, and immediate pain. He was diagnosed wtih a groin strain, and upon follow up with a physician, was placed on restricted duty.

The claimant was loading a paint can onto a work truck when he lacerated his left middle finger on a sharp edge of the paint can.

A Process Autoclave Operations employee was walking up the stairs at the mill crusher when he struck his hand against the metal handrail. He was seen at NNRH and diagnosed with a bone chip in the left middle finger. His hand was splinted and he was placed on restricted duty.

Employee was changing cutting edges on a motor grader when his fingers got caught between two cutting edges, smashing the right index finger. Xrays were negative for fracture and the laceration was closed with 12 stitches. The employee was released to full duty.

While cleaning mud containment, vacuum hose became plugged. When material dislodge from hose, the force threw nozzle into employee's face cutting his cheek.

Employee was using a hammer to put a pin in place and a piece of the hammer broke off, embedding in the employee's left cheek. Metal left in cheek as efforts to remove were unsuccessful due to the depth of embedment.

Employee was assisting with instaling a noise shield on bolter #KMB001 and was adjusting the up right. He smashed his left index finger by using an 8 lb. hammer when the handle of hammer came in contact with the upright of the window frame. He cut and fractured his left index finger, and the laceration required stitches.

Employee pinched finger between support beam and 24"" valve.

Received a call at 4:05am, an underground technician working at 4100 Rodeo smashed his right index finger while pushing on the rear door of a scissor truck to lift a ventilation fan that was already attached by hangers. The door broke free, his hand went between the rear door and the fan, lacerating the finger. Received 12 stitches to close the wound, released to full duty.

EE lifted an oil cooler off his bumper, carried it to a container so he could drain it and bumped into the container with the oil cooler. The oil cooler has sharp edges and bumping into the container caused his hand to shift and resulted in his left pinky finger being cut.

The employee was bent over at the waist replacing and O-ring in a fitting and felt a sharp ""lighting bolt"" like pain in his lower back, and he fell to the ground. He was transported by company ambulance to NNRH ER where he was diagnosed with a muscle strain and released to full duty.

Employee was making up bolts for a Mclean bolter and the deck was moved up catching his finger in the pinch point of a split set bolt. Causing a laceration.

Employee was reaching into a parts barrel to get a ball valve. In the process, he ruptured an aerosol can which lacerated the web between his right thumb and index finger. The cut required 7 stitches. He was released to full duty, but required to wear a brace.

DRIVING TRACTOR TO MINE SITE IN HEAVY & GUSTY WIND. A GUST OF WIND CAUGHT TRACTOR HOOD & FORCED IT TO BECOME UNLATCHED. THE HOOD STRUCK THE EMPLOYEE IN THE HEAD. MINE APPLIED FIRST AID & THEN HE WAS TRANSPORTED TO THE E.R.

While lifting part of gas shop hoist onto forklift, part slipped, pinching left pinky finger between part on forks and part on ground. Stitches required to close laceration on pinky finger. Released to full duty, no restrictions.

Fall onto or against objects

EE working on top of batch plant. EE was fixing to climb down off of the top of the plant. EE was walking over to get on the ladder and tripped and fell head first down inside the ladder cage, striking right side of face on cage built around ladder.
