During the commissioning and start up of a new burner management system on the Autoclave Phase III 100K steam boiler, an un anticipated ignition of natural gas occurred in the forced draft induction inlet causing damage to the ducting.
Four blasting personnel were exiting the open pit when the driver noticed smoke from under the hood. The truck was stopped and chocked and efforts begun to extinguish fire. MSHA was notified and fire was extinguished.
Employee placed arms in path of cargo lift wires.
Ee was coming down ladder on equipment. Went to reach down to next rung and heard pop in shoulder, felt immediate pain. Seen at the ER and referred to orthopedist. Orthopedist is requesting diagnostic testing for cartilage tear. Released to restricted duty that cannot be accommodated.
Ee used his own body to try to push bent handrails back into place. He braced his back against a rail and pushed hard with his feet against the bent handrails. After working remainder of shift, went home, slept, and woke with pain the next day. Seen at clinic, diagnosed with upper back strain, released to restricte duty.
While working on cleaning a stope, Ee felt he was exposed to some kind of gas and left the work area. Taken to ER, diagnosed with exposure to gas, given oxygen and released to full duty. Ee followed up on 09/26/13 and was diagnosed with an occupational illness, chemical pneumonia, resulting from the exposure.
While changing drill steel, the employee's right hand was caught in the drill steel. Taken to ER where he was diagnosed with dislocation of the 3rd, 4th, and 5th fingers and lacerations to the 3rd and 4th fingers. Stitches required to close the lacerations. Released to restricted duty.
When stepping off his dozer, employee felt a pop sensation in his left calf. He was unable to bear weight on the left side. He was diagnosed with a torn left calf muscle and released to restricted duty with crutches and no weight bearing on the left leg. The restrictions were not able to be accommodated.
The miner was pushing the man bucket away from the steel work deck and got his left pinky finger smashed between the bucket and the crosshead steel framework.
Employee slipped on ice and fell to ground. Sought treatment for her low back pain on 3/7/2013. Diagnosed with a low back strain and put off work for physical therapy.
Driver was on 3rd trip to pit, on 1st trip a break test was done, he was still in high gear and was unable to slow or stop the truck and ran into a pile of agg ore next to the underground mine wall.
Ee was walking through paste when he felt left knee pop, causing the knee to swell. He reported the issue, tried self-care, and sought medical attention on 11/06 at the clinic. Diagnosed with a knee strain and placed on restricted duty.
Ee getting into back seats of van. Started to lose balance, and braced his right leg to avoid falling. He felt pain. He reported to his supervisor on 08/07 and when the pain did not get better he asked to see a doctor. He was seen on 09/06 and referred to an orthopedist. On 09/12 he was diagnosed with a fracture to the tibial plateau and placed on restricted duty.
Employee was putting chock block back in the rack on the 966 loader and felt a pop in his right wrist. Seen at the ER and released to full duty, follow up with an ortho. Seen at the ortho, put on restricted duty and told to attend physical therapy.
Ee parking boom truck. Set brake and got off to set jacks. He slipped and thinks he released the brake causing the boom truck to move forward, pinning his lower left leg between two boom trucks. He was able to move the boom truck backwards and free his leg. Seen at the ER, no fracture, released to restricted duty with crutches and told to attend physical therapy.
Employee was performing pre-op and slipped on step of an AD30 and hit his elbow on step. Employee did not want medical treatment; said it was just sore. On Dec.17,2013 employee requested to go to doctor because elbow had swollen up during his days off. Diagnosis: Olecranon Bursitis and Right Lateral Epicondylitis. Employee on restricted duty.
Ee was lifting a heavy object and felt a twinge. He had been lifting and moving ramps and on the last ramp, felt the twinge. Seen at the clinic, released to restricted duty.
EE was pulling fuel hose to fill tank, she slipped on a step, causing her to fall, catching herself with her left hand and she felt immediate pain in her left wrist. Seen at the ER, x-rays negative for fracture, she was released to restricted duty.
Ee walking up stairs to lock out equipment, boot caught and he fell into next step, striking right knee. Seen at orthopedist and put on restricted duty.
Employee was using a RAD gun to remove nuts from a tire. His finger was caught between the wheel motor and the RAD gun. Seen at the ER, diagnosed with a fracture to the fingertip, and sutures required to close the wound at the tip of the finger. Released to restricted duty.
Employee was using a fire hose to clean in the tunnels. He tripped over the hose that was in the walkway and fell, catching himself on his braced left arm, jarring his left shoulder. Released to restricted duty and told to attend physical therapy.
While inspecting slings and comealongs, one of them broke, striking the employee in the face, hitting his safety glasses, resulting in a cut above the employee's eyes. He was seen at the ER where stitches were required to close two lacerations. Released to restricted duty until 02/21/2013 when he could return to full duty.
Employee was walking across the Barrick parking lot, slipped on ice and injured his right knee.He did not request medical treatment at that time, thought it was just bruised. Seeked medical attention 4 days later, had MRI, results tear of meniscus, wear knee support, restricted duty and referral to Orthopedic Doctor.
Employee was walking toward office when he slipped on the ice and landed on his back. He reported to his supervisor and did not immediately seek treatment. On 01/07/2013 he requested to be seen by a doctor, was seen at the clinic and released to restricted duty.
EE was fixing a blow down. While working on it, a piece of metal cut his right thumb. He was seen at the clinic and stitches were required to close the wound. Released to full duty.
Ee was washing service truck at end of shift. He was holding the wand in the right hand and when he squeezed the trigger it jumped, hitting him on the right cheek. He was referred to the eye doctor who diagnosed bruising to the right cheek and eye. He was given a prescription eye drop and released to full duty on his next scheduled shift.
Employee was installing a house roller. The roller was resting on their left leg and it slipped off and fell onto the employee's left hand between the roller and the track. It was classified a first aid, however the employee still felt pain in the left middle finger on 10/24/13, they were taken to the ER were they were diagnosed with a broken middle finger.
Employee moved a pump motor that was not fully secured on a pallet. As he sat the pump motor down, his finger was pinched between the pallet and the pump motor.
Employee was using a grinder when it ""jumped"" and hit the injured worker on the front of his left leg causing a laceration. He was seen at the ER, where the wound was examined and sutures were required. Released to full duty.
Employee doing clean up prior to drilling had rock strike the top of his foot. He asked to see a doctor on 07/29/13, was seen at the clinic and diagnosed with a fracture and referred to specialist. Follow up with specialist resulted in a full duty return to work, no restrictions.
Employees were installing man door frames on the third floor of the water treatment building. While handing a welded frame clip to another employee, EE received a laceration on his arm from the door clip straps which were protruding from the door jamb.
EE was struck in the face w/plastic bucket while removing bucket from vessel at end of shift. Bucket slipped out of his grip while elevated above his head & hit him in the nose & lip. He initially thought he had a bloody nose so he exited vessel & put ice on his nose, then returned to work. After returning to the hotel room he noticed pain in his upper lip & discovered laceration
Employee was repositioning angle iron into proper order for erection the following week. In the process of moving one piece, the angle iron rolled catching the employees finger between that piece and another piece causing the finger avulsion. Employee was wearing proper PPE and attended Pre-Task Analysis meeting prior to performing task.
Employee was using a hammer and a piece of metal struck him in the left arm. He was seen at the ER and the metal was removed; stitches required to close laceration. Employee released to full duty.
Employee was not aware he was close enough to cut his knee on the sheeting materials he was walking by. Clear weather
After welding an item, the employee blew dust off the weld and debris got into his right eye. Taken to ER where a small particle was removed from his eye and eye drops were prescribed. Reportable for treatment, eye drops.
Employee was underneath equipment greasing drive lines on a creeper. He rolled out from under truck and hit his head on the guide for the garage door. He was taken into the hospital and received stitches.
Employee was cutting a lemon with his retractable knife. Went to put the knife back on his belt and the blade was stuck open. He cut his left middle finger. Was seen at the ER where stitches were required to close the wound. Released to full duty.

While grading ramp, ee had a strange smell come in to her grader, making her sick and start to cough, unable to catch her breath. The EMTs evaluated her, put her on oxygen and transported to the ER. Diagnosed with pneumonic and kept overnight for observation. Released to full duty on next scheduled shift.
